For a number of years, Calvin Giordano &
Associates, Inc. (CGA) has the honor of
currently providing professional consulting
services to the City of Sunny Isles Beach. It is
our intent to continue and build upon that
relationship by providing enhanced
responsiveness on any and all projects and
issues presented by the City.
Douglas Taylor, PE will act as project
manager, and client contact for all projects
and tasks resulting from this contract. Mr.
Taylor has over 26 years of experience in
dealing with civil engineering projects. For
20 of those years, he has served as a project
manager on projects that have included
engineering, surveying, landscape
architecture, planning, and construction
management. In addition, for the past 17
years he has managed offices, departments,
and divisions, and served as primary client
contact for many public clients.

CGA is a multi-disciplinary firm that has been
located in South Florida for over seventy
(70) years. The company has grown from a
small, two-person surveying firm to
approximately two hundred (200)
employees, providing a broad range of
services, including civil & electrical
engineering, landscape architecture,
planning, surveying, geographic information
systems (GIS), transportation engineering,
environmental services, construction
management, indoor air quality, and
emergency management.
Since its inception, CGA has served as a
professional consultant to various
municipalities throughout South Florida, including
the Cities of, Sunny Isles Beach, Dania Beach, North
Lauderdale, North Miami Beach, Pembroke Pines,
Riviera Beach, Surfside, Bay Harbor Islands and
Weston, many of which have been repeated clients
for more than ten (10) years.
If selected, CGA's Fort Lauderdale office located at
1800 Eller Drive, Suite 600, Fort Lauderdale, Florida
33316 will be the responsible office for this contract.
Based on the RFQ, CGA proposes to perfD rm the
following civil engineering related tasks for the City
of Sunny Isles Beach:
General civil design work, creation of construction
contract plans and special provisions, post design
services, bid administration, constructio n
management, construction inspection, traffic,
electrical, biological resources environmental,
hurricane recovery and debris monitoring,
landscape architecture, survey, and public
involvement.
The following is a basic overview of these services as

GENERAL CIVIL DESIGN
(ROADWAY, DRAINAGE, UTILITIES)
Calvin Giordano and Associates, Inc. (CGA) has
provided general civil engineering consulting
services to municipalities such as the cities of Sunny
Isles Beach, Miramar, Miami Beach, Weston, North
Miami, Pembroke Pines, Dania Beach, North

CGA's roadway design experience ranges
from small, minor roadway modifications to
large multi-lane highways and interstates.
In addition, CGA designs, coordinates, and
permits new utilities and utility relocations
in a number of municipalities.
CGA's drainage experience includes acting as
District Drainage Engineer to the Indian
Trace Community Development District and
the Bonaventure Development District, and
the South Broward Drainage District, as well
as creating many stormwater master plans
and other drainage projects.
CGA's broad range of roadway design and
drainage services includes:

TRAFFIC AND
TRANSPORTATION ENGINEERING
From experience in managing Florida Department of
Transportation (FDOT), municipal and private-
sector transportation projects, CGA has developed a
unique and successful approach that integrates
"agency" and "private-sector" perspectives. This
approach has proven to be mutually beneficial for
public and private sector clients allowing projects to
proceed more efficiently. CGA is qualified to assist
with a broad range of technical and regulatory tasks

CONSTRUCTION MANAGEMENT
AND INSPECTION SERVICES
CGA has an outstanding in-house
Construction Management (Construction
Administration and Construction Inspection)
team with the training, certifications, and
experience to handle any roadway, bridge,
building, park or utility construction project
for the City of Sunny Isles Beach including
but not limited to: City facilities, urban
roadway reconstruction, widening or
resurfacing, utility relocation, fixed and
movable bridges, drainage, lighting and
signalization, as well as landscape
installation/relocation inspection and
maintenance monitoring services.
In addition, our team possesses the expertise
to assist the City of Sunny Isles Beach with

control plan analysis, environmental

The CGA Electrical Department has broad
capabilities for the infrastructure and the
end use development for commercial,

We know from experience that a project's success

clients insight and creative solutions to keep their

from governmental agencies. For example, we have
processed more than 100 dredge-and-fill
applications for roadway projects to developments

SURVEY & MAPPING
The Surveying and Mapping Division offers the
services, staff and technology to ensure that every
survey project is executed successfully, and to our
clients' satisfactions. CGA provides boundary
surveys, topographic surveys, route surveys, GPS
surveys, As-built surveys, sub-surface utility location
surveys, Right-of-Way mapping, infrastructure
layout, platting services, etc...
The Survey department maintains seven (7) field
crews out of the Fort Lauderdale office which can be
utilized for any projects for the City of Su nny Isles
Beach. Our firm has nine (9) survey grade GPS
systems available and we are capable of both static
and RTK GPS surveying.
CGA's six (6) professional surveyors (not including
field crew) have nearly one hundred forty (140)
years of collective surveying experience. In addition
to Chapter 177, CGA's surveyors are familiar with
Chapter 287.055, Chapter 472, Florida Statute, and
61G17, Florida Administrative Code.

Mr. Feoli worked closely with Town Staff to devise a strategy for improving the qualities and character of the Town's
west island's neighborhood, something the Town had been struggling with for over 5 years prior to contracting CGA.
The project consists of engineering and landscape improvements that will in essence upgrade the residential streets.
The proposal provides for designated walking areas, planted and irrigated bulb-outs and signage throughout. While
the scope of the project is similar to others in nature that CGA has performed, this project stands out as one where
CGA's heavy involvement in marketing, graphics and conflict resolution took center stage. These skills aided Mr. Feoli
is facilitating approval of the final design strategy, despite competing interests tearing it at its seams with varying

Mr. Jackson has 40 years of experience in all phases of land surveying including boundary, coastal control,

highly active in professional surveying organizations and is currently President of the Florida Surveying and

The project is located in the City of Sunny Isles Beach, in Miami-Dade County and consists of approximately 3,000
linear feet of roadway, swales, and sidewalks extending from Collins Avenue to the end of Atlantic Boulevard. By
means of a Special Assessment, the completed project will service 62 residences on an island with new water
mains, sanitary sewer, and under grounding of electrical power, cable television, and telephone. In addition,
drainage improvements including curb and gutter, retention areas, pollution retardant devices, and a storm water
collection system were constructed. Services, such as electric power, water, and sewer were financed by the City at
the current rate of interest over a period of15 years.

CGA manages the $36-million capital improvement program for the City. Current projects in the program include

The City of Sunny Isles Beach requested a traffic engineering study be completed to support the closing of the NW
175th Terrace median opening along SR AIA (Collins Avenue). The study limits of the project included SR AlA
(Collins Avenue) from NW 174 Street to NW 178 Street.

neighborhood on the north side of the Town's west island. The project repaved two miles of roadways, added

With the expansion of their wastewater treatment plant, the City is relocating its' Public Services Campus further
east to a defunct golf course site purchased by the City. Calvin Giordano & Associates, Inc. (CGA) is providing
engineering and landscape architecture services to the City for a proposed facility which consists of the
construction of a two-story administrative office building, and three other buildings that function as City garages
for maintenance and for storage. The complex will have 85,000 square feet under roof, including a City fueling
station, and will be designed to GBC LEED "silver" standards.
In addition, the overall City site includes approximately 30 acres of wetland mitigation as part of a permit issued
by SFWMD, Broward County, and the Army Corps of Engineers to compensate for wetlands disturbed with the
construction of parking at Pembroke Pines City Center, and the upgrading of perimeter canals to South Broward
Drainage District (SBDD) standards. CGA has designed the turn lanes on Pembroke Road, entry road into the site,
the 30% Basis of Design Plans for the Public Services Campus and the offsite water and sewer extensions for
issuance under a design/build RFP.
The project is currently in the permitting and bidding phase.

2. 1 understand that a "public entity crime" as defined in Paragraph 287.133(1)(g), Florida
Statutes, means a violation of any state or federal law by a person with respect to and directly
related to the transaction of business with any public entity or with an agency or political
subdivision of any other state or of the United States, including, but not limited to, any bid or
contract for goods or services to be provided to any public entity or an agency or political
subdivision of any other state or of the United States and involving antitrust, fraud, theft,
bribery, collusion, racketeering, conspiracy, or material misrepresentation.
3. 1 understand that "convicted" or "conviction" as defined in Paragraph 287. 133(1)(b ), Florida
Statutes means a finding of guilt or a conviction of a public entity crime, with or without an
adjudication of guilt, in any federal or state trial court of record relating to charges brought by
indictment or information after July 1, 1989, as a result of a jury verdict, nonjury trial, or
entry of a plea of guilty or nolo contenders.
4. I understand that an "affiliate" as defined in Paragraph 287.133(1)(a), Florida Statutes, means:
a. A predecessor or successor of a person convicted of a public entity crime; or
b. An entity under the control of any natural person who is active in the management of the
entity and who has been convicted of a public entity come. The term "affiliate" includes
those officers, directors, executives, partners, shareholders, employees, members, and
agents who are active in the management of an affiliate. The ownership by one person of
shares constituting a controlling interest in another person, or a pooling of equipment or
income among persons when not for fair market value under an arm's length agreement,
shall be a prima facie case that one person controls another person. A person who
knowingly enters into a joint venture with a person who has been convicted of a public
entity crime in Florida during the preceding 36 months shall be considered an affiliate.
5. I understand that a "person" as defined in Paragraph 287.133(1)( e), Florida Statutes, means
any natural person or entity organized under the laws of any state or of the United States
with the legal power to enter into a binding contract and which bids or applies to bid on
contracts for the provision of goods or services let by a public entity, or which otherwise
transacts or applies to transact business with a public entity. The term "person" includes
those officers, directors, executives, partners, shareholders, employees, members, and
agents who are active in management of any entity.
6. Based on information and belief, the statement which I have marked below is true in a
relation to the entity submitting this sworn statement. (Please indicate which one (I) of the
following three (3) statements is applicable.)
