Any information contained herein will be considered part of the Invitation to Bid and such will be used in the evaluation of the bid responses. If you have already submitted your bid, please review this addendum and re- submit your response should this addendum modify your response. ADDENDUM DESCRIPTION 1. A Non - Mandatory Pre -Bid Conference will be held at 10:00 a.m. on Thursday, November 17, 2005, in the City Commission Chambers of the City of Sunny Isles Beach Government Center located at 18070 Collins Avenue, Sunny Isles Beach, Florida 33160. 2. Sealed bids will be received by the City Clerk until 10:00 a.m. on Thursday, December 15, 2005 in the City Commission Chambers of the City of Sunny Isles Beach Government Center located at 18070 Collins Avenue, Sunny Isles Beach, Florida 33160. Bids received after this time will be returned unopened. Bids submitted will be opened publicly and read aloud at this time. All other documents, specifications, drawings, terms and conditions remain the same. Bidders must acknowledge receipt of Addendum on Page 00300 -4. Is there a construction budget for the project? YES. The estimated budget for the project is 53.8 Million, including a 10% contingency for the use of the City. 2. Is there any special type of bedding for the pipe required, any piles, Styrofoam, geo -type fabric? YES. The answer is in the plans. 3. The maintenance of traffic and the sequencing of the project construction, it is in three phases as he understands.? YES. Those are guidelines for MOT. The contractor could submit his own maintenance of traffic and sequence of work plan. 4. The times for construction? There is a City Ordinance for hours of operation, Monday through Friday, 7:30 a.m. to 5:00 p.m.; Saturday, from 9:00 a.m. to 5:00 p.m., and no work on Sundays and legal holidays. 5. The documents show 27 wells but sheet 40 indicates only 25, and he said he submitted it in writing [e- mail]? 25 wells is the correct number. Use this number in your proposal. 6. FDEP State Permit for drainage wells, has that been pulled, and if not who is responsible for pulling it? The construction permit will be obtained by the Contractor the City will assist with the documentation to obtain the permits. See Division II, Special Provisions, Section 7 of the Contract Documents and Specifications. 7. Also with the State FDEP, they have a new requirement of a reasonable insurance report, which is basically telling them how deep we are going to put our casing and they have to approve that report. Has anyone started that and who is going to be responsible for that? The City will contact the FDEP to obtain this information. 8. Capacity testing, is that required on all wells? YES. It will be required for each well. The testing will be done and certified by the Contractor. The City reserves the right for an independent quality assurance test to corroborate the Contractors certification findings. 9. It doesn't state anything about the casing thickness, I am assuming the standard 3/8" is okay? Use the standard FDOT approved casing thickness. 10. As far as the permits by the contractors are concerned, what are the different agencies that we have to get permits from? Department of Environmental Resources Management (DERM) and the Florida Department of Environmental Protection (DEP). 11. Are we required to get permits from the City, the Public Works Department? Yes. From the City of Sunny Isles Beach Building Department but there is no fee. 12. What is the estimated start date for the project? Expected starting date is the end of March or early in April 2006. 13. What is the average casing depth on the well? It is in shown on the plans. That is FDOT standard. The minimum well depth of 100 feet or 1,000 GPM inflow capacity. However due to possible FDEP change in requirements, Bidders shall base their unit price for each well with fifty feet (50') of casing. Should FDEP require additional length of casing, Contractor will be compensated for additional linear feet of casing only. Depth of well shall be sufficient to accommodate 1000 GPM inflow capacity. 14. If the well is 100 feet and the casing is 100 feet where are you going to get the water to drain? See well detail in sheet 40 15 Will the site be truck ready or will we have to improve the sites? Site is 100% accessible. Please see Section 100, Instructions to Bidders, paragraph 3 16 How do we dispose of the fluids and the cuttings generated? Qucstions /Comments Bid 05 -10 -01 Page 2 of 4 The Contractor shall dispose properly offsite. 17 Specifications for the pump testing? Use FDOT or FDEP specifications for the testing procedure. 18 Being that the date is extended can the last day of questions be extended? Questions have to be submitted five days prior to the new date of received bids. 19 What are the minority requirements? None. 20 Clarification on the wells: The size of the pre- sedimentation box on the well will be defined as soon as we get the approval from FDEP. 21 The MOT it is talking about possibly the use of Police Officers, do we have a rate of the Police Officers pay and how do we get reimbursed? For hourly rate verify with the Sunny Isles Police Department. For reimbursement see summary of pay items. 22 Is it going to be necessary to get a field office out there? YES. 23 On the clearing and grubbing, you have 15 then you have a lump sum and then AC. Is it going to be a lump sum of clearing and grubbing or is it going to be by 15 acres or how is it going to be done? Use 15 acres multiplied by the Unit price per acre you are proposing. This is the LS for the project. 24 Where it says lump sum and then slash AC, if we give you a price for clearing and grubbing, are we going to get the clearing and grubbing item or are you going to back it down by 15 acres? The lump sum is for the whole site. See No. 23 above. 25 How you are going to get paid by the lump sum of the clearing and grubbing? Monthly payment (1 /12th per month) of original construction contract. Questions /Comments Bid 05 -10 -01 Pagc 3 Of 4 26 Are you going to supply a construction yard first for equipment, supplies, extra fill and dirt? The staging area will be determined by the City. 27 He said that November 23, 2005 is an early date due to the fact that you are going to have another meeting and they will only have a week to put this together and then the holiday will be here, and he said that we should consider having a later bid opening date. The bid opening date was issued by Addendum No 28 What about relocation of water lines? We don't expect to relocate waterlines, the City of North Miami Beach who owns the system is very efficient in repairing broken pipes. 29 Some of the water mains are right in the middle of structures Any conflict with water mains during construction will be dealt in the field 30 What about raising existing sanitary lines with all the leveling? We do not expect to relocate any sanitary sewer lines. 31 We must include in the prices from the items 430 - 171 -126, 430 - 171 -125, 430 - 171 -124 and 440 -1 -60, the restoration shown on the Sheet #41? YES. See sheet No. 5, note No. 16, and sheet No. 41 for detail. 32 Is Well Point required or open pump dewatering is permitted? No dewatering to streets or bodies of water without proper containment and silt barriers will be permitted 33 Who is handling the FDEP reasonable assurance issue as it needs to be in place prior to FDEP permit issuance. The reason is that the report needs to state the casing depth and that depth needs to be accepted by FDEP before the bid. Since no one knows what FDEP will accept how can the drilling contractor provide a quote based on an unknown? Additionally, FDEP may require casing set that makes the wells deeper than the 100' indicated depth on the plans. See notes 7 and 13 this Addendum 34 How does the well contractor receive payment for the additional footage beyond the 100'? NOVEMBER 22, 2005. 1. Regarding clearing and grubbing and questions #23 and #25, we still do not completely understand your answer. Normally, a LS item is not combined with a unit price item (AC), it's either one or the other. For bidding purposes and the allocation of funds, combining a LS and unit price (AC) can be detrimental to a contractor bid. In order to avoid any misconceptions, we suggest resubmitting a revised page 00300 -2 and changing item 110 -1 -1 to read only LS and eliminate the 15 - acre unit quantity. Answer: The LS /AC unit price is an FDOT standard. 2. Additionally, item # 25 answers the question regarding payment of the clearing and grubbing item. To clarify, I believe you meant the contractors would get paid 1/ 12"' per month of the LS item amount and not the original construction contract. The original construction contract could be interpreted as the bidders price for performing the entire project, not just one line item. Answer: Yes, payment will be 1/12 1h per month of bid unit price for Clearing and Grubbing. QUESTIONS AND COMMENTS FROM GIC, WATER SYSTEMS, NOVEMBER 22, 2005 1. What is the average casing length? Answer: See Addendum 2, Question # 13 2. What is the average borehole depth? The information included on sheet 40 of the project drawings shows minimum well depth of 100 feet and the well depth as casing depth + 50 feet. The drawing does not show typical casing length. Because the wells are likely to be subcontracted, the two average lengths, or a "base bid" length for items 1 and 2 above are necessary for bidding purposes. Answer: See Addendum 2, Question # 13 3. Will the sites be prepared for the drilling equipment? Answer: See Addendum 2, question # 15. 4. Can "clean" drill cuttings be spread on the sites? If they have to be removed does the city have a designated location? Answer: The Contractor shall dispose properly offsite. The City does not have a designated location. 5. The same question for fluids generated by the drilling process. Are there sewers readily accessible for that purpose? Answer: The Contractor shall dispose properly offsite the "fluids" generated by the drilling process. No sewers are available for fluids disposal. 6.- Please describe the aquifer test requirements required to demonstrate the well capacity. Answer: Testing flow capacity and water quality shall comply with FDEP requirements for Class V storm drainage wells. (USDW stratum) 7.- Can the wells be constructed consecutively or are the sequenced into the project construction sequence? Answer: Construction sequences and MOT are for reference only. Contractor may suggest construction phasing. QUESTIONS AND COMMENTS FROM PETRO HYDRO, INC. DECEMBER 1, 2005. 1.- The RCP must be installed on "DRY "? Answer: See water table elevations on sheet No. 43 of the construction documents (plans) Questions /Comments laid 05 -10 -01 Page 2 of 2 City of Sunny Isles Beach 18070 Collins Avenue Sunny Isles Beach, Florida 33160 Bid No. 05 -10 -01 - Central Island Drainage and Street Improvements ADDENDUM 5 QUESTIONS AND COMMENTS FROM JAFFER ASSOCIATES CORP. DECEMBER 8, 2005. 1.- The Florida Department of Environmental Protection ( "FDEP ") will not issue well construction permits to drill wells without a "Reasonable Assurance Report" in hand. This is an extensive report with lithologic logs and water quality logs written by a professional geologist what the minimum requirements need to be in casing. Who is responsible for producing the report ? ? ? ? ?? It is essential to know in order to provide a proper quotation for this project. This report is very expensive that will require at least one test hole and extensive analysis and preparation. Please answer this question. Answer: See Addendum No. 2, item No. 6. City of Sunny Isles Beach 18070 Collins Avenue Sunny Isles Beach, Florida 33160 Bid No. 05 -10 -01 - Central Island Drainage and Street Improvements ADDENDUM 4 QUESTIONS AND COMMENTS FROM OCEAN BAY CONSTRUCTION INC. DECEMBER 5, 2005. 1. Additionally, in the Contract Specifications under Special Provisions page 4 of 16 paragraph 13, please clarify the withdrawal period of 300 calendar days. Is the 300 calendar days a typo and should be 30 calendar days? Answer: It should be Ninety days (90 days) QUESTIONS AND COMMENTS FROM JAFFER ASSOCIATES CORP. DECEMBER 7, 2005. 1.- Ms. Hines please respond to item 7 of Addendum No. 2 concerning FDEP requirements for reasonable assurance. It was not addressed in Addendum 3, but it was stated in Addendum that the City would contact FDEP to obtain the information. It is paramount to the price of the bid. Answer: For base bid price of casing depth see Addendum No. 2, item No. 13. The casing for each well shall comply with Condition Number 5 and 8 of the Class V Group 6 Storm Water Drainage Well permit application.
