ADDENDUM 5
CLARIFICA TION
A potential bidder brought up a question in connection with obtaining Public Entity
Crimes and Non-Collusive Affidavit forms with original signatures from both prime
and sub-consultants. He was under the impression that "these forms would only be
required from prime" consultants. The potential bidder requested a "one day extension
to gather original signatures from sub-consultants or the allowance to substitute the
forms with a PDF".
City's Response: We will allow a PDF only if the originalsfollow the day after.
