IIIJ I L DIN f, f, Il 0 IJ I) Executive Summary Project Overview Ocean Building Group, Inc. is please to provide this proposal to the city of Sunny Isles for the demolition of structures locat- ed at 151 and 215 Sunny Isles Boulevard and 16050 and 16100 Collins Avenue, Sunny Isles Beach Florida 33160. This proposal is a culmination of a comprehensive cost evaluation to the city of Sunny Isles for the demolition of the aforemen- tioned sites. The study included extensive site examination and analysis of historical building data. To the greatest extent feasible, we have attempted to respond to the city's desire for a competitive proposal for the demoli- tion of structures located at 151 and 215 Sunny Isles Boulevard and 16050 and 16100 Collins Avenue, Sunny Isles Beach Florida. Our proposal and implementation of this project encompasses all obligatory documentation, property surveys and plans, acquiring all permits required by the City and County agencies and any additional information necessary for the suc- cessful demolition of the existing structures described in this proposal. The implementation and completion of this project is proposed at $146.250 for the demolition of the existing structures described in this proposal. Proposal cost details and itemized costs per site are furnished in section 6 (Price Proposal). This proposal is categorized into eight (8) main sections: Section 1- This section will consist of all Company Information to include: Name of Agency/Company Company Locations Internet Web Site Address Details of Entity Business Structure Date Founded Home office address and telephone number, and local address and phone List of any outstanding litigation Proof of insurance Indication of how long it would take to implement service after authorized to begin Section 2- This section will illustrate or qualifications, relevant experience, and past performance to include the following: An explanation of why Ocean Building Group is the best qualified to perform the contract by solidifying its qualifica- tions including an item-by-item disclosure outlining how our firm meets or exceeds the requirements of this RFP. A schedule of proposed services with a clear understanding of the issues and tasks of the project at hand. Section 3-Staffing will consist of a list of experience and qualified key personnel, including key personnel of subcontractors that will be assigned to this project and experience and qualifications of subcontractors Section 4- Approach and Methodology to the proposed demolition work. Section -5 Price Proposal illustrating cost details associated with the services described in this RFP and a itemized price breakdown. Section 6 - References furnished for three (3) Current and Past Customers of which we have provided services similar in scope and size of those described in the RFP. A. 16050 Collins Ave. Demolition no earlier than 10/12/2011 B. 16100 Collins Ave. Demolition no earlier than 12/12/2011 C. 151 Sunny Isles Blvd. Demolition no earlier than 2/12/2012 D. 215 Sunny Isles Blvd, Demolition no earlier than 2/12/2012 APPROACH SITE PERIMETER Warning notices and restricted area notices will be positioned at vantage points. The permanent working area is to be protected and scaffold prior to main demolition works, solid barriers will be in place to all site boundaries though the provision of either timber, chain link or other type of fencing of utilization of existing boundary walls. ACCESS Access and egress to the workforce will via designated routes these will be agreed and implemented by the site supervisor. WORKFORCE PROTECTION All temporary barriers and signs etc, will be constantly checked and maintained before and during each working day by the site supervisors HAZARDOUS MATERIALS Identification and removal of asbestos containing materials (ACM's) will be carried out prior to our com- mencement on site. (Cost will be billed at Labor plus Material) DUST AND NOISE REDUCTION STRATEGY Noise of levels will be monitored during the course of the works. It is accepted the noise form the demoli- tion cannot be totally eliminated, but reasonable steps will be taken to reduce any adverse effects of noise generated by the works. The location of all plant and machinery, specifically if crushing plant were to be used will be positioned as far as its reasonably practicable form sensitive receptors, specifically members of the public and ad- jacent residential properties. This method minimizes the amount of the work undertaken (externally) by demolition operatives. GENERAL Careful programming of all elements i.e. soft stripping and demolition is to be adopted with areas being handed over as they are cleared for the next operation. Sectional completion of elements / areas will enable the works to progress in the manner as described below; SITE ESTABLISHMENT An induction and toolbox talk will be given by the site manager identifying specific items relevant to the upcoming works program. After completion operatives will then put on suitable PPE and transfer all necessary equipment to the working area. Permits to work will be requested / issued prior to any works commencing. TRAFFIC MANAGEMENT The suggested approach for sites of this nature is to establish as early as possible a one way system of moving skip vehicles. PERIMETER PROTECTION The site perimeter is to be protected with a mix of timber or chain link fencing. Solid panels on tempo- rary blocks will be used in sensitive areas where hoarding erections take place and may also form the perimeter barrier where site conditions / logistics will be changing directly following demolition works of short duration works are being carried out. SKIRTING BOARDS AND DOOR FRAMES Skirting boards and door frames are to be removed by site operatives using pinch bars and suitable hammers. The items are to be prized from their place of fixing. Any obstruction and nails are to be removed with all resultant materials then being transported utilizing the wheelbarrows and carrying by hand to the designated drop / loading zones. FLOOR COVERINGS The carpet coverings are to be removed by the operatives using mattock picks and shovels. Where the carpets are of a roll able nature these are to be cut into strips, whilst still laid, and then rolled up for collection. DEMOLITION OF STRUCTURE Once free of fixtures and fittings and structures are to be demolished by the 360 degree excavator fitted with suitable demolition attachments. All demolition, processing and concrete/masonry cutting works are to be damped down to act as a dust suppressant during the works, The sequence is to be roof, internal walls, side walls, and finally rear walls. The deadline for submission of Request for Proposals documents is Wednesday, August 10, 2011 at 10:00 a.m., at which time those sealed responses will be publicly opened. For the property at 350 Sunny Isles Boulevard we have a Lease Agreement with Miami- Dade County Water and Sewer (W ASD). It is a Treatment Facility/Pump Station with a holding tank and parking lot. The holding tank will be demolished and the entire lot, including the parking lot, will be leveled to the ground. W ASD is bound to provide the drawings. The Proposer shall meet with and provide W ASD with all necessary documents and tests that the agency requires for the demolition of the existing holding tank. This is more than a demolition project, and we will provide all information needed to accomplish the demolition. Whatever W ASD wants, we do, and whatever we do must be approved by W ASD. . . Director Batista opened the meeting to questions from the prospective bidders. QUESTIONS AND COMMENTS FROM PROSPECTIVE BIDDERS . 1. What is the extent of demolition, what point is existing lines? For all properties it shall he as per the RFP. As to the \VASD property, demolition of propert) up to wet wl'll as per WASD and parking lot are to the west of the tank. Demolish tank to grade and remove all aspects of the parking and the tank as allowed h) WASD. (See attached Lease Agreement for additional information on approvals) . 2. What are your expectations, the entire holding tank, may be 15 feet below ground? Level the surfaCl" ell" ation 0, above ground to grade. Remove the tank entirely and replace with suitable Ii 11 up to grade. Vendor will he responsible to provide all necessary documentation, including a sealed set of demolition plans with sufficient information to comply with all agencies having jurisdiction. Vendor shall include ALL elements of demolition and permitting for all necessary work. . 5. All properties together? It is a five-part proposal on a per location basis. One part, and it is listed in the RFP, is going to begin sooner than the rest. Just note to use the dates that are in the RFP as your approximate !';otice to Proceed. . 6. Are you going to award these separately? The presumption is that we will get a better deal if we do them all together. We will not be separating these in pieces, we will be awarding to one particular vendor. However, the City may award separate contracts for each site. 7. Is the tank empty? Please verify with WASD. . 8. Was it a water tank? Not sure, please verify with W ASD. On the W ASD site, we need to be clear that the proposal should include the entire extent of what could be expected or anticipated in a similar project. . 9. Will there be a check list of what W ASD will require? No. I f you don't have experience with \V ASD, you will soon find out with W ASD that it is a lot of hack and forth. There is nothing in writing in the form of a check list that would all(>" you to understand exactly the extent of what they are going to require. . 10. [s there a contact person? We will provide a contact person for WASD as an Addendum or Clarification on DemandStar. They are aware of it, the extent is what ~ ou arc going to have to figure out in the proposal. . 11. What are the other facilities? The other four facilities are prett~ straight forward since they are Cit)-owned properties. Scope shall be per the RFP which includes demolition of all above ground structures, trees, and others to grade. Properties shall be surveyed b~ the Vendor \\ith nccessar~ clements to obtain a permit from all agencies having jurisdiction. . 12. Are you asking for an asbestos surveyor a boundary survey? What is required is a topographic survey with a tree disposition plan included. . 14. Are there trees on the properties? Ycs. Field verify and include in the surveys as per agencics having jurisdiction. . 15. Are all the trees going? Yes. Include in proposal per DERl\1 requirements. OERM permit re<)uired. All the trees are going to the extent that it, in other words I will tell you how I did it on this other property and this is the flexibility that we have. DERM will say that we have to leave the tree, I am going to leave it. If I have to replace a tree, then it is time and material for you all to get me replacement trees that I am going to put throughout the City. That is how we are going to have to deal with that, and so it is going to be, if, and you don't know right now what DERM's requirements are going to be for tree removal. so it will be time and material. . . 16. And you don't know what the extent of asbestos is? 1\0. Vendor is to produce the necessary asbestos report with an abatement plan as necessary. The City docs not know the extent of the asbestos on these buildings. That too is going to be time and material when and in the event that we get to that point. There is no other way to try to figure that one into the equation. I 17. And so we need to get an asbestos survey? Yes. figu re on getting an asbcstos report with rccommendations for every property in accordance to State requirements. I 18. That is part of your RFP? That is part of the RFP that the proposer is going to be submitting to me. In essence what I want to see is, I want to give you an 1\TP and when I get a call from you. all permits are done, the site is leveled to lime-rock or to grade. 19. What about pilings? Pilings will be time and material because we don't know the extent of the pilings. . 20. Right, but typical foundations and footings you want out but pilings and tier caps remain? Yes, just take it to grade. . 21. Are you going to require a lead survey? Per DERM. ) am going to re<luire whatever DERM requires. A lead survey. they didn't ask for it the last time) did this, they only asked for an asbestos survey. . You do want to take into consideration that a couple of our properties have dr~ cleaners. Even though DERM does not have a problem. there is no special issues relatcd to dry clcaners except when they remove their equipment. it has to be removed according to their regulations. That is something that you are not going to be responsible for but in the cvent onc ofthc dry cleaners leave a space with material that OERM does not look on favorably. that will be time and material as well. So when you get there, the presumption is that all of that has been taken can' of. So that when DERM sees it, they al'c not going to have a problem. There is on 151 Sunny Isles Boulevard, a dry c1eancr and it is in fuIl operation. 22. Will you require a Performance Bond? Yes, Performance and Payment Bond. . 23. A Bid Bond? ]\0. . 24. Will you elaborate on the situation with 151 and 215 Sunny Isles Boulevard and the possibility that Harbour Construction may be awarded those two pursuant to a previously entered legal agreement between the City and the Gold Club? Taking it a step further, presumably ifthey are the low bidder on those two, they are going to do them? They already have an agreement with you. The RFP states that the demolition ofthcse h\o properties may be awarded to Harbour Construction pursuant to a settlement agreement as described in the RFP. However, even ifthose two demolitions arc given to Harbour Construction, the successful bidder could still be awarded the remaining four buildings to be demolished. . . 25. What 1 am getting at is that it takes a lot of effort to put this together, if they are going to get it anyway, why bother? 1\ly job is to put out there a Request for Proposal, and what it says here is a portion of the services can be provided for 151 and 215 Sunny Isles Boulevard to Harbour Construction, and that you all acknowledge that the City may award a portion of the services, and it says specifically ie. the demolition of 151 and 215 to Harbour Construction pursuant to an agreement. . 26. So the worst case scenario the demo contractor breaks down the sites? \Ve actually broke them down because there were different dates that the tenants have to be out, and that is why we put in the dates. . 27. I have a question about the trees as well. First we assume that all the trees are to be removed, and put a boundary survey together and there is a possibility that a few may stay per DERM, but we are not to assume that DERM is going to say not to remove them? In your proposal, every thing is getting fl'moved. \\ hen DER:\'1 comes back and says that we need to replace in-kind tre(~s, for example you have a certain size. when you do a tree disposition plan as part of the survey. they arc going to say the size of the trunk and the spread of the canopy. DERVI comes back and docs a little rough number as to how many plants arc fl'quired to replace that one tree. When DERM docs that. that is a time and material to you all. So your proposal is going to take into consideration, tree remo\ al. It is a little bit easier for you, I don't intend for you to kno\\ the equation by heart and try to figure out those things. so that will be time and material. . . 28. Can you just go ahead and set up the day? Yes, ifthat is what you all want fall agreedl, okay so an Addendum will be prepared for a specific date for th(~ 350 property. . I have to clarify that you have to request all the permits. \\'hen it comes to the City permit, the City does re<luire that all properties be enclosed and that it has pollution protection on the fencing. So make sure you incorporate that. The only fee that is going to be waived is any fees that arc from the City. lfthere is a Miami-Dade County fee, which there typically is, I think it is 60 cents on ever $1,000, that is still required to be paid by the contractor. And so when you go pull a permit with the City there are still some charges that might be there. The only fees that we waive are our own City fees. The contractor will be required to pull all permits under his name, and to supply all the necessary documentation for that permit. . 30. Are you talking about the construction fence, the temporary fence? All construction fencing shall have dust control. . . 31. The silt fence? The silt fence will be required if and when, in \V ASD you have got the mangroves in the back, yes the silt fences arc part of the NPDES requirements, which means that no pollutant can leave the particular demolition site and go into the right-of-way, everything has to be controlled on site. Contractor is responsible for all permit requirements and conditions. . 32. I believe that Sunny Isles Boulevard and all the surrounding streets are higher than the grade of the parking lot, and so it wouldn't make any sense to put a fence around it plus the cost for trenching through asphalt for fencing would be astronomical, it just doesn't make sense and he would ask that that requirement be waived? We can't waive the NPDES requirements, and so "hen you pull the permit and the NPDES issue does come up then it will be a requirement, if it doesn't. then it is not. I can tell you based on my knowledge that all the pollutants must be controlled on site. It 33. In the event that there is asbestos in the building, tie it in with any cost for abatement? \\'hatever the asbestos report comes in and says abatement, will be time and material. All you will be including on your pricr is drmolition as you regularly drmolish, and the asbestos report. \Vhen the rrcommendation comes in from the asbestos company if they do find asbestos in it, then we have to develop a plan that is going to be amicable for all to remo\'C it. Any abatement shall be based on asbestos report and shall be paid for on a time and material basis upon rniew by the City. . . 34. Is the City going to cap the water and the sewer or is that our responsibility? :\0. This is the Contractor's responsibility. \\ hen you pull the permit \\ith the City of Sunny Isles Beach. you will have to have the notices all clear from all TECn. And before you get a permit you need to have all the Letters of "all clear" from those companies. . 37. Are there fire hydrants for dust control? In some of them you are going to have an accessible tire hydrant, in others you have to tind the tire hydrants. You may have a problem on one ofthe properties, the large one you will not, but on these two southern most properties 16050 and 16100 Collins A venue, you might not have a tire hydrant and may have to run a line somewhere. You will have to get a floating meter from CNMB. Vendor shall do a field visit and assess existing conditions. . . It is not your typical demolish kind of project, it is a little bit more but I think with that we have touched on the highlights of these projects. The two critical ones arc the 350 Sunny Isles Boulevard with the WASD holding tank, and the 215 and 151 Sunny Isles Boulevard of the acknowledgment within the proposal itself. TENANT aclmowledges that the Leased Premises contains underground utility facilities (the "existing utilities") and the LANDLORD shall have access to the entire parcel in order to access the underground utility facilities for maiQtenance, repair or alteration.
