This atlas includes FDOT ROW which should not be included as part of the proposal. • Do we have to have the insurance for the bid or if awarded the bid, can we get the insurance within like 5 days? Insurance requirements must be met upon award. Request for Information: • Please clarify the amount of catch basins and man holes that need to be cleaned out. The bid line item is only for catch basins, are you going to add a line item for manholes? Manholes and catch basins are two different items and may take different methods of cleaning. The bid is only for catch basins. Per section 4.0 of the bid document there will be a total of 548 catch basins to be cleaned. This number includes the needs of all three (3) participating cities per section 2.19 of the bid document. • Bid Form has 598 basins and 37,400 LF of jetting, yet elsewhere you have 26,200 LF of pipeline and 464 structures. Can you please provide information with regard to actual quantities and pipe sizes of Storm Water Drainage Pipe and Drainage structures? 1"he response to #2 lies within the body of the atlas (attached). The numbers are correct as stated subsequently. The lower quantities are the ones we will use to determine the unit price. END OF SECTION i { BID SUBMITTAL FORM Bid Title: STORM WATER CATCH BASIN CLEANING & LINE JETTING The undersigned Proposer proposes and agrees, if this Bid is accepted, to enter into an agreement with The City of Sunny Isles Beach in the form included in the Contract Documents to perform and furnish all Work as specified or indicated in the Contract Documents for the Contract Price and within the Contract Time indicated in this Bid and in accordance with the other terms and conditions of the Contract Documents. The Proposer accepts all of the terms and conditions of the Advertisement or Invitation to Bid and Instructions to Proposers, including without limitation those dealing with the disposition of Bid Security. This Bid will remain subject to acceptance for 90 days after the day of Bid opening. The Proposer agree to sign and submit the Agreement with the Bonds and other documents required by the Bidding Requirements within ten days after the date of the City's Notice of Award. In submitting this Bid, the Proposer represents, as more fully set forth in the Agreement, that: ■ The Proposer has familiarized himself /herself with the nature and extent of the Contract Documents, Work, site, locality, and all local conditions and Law and Regulations that in any manner may affect cost, progress, performance, or furnishing of the Work.
