V ANUS, Inc. (V ANUS) is pleased to submit to the City of Sunny Isles Beach one original and four copies of our
qualifications to provide professional Transportation Engineering services. Between our Miami Lakes and Tampa, Florida
offices, V ANUS prides itself on developing one of the largest transportation engineering staffs in the State of Florida. We
have the capabilities and experience required to ensure that continuing professional services are provided to the City in an
efficient and timely manner. V ANUS' engineers, planners, and designers will deliver task-oriented services that are
responsive, reliable, feasible, and cost-effective. Our traditional transportation engineering services are complemented by
our Intelligent Transportation System (ITS) and communications application expertise. We have communications and
electrical engineers who work closely with our transportation engineering staff to apply their skill sets to ensure the
delivery of value-added, innovative, and practical solutions to our client's transportation problems.

V ANUS has extensive experience with on-call service engineering contracts. Within Florida, we have provided on-call
services to Collier, Lee, Pinellas, Vol usia, and Escambia counties; the cities of Boca Raton, Tampa, and Temple Terrace;
FOOT Districts One, Three, Four, Five, Six and Seven; and numerous private clients. Our assignments have included, but
have not been limited to, the preparation of traffic impact studies; intersection, corridor, access management and safety
analyses; signing and pavement marking, signal timing, lighting, sidewalk and signalization plans; and ITS studies and
design.
The V ANUS Team will be led by Ken Vorce who has extensive experience in delivering public sector engineering services.
Ken, who was the FOOT's District Six Traffic Operations Engineer from 1989 to 1993, is located in our Miami Lakes
office, and has been serving state, county, and local jurisdictions in South Florida since 1993.

SECTION TWO
EXPERIENCE
The foundation of V ANUS' transportation practice is in the transportation engineering and planning skills
and expertise of our staff. Having performed similar work for the FDOT and numerous other
jurisdictions, we understand the importance of being responsive to task assignments, being
knowledgeable of the Federal Highway Administration's (FHW A's) Manual on Uniform Traffic Control
Devices (MUTCD) and the FDOT's Manual On Uniform Traffic Studies (MUTS) requirements, and the
Florida Design Standards, having staff and resources that are readily available, emphasizing cost-effective
and implementable solutions to transportation issues, providing experienced project management, and
being committed to the delivery of quality work products.
The following paragraphs discuss our experience related to anticipated task assignments. Additional
experience is shown on Standard Forms 254/255, included in Section Four.
TRAFFIC OPERA nONS STUDIES
V ANUS has experience in all areas of traffic operations studies. Staff members have performed various
types of traffic engineering analyses, including, but not limited to: safety and traffic operations for
signalized and unsignalized intersections; corridor and arterial simulation and capacity analyses; signal
timing plan development; travel time and delay studies; highway lighting justification; roadway network
analyses; and measures of effectiveness and cost/benefit analyses. V ANUS uses modeling and simulation
software such as CORSIM, NETSIM, and SIMTRAFFIC. We also are thoroughly knowledgeable in, and
frequently use, analysis programs such as HCS, TRANSYT -7F, SYNCHRO, and PASSER II.
V ANUS specializes in performing traffic operations studies. We have four Professional Traffic
Operations Engineers (PTOEs) in the firm who have performed traffic studies for FDOT Districts One,
Three, Four, Five, Six, and Seven. V ANUS has been afforded the opportunity to work on various types
of studies and has gained insight into the best ways to complete them in a timely and efficient manner.

Project: FDOT District Six Districtwide Traffic Operations Safety Studies

V ANUS performed safety reviews at the District's 2002 high crash locations. As part of this project,
V ANUS prioritized highway sections for study from the approved 2002 District-wide High Crash List
and developed reports that evaluated each high crash location within the section by analyzing existing
crash patterns over three years of accident data, developing expected values for each location to identify
abnormal crash patterns, identifying possible causes of the abnormal crash patterns, and providing
recommendations regarding the need for further study.

As a subconsultant, V ANUS completed 22 assignments, including signal warrant analyses, lighting
justification reports, left-turn phase studies, and intersection analyses.

This is an on-going contract for which V ANUS has completed two assignments. One task involved
evaluating approximately 17 miles of US 19 through Pasco County for deficiencies in access
management. Alternative solutions to correct the problems were developed and presented to the
appropriate governmental agencies (the City Council of New Port Richey, the Pasco County Board of
County Commissioners, and the Pasco County MPO.

Project: Cape Coral Downtown Community Redevelopment Agency (CRA) Traffic Study

V ANUS prepared a traffic study that evaluated the Downtown for purposes of developing transportation
alternatives to enhance the traffic circulation and flow as it relates to vehicular traffic, and provide a
pedestrian friendly environment to improve safety and promote economic growth. Alternative traffic
controls were analyzed, including roundabouts. The plans were developed and were made available to
the public. The public had the opportunity to provide input through a questionnaire, a website, and four
public meetings.

V ANUS prepared a corridor study to identify geometric conditions required to accommodate design year
2030 traffic volumes. In addition, V ANUS prepared an access management study in accordance with
state and local requirements.

V ANUS developed interim/design year traffic volumes and determined the geometric needs for this
Pine lias County roadway and three intersections along the study corridor using appropriate traffic
engineering software. The project also included signal designs for three intersections (71st Street, 66th
Street, and 62nd Street).
TRAFFIC OPERATIONS DESIGN
V ANUS has extensive experience in design and plans preparation. Our approach consists of a thorough
collection of data, comprehensive analysis, and on-site verification of the recommendations for their
constructability prior to submitting our plans. V ANUS has prepared numerous signalization plans,
including hundreds of locations as part of signal systems in Charlotte, Collier, Palm Beach, and Broward
counties. V ANUS has also prepared highway lighting and signing/marking plans for numerous projects.
As a result, we have developed detailed Quality Assurance/Quality Control (QAlQC) processes and will
. apply these processes to the projects that the City assigns.

design included flashing beacons to warn motorists of the approaching traffic signal.

a four-lane divided facility that included the design of the four signalized intersections.

V ANUS is preparing a traffic study (design year 2030), signal, and roadway lighting designs for this
roadway widening project from SR 82 (Martin Luther King Jr. Boulevard) to SR 80 (Palm Beach
Boulevard). The project will convert a two-lane undivided roadway to a four-lane divided facility.

Project: 1-95/SR 528 High Mast Lighting

The project consists of adding two lanes to 1-95, including bridge widening, modifying ramps terminals,
and adding auxiliary lanes. V ANUS' services include preparing classification monitoring site plans for
eight locations and designing a high mast lighting system for the interchange.

V ANUS assisted the City of Tampa in implementing the signs for their speed limit reductions on City
streets in an effort to control speeding. V ANUS field-located new speed limit signs on residential streets,
provided the plans of new locations; and performed the inspection of the installed signs.
TRANSPORT A TION PLANNING
V ANUS' transportation planning capabilities include preparing traffic impact analyses for land use
rezonings and transportation concurrency issues, developing corridor studies and technical traffic
memorandums for roadway widening projects and PD&E studies, reviewing transportation studies for
developments of regional impact (DRIs) and Notices of Proposed Change (NOPCs), determining levels of
service for roadways and intersections, reviewing and amending the Traffic Circulation Elements of
Comprehensive Plans, reviewing and/or updating transportation concurrency management systems, and
reviewing traffic impact fee studies.

The V ANUS transportation planning experience includes both short and long term initiatives. V ANUS

pedestrian plans, transit management plans, traffic impact studies, transit feasibility studies, hurricane
evacuation studies, and GIS mapping. V ANUS has prepared numerous documents associated with land
development issues, such as traffic impact studies and transportation impact fee analyses. We understand
the impacts of development on a local and regional transportation system, having prepared and reviewed
many DRI studies for both private developers and regional planning commissions.

V ANUS provides on-call traffic engineering and transportation planning services to St. Lucie County. As
part of these services, V ANUS has reviewed 10 traffic impact studies and several traffic impact fee credit
analyses submitted by developers. Attendance at the Board of County Commissioners (BOCC) meetings
is required in conjunction with some of the reviews.

V ANUS, as adjunct staff to the City of Temple Terrace, reviewed the transportation analysis pertaining to
the large mixed-use development located west of 1-75 near Morris Bridge Road and Fletcher Avenue. We
provided comments related to the appropriateness of the study to ensure the City that the analysis met all
of the conditions set forth by the approved methodology as well as the guidelines and procedures outlined
in the DRI process. V ANUS also worked with the Tampa Bay Regional Planning Council (TBRPC) in
this review process.
Project: Polk County School Board Traffic Impact Studies

proposed elementary and high school. These studies were prepared in accordance with Polk County's
Land Development Code and submitted to the County's Transportation Planning Organization for review
and approval.

V ANUS developed a fair-share transportation impact methodology plan for Escambia County as a means
to offset the capital costs associated with the roadway improvements related to development traffic.
SIGNAL SYSTEMS/INTELLLIGENT TRANSPORTATION SYSTEMS (ITS)
V ANUS has experience in all aspects of ITS, including the analysis, planning, and design of Advanced
Traffic Management Systems (A TMSs), Advanced Public Transportation Systems (APTSs), Video
Monitoring Systems (VMSs), and Freeway Management Systems (FMSs). V ANUS conducts an
inventory of existing systems, defines the current communication and system component architecture, and
determines compatibility among devices. The design incorporates the most cost-efficient deployment of
equipment while ensuring the maximum return on functionality and performance.
V ANUS also has extensive signal timing experience. As the leading developer of signal timing in the
State of Florida, V ANUS has developed and installed signal timing for hundreds of intersections using
different types of controllers. V ANUS has developed signal timing plans for isolated and coordinated

signalized intersections in Broward, Palm Beach, Sarasota, Santa Rosa, Hillsborough, Hernando, Pinellas,
Citrus, Charlotte, Collier, Pasco, and Manatee counties. In addition, we currently hold the FDOT District
Seven Districtwide Signal Timing contract. Some of the software programs used to develop the signal
timings include SYNCHRO, SIMTRAFFIC, HCS, SIGNAL 2000, TRANSYT-7F, CORSIM, and
TSPPD.
V ANUS has experience working with public agencies to find solutions to complex traffic flow problems.
We are dedicated to producing signal timing plans that are consistent with the traffic movement goals,
and ensuring that the goals are realistic for the available roadway capacity. V ANUS' commitment to
producing successful signal timing plans does not end with a coded database. The field implementation
and refinement tasks are vital to transition the computer-generated timing plans into working signal
operations. We have severalIMSA-certified technicians to ensure field implementation of timing plans is
done correctly. V ANUS has also developed and presented several signal timing training courses.

Using SYNCHRO and TSPPD, V ANUS recently developed new signal timings for 50 intersections along
SR 7 and University Drive. V ANUS implemented and fine-tuned the timings, including conducting
before and after travel time evaluations.

V ANUS is under contract to the FDOT to develop signal tImings on an as-needed basis. V ANUS
recently developed and implemented signal timings for 13 intersections in Pinellas County. V ANUS has
completed developing, implementing and evaluating signal timings for 123 intersections in Hillsborough
County, Pasco County, Hernando County, and Citrus County. Intersection inventories and MUTCD
compliance reviews are part of this project.

implementation plans for 1-75 Drop Down Gates, performing Systems Engineering Management Plan
(SEMP) reviews, and managing the production of several videos for the District Four ITS program.

V ANUS prepared a Master Plan for the City of Boca Raton A TMS. V ANUS developed a Plan that
created a redundant ring topology from an existing linear system by using spare existing fibers and minor
additional fiber. This study also identified locations for the deployment of ITS components such as video
monitoring cameras, dynamic message signs (DMSs), video detection systems, emergency preemption

units, an internet web page, highway advisory radio (HAR) and kiosks as part of the regional ITS
architecture.
Project: Manatee County Advanced Traveler Information System (ATIS) Design-Build

V ANUS six DMS locations, of which five are on overhead mast arms and one is a single post ground
mounted sign. Two of the mast arm locations have signs on both sides of the mast arm. Communications
is through a cellular data modem installed in the pole-mounted cabinets. The DMSs are to be used for
notifying motorists of available parking at the beaches, alternate routes due to congestion, and/or
evacuation routes in the vicinity of the beaches.

V ANUS designed 5 DMS installations and 13 closed-circuit television (CCTV) installations for the
McMullen Booth Road corridor. The project also includes the design to upgrade 32 intersections to
adaptive signal control. V ANUS reviewed the current communications network layout for the
Countywide A TMS and made recommendations to provide a viable redundant network. V ANUS will
assist in device procurement for this project and in testing, integrating and evaluating devices and
software for the A TMS.
Project: SunNavsM ITS Incident Detection System

V ANUS developed plans to install 200 miles of detection along the Florida Turnpike Mainline. A travel
time system was also designed for this section of roadway.
Project: iFIorida Statewide Corridor Monitoring Design-Build

V ANUS designed 28 traffic monitoring sites throughout the state. The sites included video monitoring
and vehicle detection. The FDOT's microwave towers are used to transmit the data and video to the
District Five Regional Traffic Management Center (RTMC).

projects and who can focus on the real needs of a project. V ANUS has built a company around the traffic
engineering and transportation planning disciplines. Within our three Florida offices, V ANUS employs
34 professional, technical, and administrative personnel. Of those 34 employees, V ANUS has 14
registered professional engineers, thrce engineering interns, three designers, one certified technician, and
four technical specialists. The proposed staff of the V ANUS Team, shown on the following figure, brings
a balanced combination of skills and experience that are ideally suited to this project. The V ANUS Team
consists of senior engineers and hands-on technical staff experienced in all elements of transportation
engineering, transportation planning, ITS, roadway and traffic design, and signal timing. They all have
experience with conducting the type of tasks required for this contract. The V ANUS Team is confident
that the proposed staff can provide the required services to the complete satisfaction of the City of Sunny
Isles.
The following key professional staff will be assigned to the project
.
Ken Vorce, PE, will serve as Project Manager. Mr. Vorce has over 38 years of transportation
engineering experience. He served in various professional engineering positions in FOOT District Six
(including District Traffic Operations Engineer) from 1985 thorough 1993. Over the past 13 years, while
employed with various South Florida consulting firms, Mr. Vorce has performed in excess of 150 traffic
operations and safety study assignments. Mr. Vorce served as Project Manager on the FDOT District Six
Districtwide Minor Design project, where he designed several minor roadway improvement type projects
which ranged from intersection and arterial channelization improvements to minor signing and marking
upgrades. He is currcntly managing the FDOT District Six Traffic Operations and Safety Studies
contracts. In addition, Mr. Vorce serves as Project Manager on the FDOT District Four General ITS
Consultant contract.
Jay H.L. Calhoun, PE, will serve as Quality Control Officer. Mr. Calhoun has over 29 years of
transportation engineering and planning experience. He served as Project Manager for the FDOT District
Three Districtwide Miscellaneous Traffic Operations Analysis/Design Services Project, which included
signal timing, intersection analyses, and public involvement presentations for locations throughout the
Panhandle. As Project Manager for the FDOT District Seven Districtwide Traffic Operations Consultant
project, Mr. Calhoun prepared over 70 intersection analyses, signalization plans and signal warrant
analyses. He served as Project Manager for the Charlotte, Collier, and Palm Beach counties A TMS design
projects. He also served as Project Manager for the Palm Beach County Interim Traffic Management
System (ITMS), where he designed a unique ITS application that used portable devices to provide real-
time information to motorists on 1-95.
Susan Joel, PE, PTOE, will serve as Transportation Planning Leader. Ms. Joel has over 20 years of
experience. Ms. Joel served as Project Manager on the City of Cape Coral Downtown CRA Traffic
Study. She currently serves as Project Manager on the FDOT District One Traffic Operations Studies
contract. Ms. Joel is also providing on-call services for St. Lucie County, which includes reviewing
traffic impact studies, DRIs, and impact fee credits. She served as the Design Project Manager on the
Manatee County A TIS Design-Build project where she was responsible for the implementation of eight
DMSs in Manatee County to be used for beach parking, diversions, and hurricane evacuations. In
addition, Ms. Joel also prepared the updated hurricanc evacuation contra-flow implementation plan along
. 1-75, which included Alligator Alley as part of the FDOT District Four ITS General Consultant project.

Professional General Engineering Consultant-
Traflic Engineering
City of Sunny Isles Beach
Robert Skaggs, PE, PTOE, will serve as Traffic Operations Design Leader. Mr. Skaggs has 12 years
experience in traffic engineering and ITS. He served as Project Engineer on the CR 296 Connector from
118th Avenue to North of Ulmerton Road, Pinellas County project, where he prepared plans for
signalization, and signing and pavement marking for this urban roadway. Mr. Skaggs previously served
as Senior Traffic Studies Engineer at the Missouri Department of Transportation (MoDOT), where he
provided oversight of the maintenance and operations of signals, highway lighting, and signing and
pavement marking for a tour-county area. He served as Senior Traffic Engineer for the US 19 Signal
Timing project in St. Petersburg, where he reviewed new corridor signal timing plans and provided
recommendations for improvement. In addition, Mr. Skaggs serves as Project Manager on the FDOT
District Seven Districtwide ITS Consultant contract.
Vicki Castro, PE, PTOE, will serve as Traffic Operations Studies Leader. Ms. Castro has over 17 years
of experience, including eight years with the FDOT as the FDOT District Seven District Safety Engineer.
Ms. Castro has prepared numerous Traffic Impact Analysis reports for projects in Hillsborough, Lake,
Lee, Sarasota, and Seminole counties, and understands the implications of development related to access
management. Ms. Castro serves as Project Manager on V ANUS' recently-selected (second) FDOT
District Seven Districtwide Safety Studies contract. Ms. Castro also serves as Project Manager on the
FDOT District Seven Signal Retiming contract, where she has developed signal timings in Citrus,
Hernando, and Pasco counties. She served as Project Manager on the Collier County, Manatee County,
and Sarasota County signal timing projects. In addition, Ms. Castro is certified in Advanced Maintenance
of Traffic (MOT).
David Krauss will serve as ITS Leader. Mr. Krauss has over 20 years of specialized experience in ITS
and communications, including A TMSs, wireless information systems, video monitoring and data
transmissions systems. He serves as the Project Manager on the Boca Raton A TMS Integration project,
where he is performing design services, equipment testing and implementation assistance for the City's
ITS program. Mr. Krauss served as Senior ITS Manager on the Lee County Incident Management
System (IMS) Design-Build Criteria Package, which included system design for an IMS which covers
four bridges in Lee County. Included in this work are both fiber and wireless systems design. He also
served as Project Manager on the FDOT District Seven ITS General Consultant.
Sara Calhoun, PE, will serve as Senior Transportation Engineer. Ms. Calhoun has over 13 years of
experience. Ms. Calhoun was responsible for the design of five signalization plans for the Collier County
Annual Contract for Traffic Engineering Consultant Services. She serves as Project Manager on the Lee
County On-demand Traffic Engineering Services project, which includes the review and preparation of
ITS plans and specifications. Ms. Calhoun assisted on the FDOT District Three Districtwide
Miscellaneous Traffic Operations Analysis/Design Services project, where she was responsible for the
analysis of roadway corridors and intersections throughout the Florida Panhandle. She developed plans
and prepared control center design requirements for the Lakeland A TMS Design-Build Request for
Proposals (RFP) Package. Ms. Calhoun currently serves as Project Manager on the Pinellas County
A TMS project.
H. Clay Sherrod, PE, will serve as Senior Electrical and Communications Engineer. Mr. Sherrod has
over 50 years of experience in project management, design, field construction, construction management,
and project procurement for transportation and communications systems. He developed the Design-Build
bid package for the Sunshine Skyway Bridge VMS project. Mr. Sherrod served as Chief Engineer on the
1-595 IMS, where he provided conceptual development, preliminary and final design of the
communication and control system to support the deployment of an IMS on 1-595 in Broward County.
He developed a concept for an integrated control center for the City of Boca Raton A TMS Integration
project. Mr. Sherrod developed the communications architecture for the Broward County/Palm Beach
County APTS project. He served as Chief Electrical Engineer on the 1-75/1-595 VMS Phase 2 Design

Traffic Engineering
City of Sunny Isles Beach
Build project, where he was responsible for conceptualizing, designing, field review of available power
service point options, and coordinating installation of electric service with the 3 power service providers
in the area. This project includes installation of traffic monitoring devices at 0.5 mile intervals along the
entire 45 mile length ofI-75 in Broward County.
C. Michael Hoover will serve as a Senior Communications Specialist. Mr. Hoover has over 25 years of
ITS experience. His ITS experience includes the design, implementation and integration of fiber and
wireless communications systems. Mr. Hoover served as Project Manager on the Nashville Regional
ATMS, where he implemented 60 CCTV cameras, 20 DMSs, 140 radar detectors (85 wireless), 12 video
detectors, and 2 HAR stations. He served as the Technical Project Manager on the Knoxville Regional
ATMS Center, which included 72 CCTV cameras, 175 radar detectors (145 wireless), 20 DMSs, and 27
video detectors. Mr. Hoover implemented a regional ITS in Peoria, lIIinois, which consisted of 15 CCTV
cameras, 12 radar detectors, 2 DMSs, and 4 Operations Centers. He also managed all technical aspects of
the 1-75 Fog Detection and Warning System in Charleston, Tennessee, including fiber optic
communications, microwave communications, DMSs, radar traffic flow detectors, backscatter fog
sensors, remote reporting weather station, and control center. He designed, fabricated, and licensed two
BAR broadcast stations.
Phuc Duong, PE, IMSA II, will serve as a Transportation Engineer. Mr. Duong has over eight years of
transportation engineering experience. He served as Project Engineer for the FDOT District Three
Districtwide Miscellaneous Traffic Operations Analysis/Design Services project, where he was involved
in the analysis of several roadways and intersections in the panhandle area of Florida for safety, operation,
and access management. Mr. Duong has prepared numerous traffic impact studies related to
developments within the Tampa Bay area. He served as Transportation Engineer on the US 19 Access
Management Study, where he analyzed 98 full median openings along 19.7 miles of US 19 in Pasco
County. Mr. Duong prepared a corridor study for 46th A venue North between 80th Street and 62nd
Street in St. Petersburg. He also assisted on the FDOT District Seven Causeway Boulevard Corridor
Analysis, where he used CORSIM and HCS 2000 to analyze an urban interchange and fly-over for the
design of the new Causeway Boulevard corridor in Tampa.
Rosana Correa, PE, PTOE, will serve as a Transportation Engineer. Ms. Correa has over eight years of
transportation engineering experience. She serves as Project Engineer on the City of Cape Coral
Downtown CRA Traffic Study, where she is performing analyses of the existing and future conditions
using SYNCHRO, HCS, SIDRA, and TRANSYT-7F. Ms. Correa assisted with a traffic impact study for
an Elementary School and a High School in Polk County. She served as a Traffic Engineer Intern for the
1-75 at SR 54 Preliminary Interchange Operations Analysis Report in Pasco County, where she calculated
design hour volumes and traffic projections, and used HCS and CORSIM to evaluate and simulate the
study area. Ms. Correa is assisting on the Ortiz Avenue project, which includes the preparation of a
traffic study to widen the roadway from SR 82 to SR 80 from a 2-lane undivided to a 4-lane divided
facility.
Christina Florez, PE, will serve as an ITS and Communications Engineer. Ms. Florez has over six years
of ITS and transportation experience, including supporting post design services on the FDOT District Six
SunGuide ICS Package B project along 1-95 in Miami-Dade County. Ms. Florez assisted in the
development of the Design-Build Minimum Functional Requirements and the Estimated Construction
Costs for the Lee County IMS Design-Build Criteria Package and the Jacksonville 1-95 D/B Criteria
Package Development project. Ms. Florez also assisted in the development and review of equipment
specifications and the Phase I design of Broward County's EthernetlIP fiber optic communication
network. She recently led the software testing and evaluation work associated with the FDOT District
Four Broward County A TMS software project, and was an ITS engineering resource for the FDOT
District Four 1-75/1-595 Phase 2 VMS Design-Build project.

Henry Mei, PhD, PE, will serve as a Transportation Engineer. Dr. Mei has over six years of
transportation engineering experience. He served as Transportation Designer on the Collier County On-
call Professional Traffic Engineering Services contract, where he assisted in the preparation of the
signalization plans for five intersections. Dr. Mei prepared the signalization plan for the City of Boca
Raton Military Trail/Lennox Drive Signal Design project. He also prepared communication routing and
signalization plans for City of Punta Gorda/Charlotte County A TMS Design Group 2 project.
Megh Govindu, EI, Transportation Engineer Intern, has over five years of traffic engineering experience.
Mr. Govindu served as Transportation Engineer Intern on the Magnolia Landing Access Management
project, where he analyzed access alternatives and recommended an access point for Magnolia Landing
Property that is anticipated to improve access to the property and enhance traffic flow along US 41. He
has prepared DRIs for numerous mixed-use developments, including Bryan Paul in Hendry County and
Tradeport in Collier County. Mr. Govindu served as Transportation Engineer Intern for various Lee
County signal design projects, including US 411Palm Drive, the Gulf Coast Town Center Internal Signal,
and the Miramar Mall North Entrance Signal. He is assisting on the Ortiz A venue project, which includes
preparation of a traffic study to widen Ortiz Avenue from a two-lane undivided to a four-lane divided
roadway.
Suhasini Karyamapudi, EI, Transportation Engineer Intern, has over one year of experience. Ms.
Karyamapudi developed a traffic impact study for the Oak Grove Village Mixed-use Development project
in Pasco County. She determined the future traffic conditions for roadway links and intersections within
the study area; analyzed the capacity of the roadways and intersections using FDOT's Generalized
Capacity Tables, SYNCHRO and HCS; and determined whether auxiliary lanes were warranted based on
County criteria and sized the turn bays accordingly. She served as Transportation Engineer Intern on the
Sweetbay Development Traffic Impact Analysis, where she developed a traffic impact study based on
Hillsborough County guidelines. Ms. Karyamapudi assisted on the City of Cape Coral Downtown CRA
Traffic Study, where she created crash summaries and used Microstation to prepare collision diagrams.
She is currently assisting in the preparation of a traffic operations study and signal planning for a roadway
reconstruction project in Kenneth City.
Damien Rose, EI, Transportation Engineer Intern, has over three years of experience. Mr. Rose serves as
Transportation Engineer Intern on the FOOT District Six Districtwide Traffic Operations Safety Studies
Consultant Contract where he assists in the analysis of crash data and the preparation of Safety Study
reports for the 2002 Districtwide High Crash list, in accordance with FDOT requirements and procedures.
He currently serves as Transportation Engineer Intern on the FDOT District Six Miscellaneous Traffic
Operations Studies Consultant Services contract, where he performs field review and crash analyses, and
develops reports for those locations.
Vikas Saxena, Transportation Designer, has over one year of experience. Mr. Saxena serves as
Transportation Designer on the FDOT District Six Districtwide Traffic Operations Safety Studies
Consultant Contract where he performs various analyses including crash reviews and level of services,
site reviews, conducted field inventories, and assisted in the development of study reports. He served as
Transportation Engineer Intern on the FDOT District Four District-wide ITS General Consultant Services
where he assisted in the development of drop down gate plans for 1-75 contra-flow operations and
performed site reviews. Mr. Saxena also assisted in performing "before" and "after" signal timing
evaluation runs for various corridors on the Broward County Signal Retiming Contract.
Richard Novell, IMSA II, Transportation Designer, has over 16 years of traffic operations and
transportation engineering experience. He served as Designer for the District Seven Districtwide Traffic
Operations project, where he conducted crash analyses and volume and vehicle turning movement counts
for numerous traffic studies. Mr. Novell prepared collision and condition diagrams for traffic reports,

Parkway project. Mr. Novell also conducted field reviews at 408 intersections within 12 city
neighborhoods for the City of Tampa Neighborhood Speed Reduction project.
Jim Gugliotti, IMSA II, Senior ITS Technician, has over 26 years of experience. He is an electronics
expert that can install and repair any signal equipment presently used in the State of Florida. Mr.
Gugliotti conducted timing analyses and fine-tuning for the Broward, Collier, Manatee, Pinellas, Pasco,
and Hillsborough counties retiming projects. He performed construction CEI duties for the installation of
175 511 signs for the Tampa Bay A TIS - 511 Services project. Mr. Gugliotti served as Senior ITS
Technician on the Sunshine Skyway Bridge VMS, where he inspected and tested 21 CCTV cameras, a
fiber optic cable network, and the central hardware. Mr. Gugliotti recently conducted field reviews for
the City of Tampa Residential Speed Limit Sign project and the FDOT District Seven Safe Routes to
School Sidewalk project.

and (if applicable) its Federal Employer Identification Number (FEIN) is 58-1636967

2. 1 understand that a "public entity crime" as defined in Paragraph 287. 133(l)(g), Florida Statutes,
means a violation of any state or federal law by a person with respect to and directly related to the
transaction of business with any public entity or with an agency or political subdivision of any
other state or of the United States, including, but not limited to, any bid or contract for goods or
services to be provided to any public entity or an agency or political subdivision of any other state
or of the United States and involving antitrust, fraud, theft, bribery, collusion, racketeering,
conspiracy, or material misrepresentation.
3. 1 understand that "convicted" or "conviction" as defined in Paragraph 287. 133(l)(b), Florida
Statutes means a finding of guilt or a conviction of a public entity crime, with or without an
adjudication of guilt, in any federal or state trial court of record relating to charges brought by
indictment or information after July I, 1989, as a result of a jury verdict, nonjury trial, or entry of a
plea of guilty or nolo contenders.
4. I understand that an "affiliate" as defined in Paragraph 287.133(1)(a), Florida Statutes, means:
a. A predecessor or successor of a person convicted of a public entity crime; or
b. An entity under the control of any natural person who is active in the management of the entity
and who has been convicted of a public entity come. The term "affiliate" includes those
officers, directors, executives, partners, shareholders, employees, members, and agents who are
active in the management of an affiliate. The ownership by one person of shares constituting a
controlling interest in another person, or a pooling of equipment or income among persons
when not for fair market value under an arm's length agreement, shall be a prima facie case that
one person controls another person. A person who knowingly enters into a joint venture with a
person who has been convicted of a public entity crime in Florida during the preceding 36
months shall be considered an affiliate.
5. I understand that a "person" as defined in Paragraph 287. I 33(l)(e), Florida Statutes, means any
natural person or entity organized under the laws of any state or of the United States with the
legal power to enter into a binding contract and which bids or applies to bid on contracts for the
provision of goods or services let by a public entity, or which otherwise transacts or applies to
transact business with a public entity. The term "person" includes those officers, directors,
executives, partners, shareholders, employees, members, and agents who are active in
management of any entity.

6. Based on information and belief, the statement which I have marked below is true in a relation to
the entity submitting this sworn statement. (Please indicate which one (l) of the following three
