Office Location and General Information
Cross Construction Services, Inc., is a certified asbestos abatement contractor and a
certified building and general contractor. CCS is located just off Interstate 75 in Lutz,
FL with a 20,000 SF office/warehouse. We opened our second location in Panama
City, FL in April 2008 to serve the Gulfcoast and our neighboring states. Both offices
have specialized equipment and inventory is housed at each location. In April 2009 we
opened a satellite location in West Palm Beach, FL office to serve south Florida. CCS
can mobilize all environmental and demolition projects from both locations and work the
entire southeast United States.

We coordinate our projects according to EPA regulations, project size/crew size,
equipment needed, etc. and can mobilize to emergency situations in a matter of hours.
All of our management and supervisors are manned with cell phones communication
equipment and are available 24 hours a day.

President
Background and Experience:
Mr. Arney started with Cross Construction Co. in 1981 while still attending high school
and college. He trained as an apprentice to asbestos workers performing various tasks
associated with the abatement industry. He attended Retra Services, Inc. (an accredited
facility representing University of Florida/Treeo Center) and completed the AHERA
certification as and asbestos worker. After further experience working on crews, Mr.
Arney was promoted to Project Supervisor in 1988, coordinating work crews of 10 to 50
men. In 1991 he was promoted to Project Estimator, responsible for the preparation of
bid proposals.
Becoming Vice President in 1992, Mr. Arney was responsible for estimating and
preparation of bids, pre-bid attendance, specification review and take-off, coordination of
project schedules, cost control and scheduling of supervisory personnel for projects.
In 1996 Cross Construction Company incorporated Cross Construction Services, Inc. to
exclusively handle the environmental division. In 1999 Rusty became President ofCCS.
In 2004 CCC and CCS again merged to offer turn-key services to our clients for all their
environmental requirements as well as demolition services. In 2008 Rusty became
Owner of CCS and CCC and continues as President of our firms.

Experience Background:

Tony joined Cross Construction as an asbestos abatement technician and assisted
on projects where he excelled in his understanding of the environmental industry
and its potential for innovative abatement/remediation procedures. In the latter
part of 1990, Tony was promoted to Project Supervisor for his ability to take the
initiative and "get the job done". He was assigned many of our specialty abatement
projects; i.e., telephone switch stations, hospitals, schoqls, libraries and court-
houses. In 1998 he was promoted to Project Manager and in 1999 Tony successfully
passed the State of Florida Certified General Contractors test. In 2000 he was
promoted to Vice President. A portion of his duties include:

Tyler joined Cross Construction in June of 1991 with the desire to lean the demolition
industry. With a willingness to learn he started at the entry level laborer position on
demolition sites and progressed to heavy machinery operator and then foreman. In 2006
he was promoted to Vice President of demolition for the company and is responsible for
all phases of work from the big process all the way to final completion. He has also been
involved in the remediation side of the company and has a working knowledge of OSHA,
EP A and state regulations.

Background:
Skip managed the asbestos program for the State of Florida during the late 1980's
and 1990's overseeing each phase of asbestos remediation for all State owned
buildings. He then taught construction safety for two years for the same State
department. Since then he has worked on the consulting and contracting side of
the environmental remediation industry. He has been involved in projects ranging
from small commercial to large multi-million dollar industrial projects as a
regulator, consultant and contractor. His partial duties include:

TECHNICAL PLAN FOR GENERAL DEMOLITION
OUR TECHNICAL PLAN IS PREDICATED ON PROVIDING DEMOLmON SERVICES TO OUR
CLIENTS IN THE SAFEST AND MOST COST EFFECTIVE METHOD POSSIBLE. THE MOST
IMPORT ANT GOAL OF OUR PLAN IS TO MINIMIZE THE RISK OF INJURY AND PROPERTY

(B) IDENTIFY ALL OF THE UTILITY PROVIDERS ON THE FACILITY AND MAKE SURE THEY
HAVE ADEQUATE TIME TO DISCONNECf SERVICE FROM THE STRUCTURE BEING
DEMOLISHED. SINCE THE TIME REQUIRED TO COMPLETE THIS PROCESS CAN BE
DIFFICULT TO QUANTIFY, IT IS EXTREMELY IMPORTANT THAT THE DISCONNECTION
PROCESS BEGINS AS SOON AS THE CONTRACTOR RECEIVES A NOTICE TO PROCEED
(C) REVIEW THE ENVIRONMENTAL SITE ASSESSMENTS FOR THE STRUCTURE AND MAKE
SURE THAT ALL HAZARDOUS MATERIALS ARE COMPLETELY REMOVED PRIOR TO
DEMOLITION COMMENCING
(D) SEND A TEN DAY NESHAPS TO THE RESPONSIBLE AGENCY BY CERTIFIED MAIL WITH
A COPY BEING SENT TO THE OWNER

(E) REVIEW THE CONlRACT DOCUMENTS AND MAKE SURE THAT ALL APPROVALS ARE
IN PLACE AND THAT WE HAVE DUE AUTIIORIZATION TO PROCEED WITH THE WORK
(F) PERFORM A PRE-DEMO INSPECTION OF EACH WORK SITE TO MAKE SURE THE
ACTUAL WORK COINCIDES WITH THE CONTRACT DOCUMENTS, FILL OUT THE
ST ANDARD COMPANY JOB CHECK LIST AND MAKE SURE IT IS PLACED IN THE
PERMANENT JOB FILE
(G) DURING THE SITE VISIT LOOK FOR ANY PossmLE DANGEROUS SITUATIONS THAT
MAY BE ASSOCIATED WITH TIllS WORK. AND RELAY THESE CONCERNS TO THE
PROJECT SUPERINTENDENT AND OTHER TEAM MEMBERS (SUCH AS LOW HANGING
WIRES, GAS LINES, SEPTIC TANKS, TREE LIMBS, ETC,)
(H) OBSERVE HOW THE SITE IS SITUATED IN RELATION TO THE EXISTING ROADS AND
DEVISE A PLAN AS TO THE BEST HAUL ROlITE FOR ALL OF THE WASTE GENERATED
BY THE PROJECT AND BE ON THE LOOKOUT FOR SIDEWALKS, CURBS OR OTHER
ITEMS WHICH MAY NOT BE PART OF THE DEMOLITION BUT HAVE TO BE CROSSED TO
GET TO THE JOB SITE. IF THESE ITEMS ARE TO REMAIN AND NEED TO BE PROTECTED;
A PLAN TO ACHIEVE TIllS PROTECTION NEEDS TO BE DEVISED AND IMPLEMENIED
AND THE SUPERINTENDENT NEEDS TO BE MADE AWARE OF THE PLAN
(I) MAKE SURE THAT ALL CONCERNED PARTIES TO THE UPCOMING WORK ARE CLEARLY
IDENTIFIED AND ARE THROUGHL Y A WARE OF THE EXTENT AND OUTCOME OF ALL
UPCOMING DEMOLITION

1. DEMOLITION SAFETY PLAN
1. Demolition work involves many of the hazards associated with construction.
2. However, demolition incurs additional hazards due to unknown factors such as: deviations
from the structure's design introduced during construction, approved or unapproved
modifications that altered the original design, materials hidden within structural
members, and unknown strengths or weaknesses of construction materials.
3. To counter these unknowns, all personnel involved in a demolition project will be fully aware
of these types of hazards and the safety precautions to take to control the hazards.
2. DEMOLITION SAFETY PROCEDURES
1. Before the start of every demolition job, Cross Construction Services, Inc. will take a number
of steps to safeguard the health and safety of workers at the job site.
C 2. These preparatory operations involve the overall planning of the demolition job, including
the methods to be used to bring the structure down, the equipment necessary to do the
job, and the measures to be taken to perform the work safely.
3. Planning for a demolition job is as important as actually doing the work. Therefore, a
competent person experienced in all phases of the demolition work to be performed will
perform all planning work.
3. DEMOLITION ENGINEERING SURVEY
1. Prior to starting all demolition operations, OSHA Standard 1926.850(a) requires that a
competent person will conduct an engineering survey of the structure.
2. The purpose of this survey is to determine the condition of the framing, floors, and walls so
that measures can be taken, if necessary, to prevent the premature collapse of any
portion of the structure.
3. When indicated as advisable, any adjacent structure(s) or improvements will also be
similarly checked.
4. Cross Construction Services, Inc. will maintain a written copy of this survey.

5. Photographing existing damage in neighboring structures will also take place by the
competent designated person.

6. The engineering survey provides Cross Construction Services, Inc. with the opportunity to
evaluate the job in its entirety.
7. Cross Construction Services, Inc. will plan for the wrecking of the structure, the equipment
to do the work, manpower requirements, and the protection of the public. The safety of
all workers on the job site will be a prime consideration.
8. During the preparation of the engineering survey, Cross Construction Services, Inc. will plan
for potential hazards such as fires, cave-ins, and injuries.
9. If the structure to be demolished has been damaged by fire, flood, explosion, or some other
cause, appropriate measures, including bracing and shoring of walls and floors, will be
taken to protect workers and any adjacent structures.
10. It will also be determined if any type of hazardous chemicals, gases, explosives, flammable
material, or similar dangerous substances have been used or stored on the site.
11. If the nature of a substance cannot be easily determined, samples will be taken and
analyzed by a qualified person prior to demolition.
c: 4. UTILITY DEMOLITION LoeA TIONS
1. One of the most important elements of the pre-job planning is the location of all utility
services.
2. All electric, gas, water, steam, sewer, and other services lines will be shut off, capped, or
otherwise controlled, at or outside the building before demolition work is started.
3. In each case, any utility company that is involved will be notified in advance, and its
approval or services, if necessary, will be obtained.
4. If it is necessary to maintain any power, water, or other utilities during demolition, such lines
will be temporarily relocated as necessary and/or protected.
5. The location of all overhead power sources will also be determined, as they can prove
especially hazardous during any machine demolition.
6. All workers will be informed of the location of any existing or relocated utility service.

7. The telephone numbers of the local police, ambulance, and fire departments will be
available at each job site. This information can prove useful to the job supervisor in the
event of any traffic problems, such as the movement of equipment to or from the job
site.

5. MEDICAL SERVICES AND FIRST AID
1. Prior to starting work, provisions will be made for prompt medical attention in case of serious
injury.
2. The nearest hospital. infinnary, clinic, or physician will be located as part of the engineering
survey.
3. The job supervisor will be provided with instructions for the most direct route to these
facilities.
4. Proper equipment for prompt transportation of an injured worker, as well as a
communication system to contact any necessary ambulance service, will be available at
the job site.
5. The telephone numbers of the hospitals, physicians, or ambulances will be conspicuously
posted.
6. A properly stocked first aid kit as detennined by an occupational physician will be available
at the job site.
7. The first aid kit will contain approved supplies in a weatherproof container with individual
sealed packages for each type of item. It will also include rubber gloves to prevent the
transfer of infectious diseases.
8. Provisions will also be made to provide for quick drenching or flushing of the eyes for any
person is working around corrosive materials. Eye flushing will be done with water
containing no additives.
9. The contents of the kit will be checked before being sent out on each job and at least weekly
to ensure the expended items are replaced.
6. FIRE PREVENTION AND PROTECTION
A "fire plan" will be set up prior to beginning a demolition job. This plan will outline the
assignments of key personnel in the event of a fire and provide an evacuation plan for
workers on the site. Common sense will be the general rule in all fire prevention
planning:
Preparatory Operations:
1. Cross Construction Services, Inc. will be responsible for the development of a fire protection
program to be followed throughout all phases of the construction and demolition work,
and provide for the firefighting equipment as specified.
2. As fire hazards occur, there will be no delay in providing the necessary equipment.

3. When employees are required to work within a structure to be demolished which has been
damaged by fire, flood, explosion, or other cause. the walls or floor will be shored or
braced.
4. All electric, gas, water, steam, sewer, and other service lines will be shut off, capped, or
otherwise controlled. outside the building line before demolition work is started. In each
case, any utility company that is involved will be notified in advance.
5. If it is necessary to maintain any power, water or other utilities during demolition, such lines
will be temporarily relocated, as necessary, and protected.
6. Where a hazard exists from fragmentation of glass, such hazards will be removed.
7. It will also be determined if any type of hazardous chemicals, gases. explosives, flammable
materials, or similarly dangerous substances have been used in any pipes, tanks, or
other equipment on the property.
8. When the presence of any such substances is apparent or suspected, testing and purging
will be performed and the hazard eliminated before demolition is started.
C~ 9. Access to all available firefighting equipment will be maintained at all times.
10. All firefighting equipment, provided by Cross Construction Services, Inc., will be
conspicuously located.
11. All firefighting equipment will be periodically inspected and maintained in operating
condition. Defective equipment will be immediately replaced.
12. As warranted by the project, Cross Construction Services, Inc. will provide a trained and
equipped firefighting organization (Fire Brigade) to assure adequate protection to life.
13. Where a hazard exists to employees falling through wall openings, the opening will be
protected to a height of approximately 42 inches.
14. When debris is dropped through holes in the floor without the use of chutes, the area onto
which the material is dropped will be completely enclosed with barricades not less than
42 inches high and not less than 6 feet back from the projected edge of the opening
above.
15. Signs, warning of the hazard of falling materials, will be posted at each level. Removal will
not be permitted in this lower area until debris handling ceases above.
16. All floor openings, not used as material drops, will be covered over with material
substantial enough to support the weight of any load that will be imposed. Such
material will be properly secured to prevent its accidental movement.

17. Except for the cutting of holes in floors for chutes, holes through which to drop materials,
preparation of storage space, and similar necessary preparatory work, the demolition of
exterior walls and floor construction will begin at the top of the structure and proceed
downward.
18. Each story of exterior wall and floor construction will be removed and dropped into the
storage space before commencing the removal of exterior walls and floors in the story
next below.
19. Employee entrances to multi-story structures being demolished will be completely
protected by sidewalk sheds or canopies, or both, providing protection from the face of
the building for a minimum of 8 feet. All such canopies will be at least 2 feet wider than
the building entrances or openings (1 foot wider on each side thereof), and will be
capable of sustaining a load of 150 pounds per square foot.
20. During the planning stage of the job, Cross Construction Services, Inc. will determine all
needed safety equipment The required number and type of respirators, lifelines,
warning signs, and safety nets, special face and eye protection, hearing protection, and
other worker protection.
21. In the absence of an infirmary, clinic, hospital, or physician that is reasonably accessible in
terms of time and distance to the work site, a person who has a valid certificate in first
aid training will be available at the work site to render first aid.
22. A comprehensive first aid plan is necessary for any confined space entry.
7. PRE DEMOLITION CONSIDERATIONS
1. All potential sources of ignition will be evaluated and the necessary corrective measures
taken.
2. When storing debris or combustible material inside a structure, such storage will not
obstruct or adversely affect the means of exit.
3. Electrical wiring and equipment for providing light, heat, or power will be installed by a
competent person and inspected regularly,

5. A suitable location at the job site will be designated and provided with plans, emergency
information, and equipment, as needed.

7. Equipment powered by an internal combustion engine will be located so that the exhausts
discharge away from combustible materials and away from workers.
8. When the exhausts are piped outside the building, clearance of at least six inches will be
maintained between such piping and combustible material.
9. Free access from the street to fire hydrants and to outside connections for standpipes,
sprinklers, or other fire extinguishing equipment, whether permanent or temporary, will
be provided and maintained at all times.
10. All internal combustion equipment will be shut down prior to refueling. Fuel for this
equipment will be stored in a safe location.
11. Pedestrian walkways will not be so constructed as to impede access to hydrants.
12. Sufficient fire fighting equipment will be located near any flammable or combustible liquid
storage area.

13. Only approved containers and portable tanks will be used for the storage and handling of
flammable combustible liquids.
14. No material or construction will interfere with access to hydrants, Siamese connections, or
fire-extinguishing equipment.
15. A temporary or permanent water supply of volume, duration, and pressure sufficient to
operate the fire-fighting equipment properly will be made available.
16. Standpipes with outlets will be provided on large multistory buildings to provide for fire
protection on upper levels. If the, water pressure is insufficient, a pump will also be
provided.
17. Heating devices will be situated so they are not likely to overturn and will be installed in
accordance with their listing, including clearance to combustible material or equipment.
18. Competent personnel will maintain temporary heating equipment, when utilized.
19. An ample number of fully charged portable fire extinguishers would be provided throughout
the operation. All motor driven mobile equipment will be equipped with an approved fire
extinguisher.
20. Roadways between and around combustible storage piles will be at least 15 feet wide and
maintained free from accumulation of rubbish, equipment, or other materials,
21. Smoking will be prohibited at or in the vicinity of hazardous operations or materials. Where
smoking is permitted, safe receptacles will be provided for smoking materials.

22. An alarm system, e.g., telephone system, siren, two-way radio, etc. will be established in
such a way that employees on the site and the local fire department can be alerted in
case of an emergency.
23. The alarm code and reporting instructions will be conspicuously posted and the alarm
system will be serviceable at the job site during the demolition.
24. Fire cutoffs will be retained in the buildings undergoing alterations or demolition until
operations necessitate their removal.
8. STAIRS. PASSAGEWAYS. LADDERS AND CHUTES
1. Only those stairways, passageways, and ladders, designated as means of access to the
structure of a building, will be used. Other access ways will be entirely closed at all
times.
2. All stairs, passageways, ladders and incidental equipment thereto, which are covered by
this section, will be periodically inspected and maintained in a clean safe condition.
.... 3. In a multistory building, when a stairwell is being used, it will be properly illuminated by
; either natural or artificial means, and completely and substantially covered over at a
point not less than two floors below the floor on which work is being performed, and
access to the floor where the work is in progress will be through a properly lighted,
protected, and separate passageway.
4. No material will be dropped to any point lying outside the exterior walls of the structure
unless the area is effectively protected.
5. All materials chutes or sections thereof, at an angle of more than 450 from the horizontal,
will be entirely enclosed, except for openings equipped with closures at or about floor
level for the insertion of materials.
6. The openings will not exceed 48 inches in height measured along the wall of the chute.
7. At all stories below the top floor, such openings will be kept closed when not in use.
8. A substantial gate will be installed in each chute at or near the discharge end. A competent
employee will be assigned to control the operation of the gate, and the backing and
loading of trucks.
9. When operations are not in progress, the area surrounding the discharge end of a chute will
be securely closed off.
C~ 10. A substantial guardrail will protect any chute opening, into which workmen dump debris,
approximately 42 inches above the floor or other surface on which the men stand to
dump the material.

11. Any space between the chute and the edge of openings in the floors through which it
passes will be solidly covered over.
12. Where the material is dumped from mechanical equipment or wheelbarrows, a securely
attached toe board or bumper, not less than 4 inches thick and 6 inches high, will be
provided at each chute opening.
13. Chutes will be designed and constructed of such strength as to eliminate failure due to
impact of materials or debris loaded therein.
14. Except for the cutting of holes in floors for chutes, holes through which to drop materials,
preparation of storage space, and similar necessary preparatory work, the demolition of

2. Floors weakened or otherwise made unsafe by demolition operations will be shored to carry
safely the intended imposed load from demolition operations.
3. Masonry walls, or other sections of masonry, will not be permitted to fall upon the floors of
the building in such masses as to exceed the safe carrying capacities of the floors.
4. No wall section, which is more than one story in height, will be permitted to stand-alone
without lateral bracing, unless such wall was originally designed and constructed to
stand without such lateral support, and is in a condition safe enough to be self-
supporting.
5. All walls will be left in a stable condition at the end of each shift.

6. Employees will not be permitted to work on the top of a wall when weather conditions
constitute a hazard.
7. Structural or load-supporting members on any floor will not be cut or removed until all
stories above such a floor have been demolished and removed.
8. This provision will not prohibit the cutting of floor beams for the disposal of materials or for
the installation of equipment.
9. Floor openings within 10 feet of any wall being demolished will be planked solid, except
when employees are kept out of the area below.
10. In buildings of Uskeleton-steel" construction, the steel framing will be left in place during the
demolition of masonry. Where this is done, all steel beams, girders, and similar
structural supports will be cleared of all loose material as the masonry demolition
progresses downward.
11. Walkways or ladders will be provided to enable employees to safely reach or leave any
scaffold or wall.

12. Walls, which serve as retaining walls to support earth or adjoining structures, will not be
demolished until such earth has been properly braced or adjoining structures have
been properly underpinned.
13. Walls, which are to serve, as retaining walls against which debris will be piled, will not be
so used unless capable of safely supporting the imposed load.
10. REMOVAL OF FLOORS
1. Openings cut in a floor will extend the full span of the arch between supports.
2. Before demolishing any floor arch, debris and other material will be removed from such arch
and other adjacent floor area.
3. Planks not less than 2 inches by 10 inches in cross section, full size undressed, will be
provided for, and will be used by employees to stand on while breaking down floor
arches between beams.
4. Such planks will be so located as to provide a safe support for the workmen will the arch
between the beams collapse. The open space between planks will not exceed 16
inches.

5. Safe walkways, not less than 18 inches wide, formed of planks not less than 2 inches thick if
wood, or of equivalent strength if metal, will be provided and used by workmen when
necessary to enable them to reach any point without walking upon exposed beams.

6. Stringers of ample strength will be installed to support the flooring planks, and the ends of
such stringers will be supported by floor beams or girders, and not by floor arches
alone.
7. Planks will be laid together over solid bearings with the ends overlapping at least 1 foot.
8. When floor arches are being removed, employees will not be allowed in the area directly
underneath, and such an area will be barricaded to prevent access to it.
9. Demolition of floor arches will not be started until they, and the surrounding floor area for a
distance of 20 feet, have been cleared of debris and any other unnecessary materials.
10. Mechanical equipment will not be used on floors or working surfaces unless such floors or
surfaces are of sufficient strength to support the imposed load.
11. Floor openings will have curbs or stop-logs to prevent equipment from running over the
edge.
11. STORAGE OF WASTE MATERIAL
1. The storage of waste material and debris on any floor will not exceed the allowable floor
loads.
2. In buildings having wooden floor construction, the flooring boards will be removed from not
more than one floor above grade to provide storage space for debris, provided falling
material is not permitted to endanger the stability of the structure.
3. When wood floor beams serve to brace interior walls or freestanding exterior walls, such
beams will be left in place until other equivalent support can be installed to replace
them.
4. Floor arches, to an elevation of not more than 25 feet above grade, will be removed to
provide storage area for debris: Provided that such removal does not endanger the
stability of the structure.
5. Storage space into which material is dumped will be blocked off, except for openings
necessary for the removal of material. Such openings will be kept closed at all times
when material is not being removed.

12. MECHANICAL DEMOLITION WITH CRANE
1. During the demolition operations, when balling or clamming are being performed. Only
those workers necessary for the performance of the operations will be permitted in this
area at any other time.
2. The crane boom and load line will be as short as possible.
3. The weight of the demolition ball will not exceed 50 percent of the crane's rated load, based
on the length of the boom and the maximum angle of the operation at which the
demolition ball will be used, or it will not exceed 25 percent of the nominal breaking
strength of the line by which it is suspended, whichever results in a lesser value.
4. The ball will be attached to the load line with a swivel-type connection to prevent twisting of
the load line, and will be attached by positive means in such manner that the weight
cannot become aCcidentally disconnected.
5. When pulling over walls or portions thereof, all steel members affected will have been
previously cut free.

6. All roof cornices or other such ornamental stonework will be removed prior to pulling walls
over.
7. During demolition, continuing inspections by a competent person will be made as the work
progresses to detect hazards resulting from weakened or deteriorated floors, or walls,
or loosened material.
8. No employee will be permitted to work where such hazards exist until they are corrected by
shoring, bracing, or other effective means.
13. SPECIAL STRUCTURES DEMOLITION
Safe work practices when demolishing a chimney, stack, silo, or cooling tower will include the
following procedures:
Inspection and planning:
1. When preparing to demolish any chimney, stack, silo, or cooling tower, the first step will be
a careful, detailed inspection of the structure by an experienced person.
2. If possible, architectural/engineering drawings will be consulted,
3. Particular attention will be paid to the condition of the chimney or stack.

4. Workers will be on the lookout for any structural defects such as weak or acid-laden mortar
joints, and any cracks or openings.

5. The interior brickwork in some sections of industrial chimney shafts can be extremely weak.
If the stack has been banded with steel straps, these bands will be removed only as the
work progresses from the top down.
6. Sectioning of the chimney by water, etc. will be considered.
Safe work practices:
1. When hand demolition is required, it will be carried out from a working platform.
2. Experienced personnel will install a self-supporting tubular scaffold, suspended platform, or
knee-braced scaffolding around the chimney.
3. Particular attention will be paid to the design, support, and tie-in (braces) of the scaffold.
4. A competent person will be present at all times during the erection of the scaffold.
5. It is essential that there be adequate working clearance between the chimney and the work
platform.
6. Access to the top of the scaffold will be provided by means of portable walkways.
7. The platforms will be decked solid and the area from the work platform to wall bridged with a
minimum of two-inch thick lumber.
8. A top rail 42 inches above the platform, with a mid-rail covered with canvas or mesh will be
installed around the perimeter of the platform to prevent injury to workers below. Debris
netting will be installed below the platform.
9. Excess canvas or plywood attachments can form a wind-sail that could collapse the
scaffold.
10. When working on the work platform, all personnel will wear hard hats, long-sleeve shirts,
and eye and face protection, such as goggles and face shields, respirators, and safety belts,
as required.
11. Care will be taken to assign the proper number of workers to the task,
12. Too many people on a small work platform can lead to accidents.

13. An alternative to the erection of a self-supporting tubular steel scaffold to "climb" the
structure will be a creeping bracket scaffold.

14. The supervisor will make careful inspection of the masonry as to the safety of the
alternative. It is essential that the masonry of the chimney be in good enough condition
to support the bracket scaffold.
15. The area around the chimney will be roped off or barricaded and secured with appropriate
warning signs posted. No unauthorized entry will be permitted to this area. It is also
good practice to keep a worker, i.e., a supervisor, operating engineer, another worker,
or a "safety person," on the ground with a form of communication to the workers above.
16. Special attention will be paid to weather conditions when working on a chimney. No work
will be done during inclement weather such as during lightning or high wind situations.
The work site will be wet down, as needed, to control dust.
14. DEBRIS CLEARANCE
If debris is dropped inside the shaft, it can be removed through an opening in the chimney at
grade level.
1. The opening at grade will be kept relatively small in order not to weaken the structure.
2. If a larger opening is desired, a professional engineer will be consulted.
3. When removing debris by hand, an overhead canopy of adequate strength will be provided.
4. If machines are used for removal of debris, proper overhead protection for the operator will
be used.
5. Excessive debris will not be allowed to accumulate inside or outside the shaft of the
chimney as the excess weight of the debris can impose pressure on the wall of the
structure and might cause the shaft to collapse.
6. The foreman will determine when debris is to be removed, halt all demolition during debris
removal, and make sure the area is clear of cleanup workers before continuing
demolition.
15. DEMOLITION BY DELIBERATE COLLAPSE
1. Another method of demolishing a structures, chimneys, or stacks are by deliberate collapse.
2. Deliberate collapse requires extensive planning and experienced personnel, and will be
used only when conditions are favorable.

3. There will be a clear space for the fall of the structure of at least 45 on each side of the
intended fall line and 1 % times the total height of the chimney. Considerable vibration
will be set up when the chimney falls, so there will be no sewers or underground
services on the line of the fall.

4. Lookouts will be posted on the site and warning signals will be arranged. The public and
other workers at the job site will be kept well back from the fall area.
5. The use of explosives is one way of setting off deliberate collapse. Only qualified persons
will complete this type of demolition.
6. The entire work area will be cleared of nonessential personnel before any explosives are
placed.
7. Though the use of explosives is a convenient method of bringing down a chimney or stack,
there is a considerable amount of vibration produced, and caution will be taken if there
is any likelihood of damage.
16. DEMOLITION PRESTRESSED CONCRETE STRUCTURES
1. The different forms of construction used in a number of more or less conventional structures
built during the last few decades will give rise to a variety of problems when the time
comes for them to be demolished.
2. Pre-stressed concrete structures fall in this general category. The most important aspect of
demolishing a pre-stressed concrete structure takes place during the engineering
survey.
3. During the survey, a qualified person will determine if the structure to be demolished
contains any pre-stressed members.
4. It is the responsibility of Cross Construction Services, Inc. to inform all workers on the
demolition job site of the presence of pre-stressed concrete members within the
structure.
5. Cross Construction Services, Inc. will also instruct them in the safe work practice that will be
followed to safely perform the demolition.
6. Workers will be informed of the hazards of deviating from the prescribed procedures and the
importance of following their supervisor's instruction.
17. CATEGORIES OF PRE-STRESSED CONSTRUCTION
There are four main categories of pre-stressed members. The category or categories will be
determined before attempting demolition, bearing in mind that any pre-stressed structure will
contain elements of more than one category.
1. Category 1- Members are pre-stressed before the application of the superimposed loads,
and all cables or tendons are fully bonded in the concrete or grouted within ducts.

2. Categories 2- Like Category 1, but the tendons are left ungrouted. This type of
construction can sometimes be recognized from the access points that will have been
provided for inspection of the cables and anchors. More recently, un-bonded tendons
have been used in the construction of beams, slabs, and other members; these
tendons are protected by grease and surrounded by plastic sheathing, instead of the
usual metal duct.
3. Category 3 Members are pre-stressed progressively as building construction proceeds and
the dead load increases, using bonded tendons as in Category 1.
4. Category 4 Like Category 3, but using un-bonded tendons as in Category 2.
5. Examples of construction using members of Categories 3 or 4 are relatively rare. However,
they will be found, for example, in the podium of a tall building or some types of bridges.
They require particular care in demolition.
18. PRETENSIONED MEMBERS
1. These usually do not have any end anchors, the wires being embedded or bonded within
the length of the member.
2. Simple pre-tensioned beams and slabs of spans up to about 23 feet can be demolished in a
manner similar to ordinary reinforced concrete,
3. Pre-tensioned beams and slabs will be lifted and lowered to the ground as complete units
after the removal of any composite concrete covering to tops and ends of the units.
4. To facilitate breaking up, the members will be turned on their sides.
5. Lifting from the structure will generally be done from points near the ends of the units or
from lifting point positions.
6. Reuse of lifting eyes, if in good condition, is recommended whenever possible. When units
are too large to be removed, consideration will be given to temporary supporting
arrangements.
7. Pre-cast units stressed separately from the main frames of the structure, with end anchors
and grouted and un-grouted ducts:
8. Before breaking up, units of this type will be lowered to the ground, if possible. It is
advisable to seek the counsel of a professional engineer before carrying out this work,
especially where there are un-grouted tendons,

timbers, or a blast mat as a screen will be erected around the ends and demolition
commenced, taking care to clear the area of any personnel. The end blocks will be
heavily reinforced and difficult to break up.
19. MONOLITHIC STRUCTURES
1. The advice of the professional engineer experienced in pre-stressed work will be sought
before any attempt is made to expose the tendons or anchorages of structures in which
two or more members have been stressed together.
2. It will usually be necessary for temporary supports to be provided so the tendons and the
anchorage can be cautiously exposed. In these circumstances it is essential that
indiscriminate attempts to expose and de-stress the tendons and anchorages not be
made.

3. In the case of progressively pre-stressed structures, it is essential to obtain the advice of a
professional engineer, and to demolish the structure in strict accordance with the
engineer's method of demolition.
4. The stored energy in this type of structure is large. In some cases, the inherent properties of
the stressed section will delay failure for some time, but the presence of these large
prestressing forces will cause sudden and complete collapse with little warning.
20. WORKING IN CONFINED SPACES
1. Cross Construction Services, Inc. often comes in contact with confined spaces when
demolishing structure at industrial sites.
2. These confined spaces can be generally categorized in two major groups: those with open
tops and a depth that restricts the natural movement of air, and enclosed spaces with
very limited openings for entry,
3. Examples of these spaces include storage tanks, vessels, degreasers, pits vaults, casing,
and si105.
4. The hazards encountered when entering and working in confined spaces are capable of
causing bodily injury, illness, and death.
5. Accidents occur among workers because of failure to recognize that a confined space is a
potential hazard.

6. It will therefore be considered that the most unfavorable situation exists in every case and
that the danger of explosion, poisoning, and asphyxiation will be present at the onset of
entry.
21. SAFE BLASTING PROCEDURES
1. Prior to the blasting of any structure or portion thereof, a complete written survey will be
made by a qualified person of all adjacent improvements and underground utilities.
2. When there is a possibility of excessive vibration due to blasting operations, seismic or
vibration tests will be taken to determine proper safety limits to prevent damage to
adjacent or nearby buildings, utilities, or other property.
3. The preparation of a structure for demolition by explosives will require the removal of
structural columns, beams or other building components.
4. This work will be directed by a structural engineer or a competent person qualified to direct
the removal of these structural elements.

5. Extreme caution will be taken during this preparatory work to prevent the weakening and
premature collapse of the structure.

6. The use of explosives to demolish smokestacks, silos, cooling towers, or similar structures

will be permitted only if there is a minimum of 90 of open space extended for at least
150% of the height of the structure or if the explosives specialist can demonstrate
consistent previous performance with tighter constraints at the site.
22. FIRE PRECAUTIONS
1. The presence of fire near explosives presents a severe danger. Every effort will be made to
ensure that fires or sparks do not occur near explosive materials.
2. Smoking, matches, firearms, open flame lamps, and other fires, flame, or heat-producing
devices will be prohibited in or near explosive magazines or in areas where explosives
are being handled, transported, or used. In fact, persons working near explosives will
not even carry matches, lighters, or other sources of sparks or flame.
3. Open fires or flames will be prohibited within 100 feet of any explosive materials. In the
event of a fire that is in imminent danger of contact with explosives, all employees will
be removed to a safe area,
4. Electrical detonators can be inadvertently triggered by stray RF (radio frequency) signals
from two-way radios. RF signal sources will be restricted from or near to the demolition
site, if electrical detonators are used.

23. PERSONNEL SELECTION
1. A blaster is a competent person who uses explosives. A blaster will be qualified by reason
of training, knowledge, or experience in the field of transporting, storing, handling, and
using explosives.
2. In addition, the blaster will have a working knowledge of state and local regulations that
pertain to explosives.
3. Training courses are often available from manufacturers of explosives and blasting safety
manuals are offered by the Institute of Makers of Explosives (I M E) as well as other
organizations.
4. Blasters will be required to furnish satisfactory evidence of competency in handling
explosives and in safely performing the type of blasting required.
5. A competent person will always be in charge of explosives and will be held responsible for
enforcing all recommended safety precautions in connection with them.

Gentlemen:
Cross Construction Company has provided asbestos abatement
services for us on numerous contracts including two at Tampa
International Airport.

Their first subcontract at T.I.A. was for about $500,000 and
involved removal of fireproofing in the bag make-up area. The
work was done at night over a three month period. It was done
in increments and performance times were critical to accommodate
airline pairing during displacement, etc. Cross performed
exactly as scheduled to the day. Their contract administration
and dealings with the Industrial Hygienist were very professional
and the entire project went like a training film. Their second
subcontract at T.r.A. was for about $750,000 and involved removal
of fireproofing above the occupied transfer level of the landside
building. Again, their performance was exemplary.
Cross Construction Company has subcontracted other abatement work
from us up to $1,000,000 and they have always performed to the
highest standards. We are comfortable recommending them for any
abatement related work.

I wanted to take a minute to let you know how pleased I am with the job your team is doing down here
in Daytona Beach. Any demo job is dangerous, this one is especially dangerous because of its height and
close proximity to the hotels to the north and south. Hector Ramirez has put safety first while keeping
production at a high level. His professionalism is second to none. ArnolfHerrero filled in seamlessly
during Hectors time away from this project also putting safety first and making sure to consider the
needs of the hotel's on either side. Juan Orosco has run his machine like an artist at play....many locals
stand across the street for hours in amazement. He has taken the building down as strategically as it was
put up. Your entire crew has worked efficient and safe and for that I would like to say thank you. As we
continue to move closer to completion, please share my thoughts with the crew and remind them to keep
up the great work! This is my second project (Las Palm as) with Cross in the last few months and my
experienced was very much the same. I personally look forward to working with you and your team in

Dear Sirs:
I'm old. In my lifetime I have encountered only one building contractor worth writing a
letter of praise about. Most, in my opinion, range from dismal to genuine horror.
This past year a building located in the downtown area of our Historical District had to be
demolished. The site had become exceptionally dangerous, and there was question as to
whether or not the shops on either side had walls-in-common. The roof had partially
fallen-in, and some other architectural aspects of the building had collapsed. There was
serious question as to whether or not this building could be taken down without death or
serious injury. Cross Construction of Tampa got the job.
It took awhile, but the only casualty of that demolition was one small water pipe
connected to one of the adjoining shops. All Tarpon Springs held its collective breath
while the work was going on. Everyone in town was concerned. Happy outcome:
Cross had seemingly worked a miracle.
Yesterday Cross began to demolish the house next door to mine, It had a huge sinkhole
smack-dab in the middle of it. The geology in this neighborhood is definitely karst, and
there are "anomalies". That lot next door is chockablock-full of "anomalies". Naturally,
I was worried sick. I needn't have been, Cross kept everything confined to that lot alone,
they watered the demolition so there was NO DUST, and they stressed the land as little as
possible with the footprint oftheir heavy equipment. They filled-in the huge sinkhole.
But the thing that impressed me the most was their replacement of the topsoil with
beautiful, black, rich, loam. WOW!
If the job is difficult, potentially dangerous, and/or involves historical structures that need
extra measures of understanding and TLC, then Cross is likely the very best
construction/demolition outfit in Florida. The way they work amazes me.
The Florida Dept. of Business and Professional Regulation gets a constant stream of
complaints from the general public, (usually soto-voce), when things aren't done

properly. Thought you would appreciate knowing when exceptional skills are exhibited
and things are done to the nines (that's a tad rare).

properties, and noted the aerial map that was included in the documents with the location of those
properties. For the property at 350 Sunny Isles Boulevard we have a Lease Agreement with Miami-
Dade County Water and Sewer (W ASD), It is a Treatment Facility/Pump Station with a holding
tank and parking lot. The holding tank will be demolished and the entire lot, including the parking
lot, will be leveled to the ground. W ASD is bound to provide the drawings. The Proposer shall
meet with and provide W ASD with all necessary documents and tests that the agency requires for
the demolition of the existing holding tank. This is more than a demolition project, and we will
provide all information needed to accomplish the demolition. Whatever W ASD wants, we do, and
whatever we do must be approved by W ASD.
Director Batista opened the meeting to questions from the prospective bidders.
QUESTIONS AND COMMENTS FROM PROSPECTIVE BIDDERS
I. What is the extent of demolition, what point is existing lines?
FOI' all properties it shall be as per the RFP. As to the W ASD property, demolition of
property up to wet well as per W ASD and parking lot al'e to the west of the tank.
Demolish tank to grade and remove all aspects of the pal'king and the tank as allowed
by WASD. (See attached Lease Agreement for additional information on approvals)
2. What are your expectations, the entire holding tank, may be 15 feet below ground?
Level the sudace, elevation 0, above ground to grade. Remove the tank entirely and
replace with suitable fill up to gradc. Verify tank information with \V ASD to cnsure
that thc entire scope is understood.

3. What will this property be used for?
It will be a City Park, and so no structures,
. 4. As Built Plans for W ASD?
As to the W ASD property, the City has no as-built plans. We will put the contact
person for W ASD on DemandStar. Vendor will be responsible to pl'ovide all necessal")'
documentation, including a scaled set of demolition plans with sufficient information to
comply with all agencies having jurisdiction. Vendor shall include ALL elements of
demolition and permitting for all necessary ,york.
5. All properties together?
It is a five-part proposal on a per location basis. One part, and it is listed in the RFP, is
going to begin sooner than the rest. Just note to use the dates that arc in the RFP as
your approximate Notice to Proceed.
6. Are you going to award these separately?
The presumption is that we will get a better deal if we do them all together. We will not
be sepal'ating these in pieces, we will be awarding to one particular vendor. HO'\H~ver,
the City may award separate contracts for each site.
7. Is the tank empty?
Please ve!"ify with W ASD.
8. Was it a water tank?
Not sure, please verify with W ASD. On the W ASD site, we need to be clear that the
proposal should include the entire extent of what could be expected or anticipated in a
similal' pl"Oject.
9. Will there be a check list of what WASD will require?
No. If you don't have experience with W ASD, you will soon find out with WASD that it
is a lot of back and forth. There is nothing in w!"iting in the form of a check list that
would allow you to undel'stand exactly the extent of what they llre going to require.
10. Is there a contact person?
We will provide a contact person for W ASD as an Addendum or Clarification on
DemandStar. They arc aware of it, the extent is what you arc going to have to figure
out in the proposal.
11. What are the other facilities?
The other four facilities al'e pretty straight fonvard since they al'e City-owned
propel'ties. Scope shall be per the RFP which includes demolition of all above ground
structu I'es, trees, and others to grade. Properties shall be su rvcycd by the V cndor with
necessal'y clcmcnts to obtain a pcrmit from all agcncies having jurisdiction.
12. Are you asking for an asbestos surveyor a boundary survey?
Both.

13. Why do you want a boundary survey?
The boundary survey is not required. What is required is a topogl'aphic survey with a
tree disposition plan included.
14. Are there trees on the properties?
Yes. Field vel"ify and include in the surveys as per agencies having jurisdiction.
15. Are all the trees going?
Yes. Include in proposal pCI' DERM I'cquil'cmcnts. DERM permit required. All the
trees are going to the extent that it, in other words I will tell you how I did it on this
other property and this is the flcxibility that we havc. DERM will say that wc have to
Icavc the trce, I am going to leave it. If I have to replace a tree, thcn it is time and
material for you all to get me replacement trees that I am going to put throughout the
City. That is how we are going to have to deal with that, and so it is going to be, if, and
you don't know right now what DERM's requirements are going to be for tree
removal, so it will be time and material.
16. And you don't know what the extent of asbestos is?
No. Vendor is to produce the necessary asbestos report with an abatement plan as
necessary. Thc City does not know the extent of the asbestos on these buildings. That
too is going to bc time and material when and in the evcnt that wc get to that point.
There is no other way to try to figurc that onc into the cquation.
17. And so we need to get an asbestos survey?
Y cs, figul'c on gctting an asbestos rcport with recommendations for evcry property in
accordancc to State requirements.
18. That is part of your RFP?
That is part of the RFP that the proposer is going to be submitting to me. In essence
what I want to see is, I want to give you an NTP and when I get a call from you, all
pennits arc done, the site is leveled to lime-rock or to grade.
19. What about pilings?
Pilings will be time and materhtl because we don't know the extent of the pilings.
20. Right, but typical foundations and footings you want out but pilings and tier caps remain?
Yes, just take it to grade.
21. Are you going to require a lead survey?
Per DERM. I am going to require whatcver DERM rcquires. A lead sUI'vey, they
didn't ask for it the last time I did this, they only asked for an asbestos survey.
You do want to take into considcration that a couplc of our properties have dl'Y
cleaners. Even though DERM does not have a pl'Oblem, there is no special issues
related to dl")' cleanel's except when they I'emove their equipment, it has to be removed
acconJing to their regulations. That is something that you al'e not going to be
responsible for but in the event one of the dry cleaners leave a space with material that
DERM does not look on f~"'ol'ably, that will be time and material as well. So the

presumption that you all are going to place in your proposal is that the tenant will be
removing all their equipment in accordance to DERM regulations. So when you get
there, the presumption is that all of th~lt has been taken care of. So that when DEI~I
sees it, they are not going to' have a problem. There is on 151 Sunny Isles Boulevard, a
dry cleaner and it is in full operation.
. 22. Will you require a Performance Bond?
Yes, Performance and Payment Bond.
23. A Bid Bond?
No.
24. Will you elaborate on the situation with 151 and 215 Sunny Isles Boulevard and the
possibility that Harbour Construction may be awarded those two pursuant to a previously
entered legal agreement between the City and the Gold Club? Taking it a step further,
presumably if they are the low bidder on those two, they are going to do them? They already
have an agreement with you.
The RFP states that the demolition of these two properties may be awarded to Harboul'
Construction pursuant to a settlement agreement as described in the RFP. However,
even if those two demolitions are given to Harbour Constt'uction, the successful hidder
could still be awarded the remaining four buildings to be demolished.
25. What I am getting at is that it takes a lot of effort to put this together, if they are going to get
it anyway, why bother?
My job is to put out thel'e a Request for Proposal, and what it says here is a portion of
the services c~m be provided for 151 and 215 Sunny Isles Boulevard to Harboul'
Construction, and that YOIl all acknowledge that the City may award a portion of the
services, and it says specifically ie. the demolition of 151 and 215 to Harbour
Construction pursuant to an agreement.
.26. So the worst case scenario the demo contractor breaks down the sites?
We actually broke them down because there were different dates that the tenants have
to be out, and that is why we put in the dates.
27. I have a question about the trees as well. First we assume that all the trees are to be
removed, and put a boundary survey together and there is a possibility that a few may stay
per DERM, but we are not to assume that DERM is going to say not to remove them?
In your proposal, everything is getting removed, When DERM comes back and says
that we need to replace in-kind trees, for examplc you have a ccrtain size, ",-hen you do
a tl"CC disposition plan as part of the survey, they arc going to say thc size of the trunk
and the spread ofthe canopy. DERM comes back and does a little rough number as to
how many plants are required to replace that one tree, When DERM does that, that is
a time and material to you all. So your proposal is going to take into consideration,
trec removal. It is a little bit easier for you, I don't intend for you to know the equa tion
by heart and try to figure out those things, so that will be time and material.
28. Are we allowed to visit the sites?
Yes. they are opcn and availahle if you want to go by there. W ASD visit to he

coordinated.
29. Can you just go ahead and set up the day?
Yes, ifthat is what you all want (all agreedj, okay so an Addendum will be prepared for
a specific date fOl' the 350 property.
I have to c1adty that you have to request all the permits. When it comes to the City
permit, the City does requil'e that all properties be enclosed and that it has pollution
protection on the fencing. So make sure you incorporate that. The only fee that is
going to be waived is any fees that are from the City. Ifthel'e is a Miami-Dade County
fee, which there typically is, I think it is 60 cents on ever $1 ,000, that is still required to
be paid by the contractor. And so when you go pull a permit with the City there are
still some charges that might be there. The only fees that we waive are our own City
fees. The contractor will be required to pull all permits under his name, ~lIld to supply
all the necessal)' documentation for that permit.
30. Are you talking about the construction fence, the temporary fence?
All construction fencing shall have dust control.
31. The silt fence?
The silt fence will be required if and when, in W ASD you have got the mangroves in the
back, yes the silt fences are part of the NPDES requirements, which means that no
pollutant can le~lVe the particular demolition site and go into the right-of-way,
everything has to be controlled on site. Contractor is responsible for all permit
I'equirements and conditions.
32. I believe that Sunny Isles Boulevard and all the surrounding streets are higher than the grade
of the parking lot, and so it wouldn't make any sense to put a fence around it plus the cost for
trenching through asphalt for fencing would be astronomical, it just doesn't make sense and
he would ask that that requirement be waived?
We can't waive the NPDES requirements, and so when you pull the permit and the
NPDES issue does come up then it will be a requil'ement, if it doesn't, then it is not. I
can tell you based on my knowledge that all the pollutants must be controlled on site.
33. In the event that there is asbestos in the building, tie it in with any cost for abatement?
Whatever the asbestos report comes in and says abatement, will be time and material.
All you will be including on YOUI" pl"ice is demolition as you regularly demolish, and the
asbestos report. When the recommendation comes in from the asbestos company if
they do find asbestos in it, then we have to develop a plan that is going to he amicable
for all to remove it. Any abatement shall he hased on asbestos I"eport and shall he paid
fOl' on a time and material basis upon review by the City.
34. Is the City going to cap the water and the sewer or is that our responsibility?
No. This is the Contractol"s I'esponsibility. When you pull the permit with the City of
Sunny Isles Beach, you will have to have the notices all clear from all 1'ECO, CNMB
(City of North Miami Beach) fOl' the utilities, FPL and AT&T, and you are responsible

35. Does the City do their own water?
No, it is the City of North Miami Beach.
36. And W ASD is sewer?
Yes, WASD is sewer, CNMB is the water, and then you have AT&T and FPL (and
other utilities) for the rest. And before you get a pel'mit you need to havc all the
Lettcl's of "all clear" from those companies.
37. Are there fire hydrants for dust control?
In some ofthcm you are going to havc an accessible firc hydl'ant, ill others you have to
find the tire hydrants. You may have a problem 011 one of the properties, thc largc onc
you will 1I0t, but on thcse two southern most properties 16050 and 16100 Collins
Avenuc, you might not have a tire hydrant and may have to run a line somewhcrc. You
will have to get a floating metcr from CNMB. Vcndor shall do a ficld visit and assess
existing conditions.
It is not your typical demolish kind of project, it is a little bit more but I think with that
we have touched on the highlights of these projects. The two critical ones al'e the 350
Sunny Isles Boulevard with the WASD holding tank, and the 215 and 151 Sunny Islcs
Boulevard of the acknowledgment within the proposal itsclf. But hc that as it may, YOll
are welcome to propose on them and pleasc do.
