This report presents a brief description of the field procedures, and the results of the exfiltration test. Exfiltration tests were performed to a depth of twenty (20) feet below existing grade in accordance with South Florida Water Management District (SFWMD) criteria for 'Usual Open-Hole' conditions. Initially, it was proposed to perform the exfiltration tests to fifteen feet. However, due to the low permeability at this depth, it was decided to extend the depth of the test to twenty feet. Prior to starting the tests, a 6-inch diameter hole was augered to the test depth to determine the depth to groundwater and to examine subgrade soils. After establishing the above parameters, the hole was stabilized by a full-length perforated PVC pipe in accordance with South Florida Water Management District specifications. Water was then pumped into the hole maintaining a constant water level at the ground surface, The stabilized flow rates were recorded in one minute intervals for a total of 10m inutes. The exfiltration tests revealed the hydraulic conductivity ('K'-value) of the soils ranged from 3.85 X 10-5 to 8.08 X 10-5 cubic feet per second per square foot per foot of head, Soil descriptions and flow rates for the tests are shown on the attached exfiltration summary sheet. We note that the water table was encountered at depths in the range of approximately 2.0 to 3.5 feet below the existing ground surface. Sabrina Baglieri, Construction Services, Calvin Giordano and Associates, handed out Addendum No.1 which included Plan Sheets that replaces the original Plan given out with the Bid Package, and the North Miami Beach Plan Sheets 1 thru 9 to be part of Alternate No.1 of the Bid Schedule. Whoever has not picked up plans yet, can pick them up tomorrow from the City Clerk's Office and they will include Addendum No.1. Deadline for submission of bid documents is Monday, June 16,2008 at 11:30 a.m. at which time those sealed bids will be publicly opened. Any additional questions are required to be in writing and may be submitted up to five (5) days before the bid opening (Wednesday, June 11,2008), which will be answered and forwarded to all of today's participants. You may email them to Sabrina Baglieri at sbaglieri(cV,calvin-giordano.com Ms. Baglieri noted that we are looking at awarding this project at the July 17,2008 City Commission meeting, and after it is awarded, the contractor will have about 30 days to process all the contract documents, bonds, insurance, etc., and we are looking to issue the Notice to Proceed by mid August. She presented an overall description ofthe project, and encouraged everyone to review Section 500 and Section 100 instructions to bidder regarding insurance requirements. She noted that this is a critical project in time lines, and has very high liquidated damages after substantial completion, and $2,000 per day until final completion, and for the first time, we have an early completion bonus of $2,000 not to exceed $60,000. Ms. Baglieri announced that a revised Section 300 is going to be issued as Addendum No.2 that we are going to fax to everyone, including the summary minutes of today' s meeting. She noted that the requirements should not change too much but we do want to make some adjustments based on the revised plans, and also we have the North Miami Beach (NMB) Plan Sheets as an alternative item, which is for installation only, NMB has not made a decision as they are coming up with a new water main. We will add an allowance number for doing the layout of the undergrounding for FPL. NMB will be separate because it is not part of the base bid depending on how the numbers will come, NMB will decide if they will go with your numbers or if they will do it themselves. Ms. Baglier introduced Engineer Ronnie Navarro also of Calvin Giordano and Associates, who went over the technical points of the project. He said Addendum No.1 has a new set of specifications to be in accordance with DERM and W ASA comments, but we are good on the gravity sewer. For the drainage system we have taken out most of the swales, you are not going into a drainage swale but a cortex system as a solution for the stormwater system. We will be using a rubber checkmark for that called a pipeplex, all crossing pipes should be ICP, and that is the change. QUESTIONS AND COMMENTS FROM PROSPECTIVE BIDDERS 1. Budget for job? $3.5 Million Dollars, that docs not includc the altcrnatc or thc allowancc. 2. On the schedule of values you showed the allowance line I-52? The wholc Section 300 is going to be revised and reissucd as an addendum. 3. Clarify the Alternate, the alternate can go to any other contractor? No, we want the same contractor to do all the work. 4. Then when we evaluate the price, we are not going to worry about the alternate whether it is high or low? No, don't worry about the altcrnate. That will be done on the base bid. 5. The Performance Bond, does that include the alternate if you are awarded? Yes if you are awarded, but your bid bond will only be 5%1 of your base bid. 6. That $3.5 Million Dollars estimate is without alternate. Yes. 7. Will the FPL be included as an alternate? It is going to be an allowance because we don't have plans yet from FPL, and so when Section 300 is rcissued there will bc an allowance item. It will bc basically the duct work, just thc conduits and the lines will be donc by FPL 8. Do you have to be an FPL approved contractor to do that part of the work? No not for thc conduits, but that will bc verified and it will be added to the Addendum. 9. Does FPL supply the conduits? Yes and they supply thc design as wcll which we don't have at this time. They will also come up with the number for the estimate, and we will put that as an allowance item. 10. Does FPL do the Street Lighting? No. That is part of this project, not FPL, and it is in Addendum No.1. FPL has the plans done but we are waiting on a price from them on the cost. FPL is doing the service line, overhead lines that they have for the house ifthey want to go underground, ~md so basically we are going to put the conduits in the boxes so they can come up with the wiring and connect all the house underground so the overheads will be removed. 11. Remove light poles, according to the City all were to be relocated? At the beginning there was discussion as to what type of poles that the residents would like, and again, Section 300 will be reissued as an Addendum. Also, we are talking about some 60 homes there, everyone is tying into the sewer, everyone has accepted 08-06-01 Pre-Bid Conference QA Page 2 of 7 this, and we are still working it out to sec if we can get one contractor to pull the scptic and tic into thc sewers, etc., at a level price. That is not part of the contract but it is something that might be included for whoever is choscn or we may have to go out to bid. We have to talk to the City Attorney's office regarding that. 12. Do the new plans have light fixtures? Yes, there is an automatic lighting layout and pictures with specifications, and that is mainly why Scction 300 is being reissued as an Addcndum. 13. When we do the demolition, the driveways will be affected and it will be very difficult to match the driveways. All the owners havc been put on notice, we told them what is right-of-way, if they have pavers and somc of thcm have started to remove their pavers out of the way, anything that is in the right-of-way that they want to keep they should remove, and if they are not removed by the time we start the project, go ahead and bulldoze it out. The replacement is there, the first tcn feet is asphalt and all uniform, and from that point on they are going to put in their own pavers. If you damagc anything on private property then it will have to be replaced. 14. Do the new plans have a clear definition of what you want in restoration, as in the previous set the demolition plans did not match the restoration plans? We will double check and include in Addendum. 15. Information on soil bars. Information is in Section 210C, information will be put in Addendum 2. 16. On restoration, will you make it clear whether or not we have to restore the right-of-way? Yes. Anything outside the area is the contractor's responsibility to restore it, i.e. knock down a tree on private property. W c did something similar in Golden Shores where we redid the drainage areas and we came to an understanding that the contractor would restore it at whatevcr cost it was to the owner. Right now the first ten feet is yours. Thcre is ~l temporary construction casement within their property so you are able to work in that arca so you can match the driveways. 17. There is not a pipeline going through the swale area? You need to follow whatever thc plan says, the whole idca is that you will re-grade it so the drainagc will work. 18. How many days for the project? 160 days for project completion. 19. Is that enough time to allow access for the homeowners? Yes. Maintenance of traffic: this is the only way in and out of this area. The homeowners undcrstand that there will be some certain times or hours that you will have to block a lane, but you must have constant communication with them and the goal is to have at least one lane open at all times. After 5:00 p.m. and you are finished for the day, make sure it is accessiblc as well as to the houscs. 20. What are the hours for noise? City Ordinance is from 7:00 a.m. to 7:00 p.m. Monday through Friday, 9:00 a.m. to 5:00 p.m. on Saturday, and no Sundays or Holidays. City can make some cxtensions. 21. Allowances for bringing in materials? We have DOT fabric, we calculated that on-line items but we will check and add to Addendum. 22. Testing? We are going to concentrate more on the sewcr to make surc it has a good bedding. Road wise, that road has been there a while, so hopefully it is already pretty well compacted except the widening of the roads because of the curbing. 23. Do you have definitions as far as how deep. We are still debating but about 3-feet. 24. Trench as well? .Just for the 25. Do you have a special Control Plan for the middle while we are working there? Fencc around it. 26. Staging area? Ycs. 27. Coding for the lift station and the manholes, any special liner? Everything regarding the sewer is based on Miami-Dade Water and Sewer Standards, so just follow that. 28. Compaction testing is the contractor's responsibility and we have to pay for them according to contract documents? Yes, any type of tcsting that needs to be submitted signed and scalcd by your lab, and should bc included in your costs. 29. Do you want per lift on the base, three for every 1,000 feet, two-tiers, sub-base will be expensive. Yes, we will confirm exactly how many tests we are looking for. 30. What is the holiday schedule? Pipe trench envelopes, you had talked about 6-inches below, is that as far as you are looking for mucking, or do we need to go beyond that? The specs will state that, usually the pipes go down until you find the material. 32. What is the definition, just until you find the muck/material? We will dcfine what the worst scenario will be for us and add to the Addendum. 33. Are there any wells? No. All the designs, permits will be placed by the time of the bid opening C.June 16,2(08), the contractor is going to be responsible for ~lDY additional cost of permits, you need to get your dewatering permits, etc., and so it is the contractor's responsibility to obtain it and pay for it, so those fees should be included. 34. Will those fees be reimbursed by the City? No, you need to put it in you number. 35. Sprinklers on Collins Avenue, Manhole, is that a DOT right-of-way? Yes, and the City will apply for the spccial permit for connection, but it will be the contractor's responsibility for the MOT on lane closures, etc. 36. Do you have to be prequalified by DOT to do that work? No. 37. Do we attach that force main to the outside of bridge? It will be a terminating manhole prior to W ASA's existing manhole, it is a drop manhole. 38. Do you have to go over the bridge? Yes. The pipe will be on top of the deck. There is a detail in plans. 39. Inside or outside of wall? Outside. 40. Can we get trucks and equipment over the bridge? Everything is ol<.ay for those two front bridges. The only bridge that you can't use is the interior small bridge that goes over the little lagoon, it is historic. Do not touch it. 41. Pump station, lift. We are still in the process of application for the lift station. County standards need to be followed. 42. For W ASA we have to have proof of bills before starting the countdown on delivery, can you accelerate that process? Not at this time unless we cncounter somc kind of problem with permits, etc. 44. Will you extend time for the contract if conduits are not in place? Wc are open to any of your input. 45. Are we going to make a Site Visit? No, not today. 46. You mentioned that in the right-of-way there is an additional 1 O-foot construction easement? Yes, just to go insidc their propcrty but you probably won't necd it, it is in place as a safeguard. 47. You are saying that there are agreements in place, everything is in writing? Yes, we have permission to be thcre. 48. The clean house that needs to be installed, install in front of property line? The clean house for when you change to scptic, .iust at the border. It is in the plan and is W ASA standard. 49. Striping? Yes, there should be a pavement marking signs. 50. How many? Will be in plans, a lump sum, page 300-04. We want to make sure you are familiar with this area becausc you will have water problems, you will havc to figurc out what to do with thc water. It is in the contractor's ballpark, wc are not going to tell you the means and methods but we arc going to tell you if )'OU are not doing it right. 51. You said you will not get into means and methods, and so you are alright with wet installations? Ycs. 52. Milling and Overlay: in the metric pavement, cost for %-Inch of milling? We will take a look at it and that item will bc changed. 53. Any septic tanks in front? We don't have the location of the scptic tanks at all, cach one is individual. The propcrty owncr has to have it insidc their property line and you won't be going into their property at all. You have a swale arca of around 15 to 20 feet and you should stay within that. We necd to make surc you submit MOT plans for approval within the City, it is City standards whcn you are down to one lanc you will have to have thc Police in therc. If you are going to obstruct the entrance on Collins and have lane closures you may have to have Police, but we will meet with Robert Solera who is in charge of traffic here. 54. A special time to perform work in that section there? As quickly as possible, the quicker the better, the City does not have any set time but you might want to do it at night to ~llleviate connections, etc. This is why you must bring your MOT to DOT because we don't know how much you will need to encroach in there. 55. What color for sidewalk? No sidewalk there at all. We would like to get feedback from you on how you envision this project going along, opening and closing sections as you go, or gutting it out all at once. It is depending on time constraints, it would be faster to gut it all out at once, but we don't want any holes. 56. In October there is high tide, would you consider another time, as the water is a foot higher than it is now. The Mayor, an ex-Commissioner, and a new Commissioner lives there, and we would like to give them a realistic timeline. If you piecemeal it, it is going to take longer, but if you tear it up all at once you may be able to do it faster. It is either or. 57. Will we get a copy of the sign-in sheets?
