BUSINESS HISTORY
Keith and Associates, Inc. was incorporated in 1998 and has since provided
Redevelopment and Civil Engineering, Construction Engineering Inspections and
Certification Services, Urban and Comprehensive Planning, Feasibility Studies, Land
Surveying and Mapping / G.P.S., Platting and GIS Services.
In 2007, Keith and Associates, Inc. was certified as a
Minority Woman Business Enterprise (M/WBE)
Owners of Keith and Associates, Inc. previously founded and operated other
businesses known as:
Keith and Schnars, P.A. that was incorporated in 1972, offering engineering,
planning, environmental, surveying, and mapping services.
Wiggins, Keith and Associates, Inc. that was incorporated in 1929, offering
engineering and surveying services.
FIRM FINANCIAL STATUS
Keith & Associates, Inc. has a nine (9) year history of sound financial management. Each
year the company hires an independent CPA to perform an Audited Return for the company,
which continues to show prudent financial operations. The firm currently has no long term
debt and pays all it's bills upon receipt.

STATEMENT OF QUALIFICATIONS
The K&A Team would be pleased to provide continuing General Engineering Consultant
Services including surveying, construction management; construction inspection,
construction contract plans, public involvement, post design services, bid administration,
hurricane recovery and debris monitoring services, and general design work services that
may arise.
CIVIL ENGINEERING:
K&A has an extensive experience in
providing professional services required
for the development or redevelopment of
land including the permitting, design and
construction of stormwater, potable water,
and wastewater systems. Recognizing
that each development or redevelopment
project has its own unique site issues,
K&A performs a thorough investigation
into these issues in order to avoid
planning, design and construction issues.
Our engineers have the knowledge and
expertise to meet the needs of a wide
variety of general civil engineering issues.
K&A Staff has provided comprehensive
planning and engineering services in
various disciplines involving many
government agencies and municipalities.
Our ability to work with municipalities,
government agencies and other
consultants while providing close
coordination with the client will result in
projects being completed on time and
within budget. K&A has found great

has lead to effectively identifying problems

STORMWATER MANAGEMENT:
K&A has the knowledge and proven
abilities to understand the complex nature
of stormwater management. Especially in
the urban environment, where balance
must be achieved between the permitting
requirements, restricted positive outfalls,
budget limitations and existing historical
drainage patterns. Our team has the
professional acumen to fully investigate
the existing conditions and propose
creative solutions that will resolve and
simplify the feasible alternatives. In
addition, we are familiar with the National
Pollutant Discharge Elimination System
(NPDES) Program and the requirements
to implement it.
PUBLIC INVOLVEMENT:
K&A recognizes that among the many
contributing factors to a successful
project, pubic involvement and consensus
building are critical elements and we try to
incorporate them into every major project.
Our public involvement program has been
extremely successful in establishing lines
of communication among the affected
parties so that everyone has an
opportunity to contribute towards the

facilitated charrettes for numerous
municipal projects to gain awareness of
community preferences and concerns on
upcoming developments. On a number of

LEED (Leadership in Energy & Environmental Design):
The Leadership in Energy and Environmental Design (LEE D) Green
Building Rating System ™ is the nationally accepted benchmark for
the design, construction, and operation of high performance green buildings. LEED gives
building owners and operators the tools they need to have an immediate and measurable
impact on their buildings' performance. LEED promotes a whole-building approach to
sustainability by recognizing performance in five key areas of human and environmental
health: sustainable site development, water savings, energy efficiency, materials selection,
and indoor environmental quality.
C8 K&A is a member of U.S. Green Building Council (USGBC) and is in the
o ft process of obtaining LEED Professional Accreditation for our entire
~.J.~ engineering and planning staff. We are currently providing integrated
. engineering design components on a number of public facilities to facilitate a
designation of a LEED-Certified Building.
PERMITTING:
Permitting to facilitate construction of improvements is a critical element in the overall project
management process. Permitting can be extremely time consuming and may result in
project delays and cost overruns. It is therefore critical that members of the design team
work closely with the client, other consultants and the respective agencies to identify issues
of importance to regulatory agencies during early stages of the design process.
To facilitate an orderly flow of our design services, K&A utilizes a hands-on approach,
meeting early in the process with any regulatory agencies having jurisdiction over the site to
evaluate their standards and assess the issues relative to their approval process. Our
project approach includes permitting as a specific task in our scheduling process and given
the magnitude of the assignment, schedules may be updated more frequently to assure
compliance with the overall project schedule. K&A is routinely familiar with the permitting
process and our experience in dealing with the regulatory agencies enables us to secure the
necessary permits in a most expeditious manner.
CONSTRUCT ABILITY ANALYSIS,
CONSTRUCTION ADMINISTRATION AND INSPECTION:
A significant element of every project is the constructability analysis. K&A's QC Engineer
has the critical role of performing a thorough review of all final engineering design plans to
ensure that the project team's services are being rendered in strict conformance with our
Quality Assurance 1 Quality Control Program. The depth of the QC reviews will be sufficient
to assure the design plans adequately provide the information required to construct the
project. The QC reviews will assure that the team is complying with all City requirements
and that the project documents reflect that compliance. K&A currently provides engineering
plan review services for the City of Deerfield Beach and the Seminole Tribe of Florida Roads
Department.

STATEMENT OF QUALIFICATIONS
K&A has provided construction services for projects ranging from
residential communities, commercial projects and recreational parks
including roadways, parking lots, water mains, sanitary sewers,
storm sewers, lakes, retention areas, driveways and other civil
engineered facilities. The best construction management practice is to maintain a close
working relationship with all parties involved and to respond quickly and effectively to any
issues that may arise. This approach coupled with our experience and knowledge, offers our
clients the opportunity to minimize cost overruns and delays during construction.
SURVEYING & MAPPING:
The expertise of our Land Surveying staff is evidenced by Dodie Keith-Lazowick and Greg
Mire's combined experience (over 60 years) of surveying in the South Florida area. This
experience has resulted in a tremendous database of knowledge and information. The
ability to offer in-house surveying and
mapping capabilities provides for a more
comprehensive unified team. Services include
boundary, topographic, control, wetland,
mitigation, bathymetric, GPS, as-built,
American Land Title and coastal surveys,
legal descriptions, right-of-way mapping,
design base sheets, title review, DTMs,
differential leveling, construction stakeout,
platting, expert witness surveying, and
mapping services.

GEOGRAPHIC INFORMATION SYSTEM (GIS):
As previously indicated, K&A believes in efficiency and the utilization of state-of-the-art
technology to service our clients. Our Geographic Information System (GIS), running
ARCIINFO software, was specifically chosen to match the systems used by many of our
public sector clients to ensure compatibility of information during the permitting process.
This system incorporates all aspects of city governmental functions, from police and fire
through to the monitoring of the utility system, also including detailed information relative to
site and landscape architectural plans being processed through the City.
POST STORM RECOVERY:
K&A has experience in providing total project management services for hurricane
preparedness and post storm recovery. These services include preparation 48 hours
prior to storm events, initiation of a 72-hour emergency response after the storm,
preparation and submittal of applications to FEMA and NRCS, well as management of
the debris removal effort to insure compliance with funding requirements. K&A is
currently providing Disaster Recovery services to the City of Fort Lauderdale.

Robert McSweeney, P.E., Vice President

standards. Mr. McSweeney has more than
17 years experience in land development,
utilities, roadway engineering and
construction. His work knowledge
includes civil related engineering,

McSweeney's broad background entails
employment with civil and environmental
engineering firms providing service to both
public and private sector clients in Miami-
Dade, Broward and Palm Beach Counties.
His experience also involves FDOT Utility
Coordination, FDOT construction
administration and designlconsulting for
the U.S. Air Force.
Director of EnQineerinQ:
Charles DaBrusco, P.E. will manage
overseeing specific project design
activities. As a Professional Engineer, he
has 26-years of experience in municipal
engineering and private sector residential
project design and construction. His
experience encompasses many areas
such as general civil engineering services,
design plan reviews, public works
projects, inspections, roadway
improvements, recreational facilities,
safety improvements, drainage, paving
and grading, utility infrastructure,
permitting and construction administration.
He has served as Senior Project Manager
for a number of contracts and completed
numerous projects for various agencies.
He is currently providing Professional
Engineering Plan Reviews and
Constructability Assessments under an
ongoing contract with the Seminole Tribe
of Florida's Roads Department and the
City of Deerfield Beach. Mr. DaBrusco's
years of experience and his particular
emphasis on community engineering and
construction projects will provide the
leadership necessary to provide the
orderly execution of this contract.

Mark Castano and Traci Scheppske
have been assigned the critical role of
Senior Project Engineers. They have
extensive technical knowledge in water
distribution systems, stormwater
management systems and sanitary sewer
systems including sewer lift stations,
gravity sewers and force mains, roadway
design and permitting throughout South
Florida. As Senior Project Engineers with
over 30-years of experience in Civil
Engineering projects between them, they
will be responsible for all engineering
design elements of the projects as well as

STAFF EXPERIENCE AND QUALIFICATIONS
Director of SurvevinQ
Gregory Mire, P .S.M will coordinate our survey department efforts
to meet project goals. Mr. Mire has over 38 years of experience in
all aspects of surveying operations related to various municipal, commercial and residential
projects in Miami-Dade, Broward, Indian River and Palm Beach Counties. His expertise
includes the calculation and preparation exhibits to be utilized in granting of easement rights
and property rights. He coordinates these activities with state and local authorities for road
related projects and with utility providers for land development projects. Mr. Mire's
experience includes the representation before public bodies of these and similar elements
requiring governmental approvals. His experience includes over 18 years of field work on a
wide range of construction projects from high rise to underground construction.
Director of PlanninQ
Michael Vonder Meulen, AICP will contribute his expertise of over 20 years of local,
national and international experience in urban planning, zoning and project management
systems to any of the project needs. Since 1986, Mr. Vonder Meulen's experience has
included many phases of Public Sector and Private Sector Land Planning, Site Planning and
Permitting services. As a Planner and Project Coordinator, he has directed project services,
from planning, design and construction management throughout the United States,
Caribbean and Brazil. He has drafted numerous zoning text amendments for many Cities
throughout South Florida including new and revised residential, commercial, educational,
mixed use and transit oriented districts, density monitoring systems (DMS) and transfer of
development rights (TDRs). Mr. Vonder Meulen also has extensive comprehensive planning
experience preparing many Comprehensive Plans, Evaluation and Appraisal Reports
(EARs) and Comprehensive Plan updates and amendments.

Deerfield Beach. Objective was to resolve significant drainage issues and provide traffic

construction engineering inspection and extensive

S-Curve Phase III, Beach Area Conceptual Master Plan,
Deerfield Beach CRA: Project Area is bounded on the north by the
BrowardlPalm Beach County line, on the south by SE 3rd Street, on the west by SE 19th
Avenue and on the east by Ocean Way. Project included base map indicating record plats,
street names and widths, address and ownership data pursuant to the records maintained
by the Broward County Property Appraiser's Office, conceptual drainage analysis, analyzing
the existing drainage areas and addressing the proposed project
improvements and their impact on the existing drainage
conditions, and numerous public workshops. Project to be
completed in 2007. Project Budget $129,000.00

Harbor Drive, Pompano Beach CRA: Project included complete engineering design and
topographic surveying for a 2-mile portion of NE 26th Avenue (Harbor Drive) from Atlantic
Boulevard to NE 14th Street in the City of Pompano Beach.
Specific project details involved design for full service roadway,
replacement of aging utilities, beautification and traffic calming.
The project was completed 2 months ahead of
1 schedule with installation of over 30,000 linear feet

of utilities including 6" to 36" force mains; a lift
station, reclaimed water mains and drainage
infrastructure installation. Traffic calming measures
were implemented without adversely impacting
adjacent single-family properties. The roadway
reconstruction of the existing two-lane roadway
I included landscape medians and roundabouts
throughout the corridor. Project also included permitting,
construction engineering inspection and administration
services, and extensive community liaison services. Keith
and Associates, Inc's engineering design team provided
community relations services throughout the 24 month
construction by establishing a project information hot line to
address resident concerns immediately alleviating construction issues and resident calls into
city hall. Project completed in 2005 - project budget $8.6 million.

1. This sworn statement is submitted to City of Sunny Isles Beach, Florida
by Keith & Associates, Inc.
for RFQ No. 07-07-01, Professional Engineering Consultants
whose business address is 301 East Atlantic Boulevard, Pompano Beach, Florida 33060
and (if applicable) its Federal Employer Identification Number (FEIN) is 65-0806421

2. I understand that a "public entity crime" as defined in Paragraph 287.133(l)(g), Florida Statutes,
means a violation of any state or federal law by a person with respect to and directly related to the
transaction of business with any public entity or with an agency or political subdivision of any
other state or of the United States, including, but not limited to, any bid or contract for goods or
services to be provided to any public entity or an agency or political subdivision of any other state
or of the United States and involving antitrust, fraud, theft, bribery, collusion, racketeering,
conspiracy, or material misrepresentation.
3. 1 understand that "convicted" or "conviction" as defined in Paragraph 287.133(1)(b), Florida
Statutes means a finding of guilt or a conviction of a public entity crime, with or without an
adjudication of guilt, in any federal or state trial court of record relating to charges brought by
indictment or information after July I, 1989, as a result of a jury verdict, nonjury trial, or entry of a
plea of guilty or nolo contenders.
4. I understand that an "affiliate" as defined in Paragraph 287 .1 33(1)(a), Florida Statutes, means:
a. A predecessor or successor of a person convicted of a public entity crime; or
b. An entity Wlder the control of any natural person who is active in the management of the entity
and who has been convicted of a public entity come, The term "affiliate" includes those
officers, directors, executives, partners, shareholders, employees, members, and agents who are
active in the management of an affiliate. The ownership by one person of shares constituting a
controlling interest in another person, or a pooling of equipment or income among persons
when not for fair market value tu1der an arm's length agreement, shall be a prima facie case that
one person controls another person. A person who knowingly enters into a joint venture with a
person who has been convicted of a public entity crime in Florida during the preceding 36
months shall be considered an affiliate.
5. I tu1derstand that a "person" as defined in Paragraph 287. 133 (l)(e), Florida Statutes, means any
natural person or entity organized tu1der the laws of any state or of the United States with the
legal power to enter into a binding contract and which bids or applies to bid on contracts for the
provision of goods or services let by a public entity, or which otherwise transacts or applies to
transact business with a public entity. The term "person" includes those officers, directors,
executives, partners, shareholders, employees, members, and agents who are active in
management of any entity,
