Threshold Inspection Services for the Heritage Park Parking Garage
To Whom It May Concern:
Having performed over 36 projects within and around the City of Sunny Isles Beach including working
directly for the City and on several parking garages, MACTEC Engineering and Consulting, Inc. (MACTEC)
is highly interested in providing the City of Sunny Isles Beach with Professional Threshold Engineering
Services for the Heritage Park Parking Garage project. We have reviewed the City of Sunny Isles Beach
RFQ and understand the need to establish a contract with a licensed threshold engineering company for
this project. Our experienced staff and their families live, work, and play in Miami-Dade County and
have been doing so for over 30 years. MACTEC brings a strong understanding of the critical details
associated with threshold engineering and inspection, construction, and representing the best interests
of the Owner.
This contract will be managed by Mr. Armando Hernandez, P.E. who is a Senior Project Manager and
Licensed Threshold Engineer with MACTEC. Mr. Hernandez has a tremendous background working and
managing threshold engineering projects including most recently on the South Terminal Expansion
Program at Miami International Airport and the u.s. Army Southern Command Headquarters Facility in
Dora!. Furthermore, MACTEC is pleased to present our fully qualified team of engineers, metallurgical
inspectors, building inspectors, certified welding inspectors and concrete technicians which have been
performing engineering, inspections and construction services in Miami-Dade County on similar parking
garage projects.
As a high quality engineering firm serving the City of Sunny Isles Beach, overseeing this contract we will
commit all available resources to the City of Sunny Isles Beach in order to provide the services required
during project construction.

Mr. Davis is a Senior Civil Engineer with almost four decades of experience in materials testing, engineering construction, and as a Threshold Inspector. Mr.
Davis has extensive and varied experience in materials engineering. Pavement evaluations have included numerous failure investigations and design of
pavement sections for roads, parking garages and surfaces, airfields for both rigid and flexible pavements. Structural evaluations have employed both
destructive and nondestructive techniques, including multiple types of load testing. Other materials engineering experience includes concrete evaluations,
failure investigations, mix designs, and concrete component evaluations for compliance with codes and specifications.
Selected Proiect Exoerience
iI Threshold Inspector: Fidelity National Financial Garage, Jacksonville, Florida.
MACTEC performed geotech pile inspection, CMT, and threshold inspections on this five story parking garage constructed using a combination of site
cast tilt wall panels and prefabricated concrete panels. Structure supported by shallow foundations.
:1 Threshold Inspector: BB& T / Trammell Crow Company Florida Sites Phase I Environmental Site Assessments, Due Diligence Services, Various
Locations, Florida. Under direction of Trammel Crow / CB Richard Ellis, performed due diligence services, Phase I threshold inspecetion site
assessments, geotechnical evaluations, property surveys and civil site development, construction document and regulatory permitting services for
more than a dozen sites throughout the State of Florida for BB& T prototypical bank branches with drive-thru aisles. Responsible for reviewing
technical accuracy of all contracts, and overseeing due diligence activities.
:1 Threshold Inspector: Emerald Beach Resort LLC Construction Materials Testing and Monitoring, Threshold Inspection and Geotechnical Services,
Panama City Beach, Florida. Geotechnical consulting, foundation recommendations, auger-cast pile installation monitoring and pile-load testing,
construction materials testing, and Special/Threshold Inspection for a multi-building resort development comprising four high-rise (24-story)
condominium towers providing 1,100 residential units, a pedestrian bridge, retail space, multiple mid-rise parking structures, convention center, and
mid-rise condominium towers north of the waterfront development. Responsible for ensuring that all materials testing, geotechnical explorations, and
threshold inspections are performed in accordance with correct standards.

Mr. Elizee has 29 years of experience as an inspector and engineering technician in the construction industry. Mr. Elizee is experienced in the inspection and
testing of concrete, reinforcing steel, soils, asphalt, roofing and fireproofing materials, including threshold inspections. Mr. Elizee has also provided
engineering inspection of pile foundations, drilled piers, structural concrete, fill soils and pre-cast concrete plants. Mr. Elizee has worked on a full-time basis

Statutes, means a violation of any state or federal law by a person with respect to and
directly related to the transaction of business with any public entity or with an agency

limited to, any bid or contract for goods or services to be provided to any public entity
or an agency or political subdivision of any other state or of the United States and
involving antitrust, fraud, theft, bribery, collusion, racketeering, conspiracy, or

of an affiliate. The ownership by one person of shares constituting a controlling
interest in another person, or a pooling of equipment or income among persons

United States with the legal power to enter into a binding contract and which bids or
applies to bid on contracts for the provision of goods or services let by a public entity,
