NOTICE TO CONSULTANTS
NOTICE IS HEREBY GIVEN that the CITY OF SUNNY ISLES BEACH is seeking qualifications for
the following:
Request for Qualifications No. 07-04-02
Professional General Engineering Consultant
for the Heritage Park Parking Garage
Notice if hereby given that the City of Sunny Isles Beach is seeking Professional Engineering Services for
the design and preparation of complete construction contract plans and special provisions for the Heritage
Park Parking Garage. Sealed submittals will be received by the City Clerk until 10:00 a.m., Wednesday,
May 30, 2007, at City of Sunny Isles Beach, City Hall located at 18070 Collins Avenue, First Floor
Meeting Room, Sunny Isles Beach, Florida, 33160. Submittals received after this time will not be
accepted. Submittals will be opened publicly and read aloud at this time.
A mandatory Pre-Submittal Conference is scheduled for 10:00 a.m., Wednesday, May 16,2007 at the
Sunny Isles Beach Government Center, 18070 Collins Avenue, First Floor Meeting Room, Sunny
Isles Beach, Florida. All consultants planning to submit a submittal are required to attend this meeting.
Consultants should allow sufficient time to ensure arrival prior to the indicated time. Submittals from
those who have failed to attend will not be accepted.
The garage must accommodate at least 450 parking spaces, meeting minimum City Codes in a maximum
of four (4) levels of parking (including the ground level). Parking will be allowed on the top (open) level.
A driveway entrance from Gallahad Dade Boulevard will be required. The structure's foot print should
not exceed one acre and is anticipated to be approximately 125 feet by 350 feet.
Consultant services shall include but not be limited to Project Management; Public Involvement; Full
Design Survey; Driveway Design; Signing and Pavement Markings; Drainage; Lighting; Environmental
Permitting, including any necessary mitigation plans; Utility Coordination; Geotechnical Engineering;
Specifications; Post Design Services (Shop drawing review and Response to Request for Information);
Bid Administration (Selection and Letting); and Construction Management and Inspection (CEI).
RFQ packages may be obtained from the Office of the City Clerk, City of Sunny Isles Beach, 18070
Collins Avenue, Sunny Isles Beach, Florida 33160, at no cost.
The City reserves the right to reject any or all submittals, with or without cause, and to waive technical
errors and informalities, and to accept the submittal which best serves the interest of the City.

Notice is hereby given that the City of Sunny Isles Beach is requesting
qualifications for a qualified Professional General Engineering Consultant
as authorized by Section 287.055, Florida Statutes (1997) to design the
Heritage Park Parking Garage. This garage will relocate the existing
surface parking so that a new City Park can be constructed.
The City is soliciting professional engineering services for the design and
preparation of complete construction contract plans and special provisions
for this project.
Proiect Description:
The garage must accommodate at least four hundred and fifty parking
spaces meeting minimum City codes in four levels of parking (including the
ground level). Parking will be allowed on the top (open) level. A
driveway entrance from Gallahad Dade Boulevard will be required. The
structure's foot print should not exceed one acre and is anticipated to be
approximately one hundred and twenty-five feet by three hundred and fifty
feet.
Scope of Services: Consultant services shall include but not be limited to
the following:

. Public Involvement
. Full Design Survey

. Signing and Pavement Markings
. Drainage
. Lighting
. Environmental Permitting, including any necessary mitigation plans

Conceptual sketches of this structure shall be presented at a mandatory
Pre-Submittal Conference to be held on at 10:00 a.m., Wednesday, May
16, 2007 in the First Floor Meeting Room of the Sunny Isles Beach
Government Center Building located at 18070 Collins Avenue.
Attendance at this meeting is mandatory, and submittals from firms not in
attendance will be rejected.
All responses by consulting firms to the RFQ (hereinafter referred to as
their submittal) shall contain no more than fifteen pages. To be eligible, the
consultants must submit five (5) complete copies of the submittal that
include, at a minimum, the following information:
1. Name, address and company, including, but not limited to, a
business overview, financial state of the business, annual revenue
for past two years, and names and addresses of persons having
financial interest in the firm. If sub-consultants are used, the bid
must specify the type of contractual agreement between all firms
and include certificate(s) of authorization to offer professional
services through the Florida State Board of Engineers, including
the prime and supporting firms.
2. A list of all projects (including governmental projects) completed or
in process for the past five years, and a corresponding list of client
references with contact names and telephone numbers.
3. Proof of authorization to transact business in Florida from the
Florida Secretary of State, from the prime as well as supporting
firms.
4. Appropriate current Federal 254 and 255 forms showing the firm
data and supporting the firm's ability to perform each category of
work required. NOTE: These forms will not be counted as
part of the fifteen paQe maximum.
5. The name, title, qualifications, experience, and all contact
information of the proposed project manager.
6. A listing of the proposed personnel to be assigned to the project,
listing their qualifications and experience.
7. A listing of all permits expected to be necessary, and the issuing
agencies that will be involved.

8. A general schedule showing the mileposts for design, and
estimates on the construction timetable.
Submittals will be received by the City Clerk until 10:00 a.m., Wednesday.
May 30. 2007, at the Sunny Isles Beach Government Center, First Floor
Meeting Room, 18070 Collins Avenue, Sunny Isles Beach, FL 33160.
Submittals will be opened publicly and read aloud at this time. Submittals
may be rejected after this time. The City is under no obligation to return the
submittals. The City reserves the right to reject all submittals and to waive
any defects.
