These long-term relationships have afforded us many opportunities to meet our clients' needs and perform municipal engineering services. Our team has extensive experience in working under continuing contracts and is available for planned projects as well as unplanned emergencies. Work included paving of approximately 4,350 lineal feet of dirt roads, excavation/grading of roadside swales and relocations of roadside improvements to accommodate the construction of this project. Winter Springs East Lift Station Analysis - This project was performed to determine the best way to divert the flow from 5 lift stations from the gravity system where it experienced long detention times using a WaterCAD model. Various runs of the model were performed to optimize routing. Magnolia Square - CPH Services included the design of the streetscape, fountain, hardscape, furniture selection, landscape, irrigation and CEI services. The fountain was developed as a focal element from both Tuskawilla and Blumberg Avenue. Pavers were used to identify pedestrian crossings and different patterns were selected for visual interest along the sidewalks and around the fountain. Town Center Trail - The Winter Springs Town Center Trail is a pedestrian trail that will connect the cross State Trail with the Winter Springs Town Center. Some of the most recent projects for the County include: Southwest Wastewater Treatment Plant- CPH provided planning, design (including environmental, civil, structural, electrical, etc.), permitting (construction and operating), and services during construction for a .5 MGD wastewater treatment plant. The plant was designed for future expansion to 2 MGD. Initial effluent disposal was accomplished through use of on-site percolation ponds. The plant was designed to meet Class I Reliability Criteria. The design includes: Headworks, Biological treatment system, Clarification, Chlorination, Sludge handling, Effluent pump station, Operations building, and Percolation ponds. Tenth Street - This task consisted of reconstructing 1.2 miles of an existing 2-lane rural road to a 4-lane divided urban road. The proposed cross-section included four 12-ft lanes with 4-ft bike lanes in both directions. CPH also assisted Vol usia County by preparing Right of Way Maps, and legal sketches and descriptions for the additional right of way and pond sites. The Maintenance of Traffic was also a critical element to maintain and ensure full access to two schools, by both vehicles and pedestrians, during construction. ~ Rhode Island Avenue- Task consisted of designing 1.4 miles of a new 2-lane divided rural road with a raised curb median, and re-design of an existing signalized intersection at US 17-92. The design included a 2-lane cross section that can be widened in the future to a 4-lane urban road with minimal construction. The stormwater management system was designed to collect and treat runoff from the current 2-lane rural road, but also has the capacity and the infrastructure in place to handle the future widening utilizing the same storm water pipe/treatment system. Spring Hill Park/Trail - Engineering and Landscape Architectural services for the expansion of Spring Hill Park. CPH provided preliminary engineering and site planning along with construction estimates. Assistance was provided to the client in public presentations and community meetings. The final product consisted of phased construction plans with final engineering, a basketball court, restroom facility, tot lot, and pavilion. The existing walking trail was expanded to encompass the additional park area. Park landscaping was improved to provide shade to trails and vegetation screening at parking areas. Lake Monroe Trail - CPH prepared the design for a 12' wide multi-purpose trail approximately 2.4 miles in length which winds through wetlands, wooded uplands and open pasture land. Some of the most recent projects for the City include: 1st Streetscape/Gateway Improvements - CPH is currently contracted with the City of Sanford to provide a streetscape design package for a 6 block section of Historic First Street. The intent of this project is to closely match the streetscape of improvements previously completed on First Street. Close coordination with City Staff involved in adjacent projects is of utmost importance as we provide seamless transition from one phase to another. Plans and services include the conceptual and final design of a Gateway feature introducing the historic district. CPH conducted design charettes to ensure input and direction from City officials and other interested parties. CPH is currently providing final design services for the final package that will include: Sidewalk and street curb layout, Planting and paver design, Parking layout, Irrigation plans, Lighting and electrical plan, Site furniture selection and layout, Streetscape details, and Gateway entrance architectural design details. Sanford South Water Resource Center - This included the design and construction of the new Sanford South Water Resource Center.The Sanford South uses energy efficient treatment processes and cutting-edge technologies to produce a public access quality effluent. Reclaimed water is distributed throughout the City of Sanford service area consisting of 150 miles of distribution piping. Sanford South construction includes the following: Headworksstructure with odor control system, Energy efficient oxidation ditch biological treatment systern, Office/laboratory building, Tertiavy filtration of the secondary effluent, Sodium hypochlorite storage and handling facilities, Chlorine contact chamber, Effluent transfer pump station, Solids handling facility and Class A sludge treatment system, Reclaimed water distribution pump station, City-wide SCADA control systern, 3 MG reclairned water ground storage tank, 2 MG reject water ground storage tank, Internal roadways, Fencing, Landscaping, Yard piping, and Storm water management system St. Johns Parkway (Phases I, II & III) - CPH provided services for Phases I and II of the St. Johns Parkway project consisting of widening existing Jewett Lane (renamed St. Johns Parkway) from the Smith Canal to approximately 400-feet west of Meisch Road, resulting in 1,650 linear feet of new three-lane road. Major project components included: 28-foot span (7-Ioot rise by 72-foot long) concrete arch culvert, Wingwalls at the Canal, Curb and Gutter, Sidewalk, Guardrail, Aluminum Handrail, Storm Sewer, and Utilities construction of approximately 1,750 linear feet of new 12-inch water main and 1,900 linear feet of new 8-inch reclaimed water rnain. Phase III of the St. Johns Parkway project additionally included construction of approximately 2,800 linear feet of new four-lane divided roadway between Rinehart Road and Upsala Road. City of Sanford - Bio-Solid Gasification System- CPH is working with Max West Environmental Systems Inc. to provide a Design, Build, Operate, Finance project that will utilize the Cities current biosolids (sludge) to generate heat to operate their existing sludge driers. This unique system uses Sanford's wastewater treatment plant's biosolids residuals to provide a green energy solution for the City. The Max West system will provide heat through a gasification process that will eliminate the current use of natural gas to provide heat to the driers. 25. Some of the most recent projects for the County include: S.R. 50 Utility Relocation Project - This project provides improvements to over 13 miles of the water distribution system and wastewater collection and transmission system within the S.R. 50 Corridor, one of the busiest roads in Central Florida - which is being expanded from 4-lanes to 6-Janes. The utility improvements include building new facilities as well as the removal of facilities in conflict with the FOOT roadway design. The utility improvements will be constructed as part of the construction of the FOOT S.R. 50 projects under a Joint Agreement between Orange County and FOOT. The construction documents meet FOOT plan preparation standards, FOOT roadway design schedule and are coordinated with the FOOT road design and other utilities. The project includes the installation of approximately 5.5 miles of 8,12,16,20 and 24-inch diameter water main and 7.7 miles of 4,8,12,16,20 and 30-inch diameter FM along S.R. 50 from west S.R. 436 to Old Cheney Road (the Project). Connection of existing services and lateral mains will be required while maintaining service to customers. S.R. 528 Utility Relocation Project - This project was initiated by Orange County Utilities (OCU) to relocate a 16" forcemain as a result of the bridge widening of SR 528 from 1-4 to the Airport Toll Plaza. The widening of the SR 528 had the potential to impact lines owned and maintained by Orange County in Multiple locations. The challenges of this project included replacing the 16" forcemain with a new 30" forcemain within the John Young Parkway right-of-way under the existing SR 528 bridge, having minimal disruption to the roadway and still maintain operation to the existing forcemain. The project included two 54" jack and bores of 280 feet and 60 ft each, three 16" lines stops, two large thrust blocks, and 1,500 feet of 30" forcemain. The construction documents meet FOOT and Turnpike plan preparation standards, FOOT roadway design schedule, and were coordinated with the FOOT road design and other utilities. Lake Standish Heights Water System Improvements - The Project consisted of the design of approximately 3600 LF of 6" and 8" water main, fittings, valves and appurtenances to replace an existing substandard water distribution system. CPH prepared a Preliminary Design Memorandum that outlined the pipeline alignment, permitting requirements, presentation of known future plans of other utilities and agencies for the project area and presented conclusions and recommendations for the execution of the project. CPH also provided construction drawings and technical specifications for the project prepared the appropriate applications and obtained the water permit from FOEP and a utility permit from FOOT. CPH also performed limited inspection services, shop drawing review, post design services, attendance at progress meetings, as-built drawings, clearance letter, certification of completion of construction, substantial completion and final inspections. 25. Some of the most recent projects for the City include: Oviedo Water Treatment Plant - CPH provided design and construction services for a new well for the City of Oviedo. The project was an implementation of a well which had been planned as a replacement for an existing well that had water quality degradation. The new well was a larger diameter and shallower well in accordance with recommendations from the groundwater hydro-geologist employed by the City. Reclaimed Water Main Phase I and II - Phase I -The project consisted of engineering design and construction services for 2000 lineal feet of 20-inch and 7000 lineal feet of 24-inch reclaimed water main on Lockwood Boulevard from Mitchell Hammock to Seminole Creek Drive, all within public rights-of-way. This reclaimed water system was started in 1992 as the City needed an alternate source of water. The City does not have a readily available source of reclaimed water. CPH Engineers developed Reuse Feasibility Plan for the city and obtained funding from FDEP SRF sources. The planning function was part of the overall needs to obtain funding. The program is a joint effort with the City of Orlando (Iron Bridge Facility) and Seminole County. Phase 11- - The project consisted of engineering design and construction services for 6600 LF of 16-inch water main, 800 LF of 1 O-inch water main, 9780 LF of 24-inch reclaimed water main and 2900 LF of 24-inch directional bored reclaimed water main on Old Lockwood Road from McCulloch Road to Lockwood Boulevard. This work was completely within public rights of way. Following the completion of the Phase I portion of the project, the City of Oviedo proceeded with Phase II to connect to the City of Orlando main. Seminole County also desired to install a water main in the same area and therefore, all of the coordination and layout for both lines was performed by CPH. CPH provided drawings and technical specifications for the construction of this project and associated items. Plan and profile drawings were provided based on topographical surveys. The alignment followed along existing roadway rights-of-way within Seminole County and was within a developed urbanized area with roadway conflicts and traffic maintenance issues. S.R. 426 Utility Relocation Project - This project is being constructed in conjunction with the FDOT widening project of 1.2 miles of S.R. 426 and will be constructed as a Joint Project Agreement (JPA). The project includes the relocation of the City's 10" WM along S.R. 426 that will be in conflict with FDOT's improvements. The existing 10" WM was replaced with a 16" main along the project route which also included reconnection of existing water services. In addition to the WM relocations, an additional 12" WM was designed to provide an interconnect to another City owned WM to improve reliably in the City's water system in this area. The City did not have wastewater service in this area and the design included over 4,000 feet of a gravity sewer collection system with sewer lines ranging from 8" to 15". 25. Some of the most recent projects for the City include: City of Eustis Wastewater Treatment Facility Expansion - CPH provided master wastewater planning for the design service area, design of the facility improvements (environmental, civil, structural, electrical, mechanical, etc.). and permitting for an expansion of the City of Eustis Wastewater Treatment Facility. The existing facility used Trickling Filters to provide treatment of the incoming wastewater prior to final disposal via hayfield irrigation. Improvements to the facility include the following: expansion and modification of the biological treatment system (activated sludge process); the addition of secondary clarifiers, tertiary filters, RASNVAS facilities, grit removal facilities, an effluent pump station; a mechanical barscreen system; rehabilitation and modification of the existing treatment train; master pump station improvements; odor control systems; aerobic digester improvements; rehabilitation of existing lift stations; and rerating of the hayfield effluent disposal system. Master Water/Wastewater Plans - In 2005, CPH provided an update to the 2001 Eustis Master Water and Wastewater Plans with the specific objective to address the issues of the wekiva Parkway and Protection Act, Past II of Chapter 369, F.S. The master plans were developed to provide the City with the necessary tools to allow the utility systems to meet existing and future needs. We developed short term and long range capital improvement plans that the City uses to create their capital budgets. Master Storm water Plan Update - In 2005, CPH provided an update to the 2002 Eustis Stormwater Master Plan with the specific objective to address the issues of The Wekiva Parkway and Protection Act, Part II of Chapter 369, F.S.; to identify the completion of projects described and identified in the 2002 Plan; and to identify and describe projects that require attention for the future. The Update also describes the National Pollution Discharge Elimination System (NPDES) program of which the City of Eustis is a party. Some of the most recent projects for the City include: lake Alfred WWTF Upgrade and Expansion Project - The project consisted of designing and permitting the replacement of the existing 0.600 MGD trickling filter facility with a new 1.0 MGD modified activated sludge process WWTF. The design included adding a new pretreatment headworks, a new 1.0 MGD rated WWTF (anoxic zones, aeration basins, and clarifiers), and new microscreen filtration (two units). The existing trickling filter tank was converted to a 260,000 gallon equalization addition, the primary clarifier was demolished, and the existing sand filters were modified and converted to a second chlorine contact chamber. Echo Terrace Drainage Improvements - This project is located in the City of Lake Alfred, south of Lake Echo. The project area consists of an existing single-family residential neighborhood with paved streets that prior to improvements did not have an existing stormwater collection system. The runoff from the existing residential lots and the roads runs directly to a swale located east of the end of N Echo Avenue and David Avenue. This swale discharges directly to a wetland area (Echo Terrace Wetland). In the existing condition, the parcels located at the intersections of Cross Avenue and N Echo Avenue, and Cross Avenue and Davis Avenue experience frequent flooding. The purpose of the project was to construct new inlets, storm sewer, and stormwater ponds in order to alleviate flooding. CPH's services included design consisting of stormwater modeling and permitting through the SWFWMD, Polk County, and FOOT, and assisting the City with securing CDBG funding (CDBG Disaster Recovery Initiative) and providing documentation for City submittal to the Dept. of Community Affairs. Major construction items include installation of seven new storm inlets, new storm sewer, and the construction of two new stormwater ponds and structures. CUP Renewal - CPH assisted the City in obtaining a renewal of their water use permit. The main issue associated with this permit dealt with the population projections, groundwater modeling, wetlands monitoring and mitigation, as well as conservation efforts. 25. Some of the most recent projects for the City include: Wood Street I Soldier's Creek Crossing Improvements - The Soldier's Creek crossing of Wood Street was subject to frequent flooding since the existing culverts were severely undersized and the road grade was well below the flood stage, even during the 10 year storm. CPH's services including design and permitting through the SJRWMD. Even though other consultants had prepared for Seminole County an overall Soldier's Creek Drainage Basin Study, CPH had to create a new model for the City's project (using ICPR) and had to show that the new crossing and a change in the road grade (the road elevation was increased so it was above the 10 year flood stage), did not increase or decrease channel flood elevations upstream and downstream of the crossing. The project consisted of replacement of the existing creek crossing with a new triple barrel concrete box culvert including headwalls and wingwalls, roadway re-grading and widening at the crossing, creek and embankment improvements, sodding, and other improvements associated with the new crossing. Sports Complex - CPH was retained by the City of Lake Mary to provide master planning, design, bidding and services during the construction phase for this project. The primary facilities included a multi-purpose playing field, two softball fields, two Little LeaguelT- ball fields, and one picnic area with a fishing pier. Support facilities and improvements consisted of a restroom facility, stabilized parking area, site grading and development, and bleachers and bike racks. CPH provided coordination with other consultants, including a landscape architect, a surveying firm, soils engineering, and an electrical engineer. 25. On the front, it mentions RFQ No. 10-04-01, however, inside on the page that mentions 1.1 Introduction, 1.2 Time and Date of Submittal, etc., there is another RFQ No. This time, RFQ No. 07-07-01. Which RFQ No. do we use as correct? A.' The correct number is thejirst one, the 10-04-01. 6. Q: Is SF 330 substituting SF 254 and SF 255? Or one can submit either the 330 or 254/255? A: Eilhcr way, (he 330 or the 254 255 7. Q: Item 2 (list of projects for the past 5 years) can be covered through forms SF 330 and 254/255: is it going to be considered separately anyways? A: Please list the relevanr projects in the ten pages (though they are not required to be in (he same(ormat as the JJO 25-1 255). 8. Q: According to Addendum 3, proof of authorization to transact business in Florida & the financials will count towards 10-page limitation. Please see Q 2 and Q4 on addendum. We had understood at Pre-Proposal that these pages would not count. Please clarify. A: The authorization themselves are not included in the ten pages, and though the financial statements (tax return, etc.) are not either, a summary of the status of the .firm should be. Please clarify if the list of projects for the past 5 five years is included or excluded from the 10-page limit. A: My apology for the confusion. In the ten pages, you should include anything that you feel is relevant to your presentation, obviously potentially including experience. The formal presentation of experience for the past five years will not be counted as part of the ten pages. 10. Q. If the contact person in Item 5 is also one of the listed in Item 6 (where all her information will be shown), is necessary to write her information in Item 5? A: You only need to slate Ihat they are one in {he same. 11. Q: We just received Addendum #4 for the RFQ #10-04-02 and there is conflicting responses to critical submittal items. Addendum #4 question 7 indicates that relevant projects are to be included in the ten pages BUT Addendum #3 question #4 indicates that the project list are excluded from the 10 page requirement. Also Addendum #4 question #8 indicates the authorizations are not included in the 10 pages but addendum #3 indicates proof of authorization is not excluded.
