Sealed Bids will be received at City Hall, City of Sunny Isles Beach, 17070 Collins Avenue, Suite #250, Sunny Isles Beach, Florida, 33160, until 3.00 p.m., local time Friday November 16, 2001, at which time they will be publicly opened and read in the City Hall Commission Chambers. The envelope containing the sealed bid must be clearly marked "IMPORTANT -BID ENCLOSED FOR EMPLOYEE DENTAL LIFE LONG TERM DISABILITY AND VISION [RFP No. 01 -10 -011 ". Each proposer must submit one original and ten (10) copies of the com- plete proposal package. Faxed or e- mailed bids will not be accepted. Proposals, once received, become the property of the City, cannot be withdrawn, unless withdrawal is based on the reasons set forth in the City's procurement ordinance, and will not be returned to the proposers. The City of Sunny Isles Beach reserves the right to accept or reject any or all bids, to waive any informality, irregularity or technicality in any bid, to readvertise for bids, or take any other such actions that may be deemed to be in the best interests of the City. At the discretion of the City, firms sub- mitting proposals may be requested to make oral presentation as part of the evaluation process. If any addenda are issued to the RFP, a good faith attempt will be made to furnish a copy of such addenda to each of the persons or firms who, ac- cording to the records of the City, previously received a copy of this RFP. However, prior to submitting the proposal, it shall be the sole responsi- bility of each proposer to contact the City to determine if addenda were is- sued and, if so, to obtain such addenda.
