Submittals received after this time will not be accepted. Submittals will be opened publicly and read aloud at this time. A mandatory Pre-Submittal Conference is scheduled for 10:00 a.m., Wednesday, April 11, 2007 at the Sunny Isles Beach Government Center, 18070 Collins Avenue, Fourth Floor Conference Room, Sunny Isles Beach, Florida. All consultants planning to submit a submittal are required to attend this meeting. Consultants should allow sufficient time to ensure arrival prior to the indicated time. Submittals from those who have failed to attend will not be accepted. RFQ packages may be obtained from Jane A. Hines, City Clerk, City of Sunny Isles Beach, 18070 Collins Avenue, Sunny Isles Beach, Florida 33160, at no cost. These documents may be examined at the City of Sunny Isles Beach, at the above referenced address. The City reserves the right to reject any or all submittals, with or without cause, and to waive technical errors and informalities, and to accept the submittal which best serves the interest of the City. Page 3 01'9 RFQ 07-03-0 I Engineering Srvs Bridge Revised (Addendum.l) REQUEST FOR QUALIFICATIONS THE CITY OF SUNNY ISLES BEACH REQUEST FOR QUALIFICATIONS NO. 07-03-01 PROFESSIONAL GENERAL ENGINEERING CONSULTANT FOR THE THE NORTH BAY ROAD PEDESTRIAN AND EMERGENCY VEHICLE BRIDGE CONNECTING 174TH TO 172nd STREET Notice is hereby given that the City of Sunny Isles Beach is requesting qualifications for a qualified Professional General Engineering Consultant as authorized by Section 287.055, Florida Statutes (1997) to design the North Bay Road Pedestrian and Emergency Vehicle Bridge, and the Town Center Park Observation Deck and Boardwalk. This bridge will connect North Bay Road, from 174th Street to 172nd Street. The City is soliciting professional engineering services for the design and preparation of complete construction contract plans and special provisions for these two related and interconnected projects. Proiect Description: North Bay Road Pedestrian and Emergency Vehicle Bridge The first project is the construction of a bridge that will serve as a pedestrian/bike and emergency traffic bridge. The bridge will connect the southern end of North Bay Road, just south of 174th Street, to the north bank of the canal, with a pedestrian section continuing to 172nd. The proposed bridge will have two thirteen-foot wide vehicular lanes and two full seven-foot wide outside shoulders, to be used as pedestrian/bike lanes. At landfall on the southern side of the canal, it will be narrowed toward its western edge, to a twenty foot wide pedestrian/bike path capable of carrying emergency vehicles. At its southern terminus, it will tie into 172nd Street just east of North Bay Road. This structure shall be an at-grade bridge, not elevated. Page 4 01'9 RFQ 07-03-01 Engineering Srvs Bridge Revised (Addendum.l) Proiect Description: Town Center Park Observation Deck and Boardwalk The second project is the construction of an observation deck in Town Center Park and a boardwalk connecting the deck to the North Bay Road pedestrian/bike and emergency traffic bridge described above. This project will be extremely environmentally sensitive and will require a great deal of interaction with the appropriate permitting agencies. Scope of Services: Consultant services shall include but not be limited to the following: . Project Management . Public Involvement . Full Design Survey . Roadway Design . Signing and Pavement Markings . Drainage . Bridge Design . Lighting . Environmental Permitting, including mitigation plans . Utility Coordination . Geotechnical Engineering . Landscape Architecture . Specifications . Post Design Services (Shop drawing review and Response to Request for Information) . Bid Administration (Selection and Letting) . Construction Management and Inspection (CEI) Conceptual sketches of this structure shall be presented at a mandatory Pre-Submittal Conference to be held on at 10:00 a.m., Wednesday, April 11, 2007 in the Fourth Floor Conference Room of the Sunny Isles Beach Government Center Building located at 18070 Collins Avenue. Attendance at this meeting is mandatory, and submittals from firms not in attendance will be rejected. All responses by consulting firms to the RFQ (hereinafter referred to as their submittal) shall contain no more than fifteen pages. I ) consultants must submit five (5) complete copies of the submittal that include, at a minimum, the following information: 1. Name, address and company, including, but not limited to, a business overview, financial state of the business, annual revenue for past two years, and names and addresses of persons having financial interest in the firm. If sub-consultants are used, the bid must specify the type of contractual agreement between all firms and include certificate(s) of authorization to offer professional services through the Florida State Board of Engineers, including the prime and supporting firms. 2. A list of all projects (including governmental projects) completed or in process for the past five years, and a corresponding list of client references with contact names and telephone numbers. 3. Proof of authorization to transact business in Florida from the Florida Secretary of State, from the prime as well as supporting firms. 4. Appropriate current Federal 254 and 255 forms showing the firm data and supporting the firm's ability to perform each category of work required. 5. The name, title, qualifications, experience, and all contact information of the proposed project manager. 6. A listing of the proposed personnel to be assigned to the project, listing their qualifications and experience. 7. A listing of all permits expected to be necessary, and the issuing agencies that will be involved. 8. A general schedule showing the mileposts for design, and estimates on the construction timetable. 9. An estimate of the total construction costs (not includinq desiqn fees) for the finish project as designed by the firm. 10. The general breakdown of person hours for the completed design of the project (including the time necessary to obtain of all permits). Submittals will be received by the City Clerk until 10:00 a.m.. Wednesday. April 18. 2007. at the Sunny Isles Beach Government Center, Fourth Floor Conference Room, 18070 Collins Avenue, Sunny Isles Beach, FL 33160. Submittals will be opened publicly and read aloud at this time. Submittals may be rejected after this time. The City is under no obligation to return the submittals.
