Dear Selection Committee:
Calvin, Giordano &Associates, Inc. (CGA), together with our sub-consultant partners
Professional Service Industries, Inc. (PSI) and EAS Engineering, Inc. are pleased to submit
our Statement of Qualifications for the above referenced RFQ. We have reviewed the scope
of services for this project, and have selected a project team with the necessary skills to
successfully complete all requested services.
CGA is a well-established multi-disciplinary firm with offices located in Fort Lauderdale,
West Palm Beach, Port St. Lucie, Homestead, Clearwater, Jacksonville, Florida and Atlanta,
Georgia. For more than 74 years, our firm has been providing consulting services to the
South Florida area. CGA's long standing relationships with various municipalities and our
diverse personnel makes us uniquely qualified to deliver the required services as described
in the scope of services.
If selected, CGA's Fort Lauderdale office located at 1800 Eller Drive, Suite 600, Fort
Lauderdale, FL will be the responsible office for this contract.
Dennis Giordano, will serve as the Principal-in-Charge of the proposed project, and Joel

surveying firm in Florida over 76 years ago. From 1937 to 2011 we have grown to over 200 employees;

Litigation Statement
Customer satisfaction is CGA's primary objective for every project undertaken by the firm. As such, CGA
endeavors to complete each project on time and within budget. CGA is proud to assert that CGA has not
failed to provide products, perform services, or furnished goods for which we have been contracted.
Furthermore, CGA has no judgments or claims pending or outstanding.
In the last five (5) years CGA has been involved in the following litigation:
1. James A. Cummings, Inc. v. Calvin, Giordano & Associates, Inc., Case No: 06-019973(18) in the 17th
Judicial Court, in and for Broward County. This case which was filed in 2006 was related to the
Palm Beach County Airport Parking Structure. The case was settled in August 2007.
2. Yvette Lorenzo, personal representative of the estate and survivors of Orestes Lorenzo v. Florida
Department of Transportation, City of Pembroke Pines, William J. Russo, Natasha Russo, and Calvin,
Giordano & Associates, Inc., Case No: 0601924(12) in the 17th Judicial Court, in and for Broward
County. CGA was a co-defendant in a wrongful death action brought by the deceased's relatives.
This case was filed on or about June 30, 2006, and was settled to the satisfaction of both parties in
September 2008.
3. Vue Condominium Association, Inc. vs. Vue Fort Lauderdale, L.L.c., ET AL, Case No. CACE09012422
.Suit was filed March 3, 2009. This is a typical condominium construction defect suit involving the
developer, contractor, numerous subcontractors and all the design team. The condominium
association voluntarily dismissed its claim against Calvin Giordano February 2011.
4. Walnut Creek Community Development District vs. Standard Pacific of South Florida, GP, Inc., Et AI,
Case No. CACE100193s3. Suit was filed May 4, 2010. The case concerns the failure of an HDPE
drainage system and is in the early stages of discovery. The other parties include the developer, the
pipe manufacturer and the contractor which is defunct but which may have insurance coverage.
Our understanding is that the problems are primarily the result of defective manufacture of the
HDPE pipe although design and construction deficiencies also are alleged.

Indication of How Long It Would Take to Implement Service After Authorized
to Begin:
Part of the unique qualifications CGA brings to the table is a depth of knowledge having been connected
with both the Newport Fishing Pier and North Bay Road Bridge over the past year managing the permitting
efforts. In the case of the Pier, our successful efforts have resulted in approvals through FDEP, USACE and
DERM with the final permits expected soon. Our connection has also led to the retention of a qualified
marine contractor to execute the work. Upon NTP the full efforts of CGA and the assigned personnel will
commence immediately with the administration, management and oversight of the project as well as the
steps necessary to get the required sub-consultants under contract.
Work in connection with the Bridge will also continue seamlessly. Having been involved with the
permitting efforts for the past six months, CGA is well up to speed on the Project's status. The CGA PM will
continue the collaborations with the permitting sub-consultant and simultaneously work with the CGA
Prime Consultant to organize the required design team efforts and get the required sub-consultants under
contract.
Our estimation is that CGA will begin to implement service immediately after authorized to begin.

SECTION 2 - QUALIFICATIONS
The CGA team approach centers on a 'gold standard' of attention to detail. Our onsite presence will be
highly visible, responsive and dynamic. We will examine the project issues and negative impacts on the
residents and will act to minimize them immediately. We have conducted these services in similar
contexts; accomplishing improvements within a specific framework, using thorough knowledge and
application of processes, order and fluidity, all the while being attentive to the needs of the client and
residents.
CGA as a firm has a vast experience in completing projects on-time and under budget as a Construction
Management and Prime Consultant. In addition to this, each individual listed in the organizational chart
brings a different history of completed projects from every position and side of the Construction Industry.
Our team includes former municipal employees, contractors, project managers and construction
estimators. This vast experience from every angle of the project allows CGA to circumvent and interpret
issues throughout the duration of the project. More specifically, below are reasons why CGA is best
qualified for these particular projects.
CGA has been involved in the Newport Fishing Pier project for the past year and is uniquely qualified to
provide the coordination, management and inspection services required by the RFQ. Over the past year the
firm has become intimately knowledgeable of the components of the varied phases of the work including:
permitting, plan review and construction administration as well as coordination of design team, sub
consultants and the Contractor. Specifically:
. CGA provided surveying services in connection with the original Pier Park development in 2003.
. CGA was retained in July 2010 to manage the permitting of the Pier and Restaurant. CGA
coordinated the permitting and design consultants as well as the state and federal agencies and to
date obtained:
o FDEP Notice of Completion
o FWC final permit conditions
o USACE Biological Opinion allowing land based construction during turtle nesting season
o NMFS review and Biological Opinion
o DERM "Draft" Class I Permit (Final pending FDEP permit)
. CGA obtained FDEP and DERM permits to allow Pier & Restaurant Demolition and the installation
of land based test piles
. CGA coordinated on behalf of the City the Subconsultants and Contractor to complete the
Restaurant demolition and test pile installation
. CGA Civil Engineering has initiated the investigation of the existing utility services and met with
DERM, WASD and CNMB in connection with same.
CGA has also been involved in the North Bay Road Bridge project for the past six months and is singularly
qualified to provide the coordination, management and inspection services required by the RFQ. Over the
past six months the firm has become intimately knowledgeable of the components of the varied phases of
the work including: permitting and plan review.

. CGA was retained in July 2010 to manage the permitting of the North Bay Road Bridge. CGA
coordinated the permitting and design consultants as well as the state and federal agencies and to
date accomplished:
o A USACE RAI was received April 26, 2011. Response is pending. Issues: Seagrass mitigation,
endangered species, wetlands fill (berm), construction methodology, navigational access,
conservation easement.
o A meeting with DERM was held on April 28, 2011 10:00am at DERM offices to review the
unresolved issues: Covenant, conservation easement, co-applicant submerged lands,
resident objections, seagrass and mangrove mitigation.
o A telephone conference with SFWMD was held May 4, 2011. An RAI was received May 9,
2011. Response is pending. Issues: S&S construction plans and calculations, mitigation -
mangroves and seagrass (on-site), construction methodologies, impacts to Conservation
Easement, USCG approval.
o The city is coordinating the co-applicant issues
. CGA is currently coordinating the permitting consultant with the preparation of an updated
mitigation plan necessary for submittal to the agencies
. CGA has reviewed the current design plans and assessed the missing components and is prepared
to address the issues in the areas of:
o Civil Engineering Drainage Plans & Calculations
o Roadway Design, geometric Plans & Cross Sections
o Pavement Markings & Signage
o Street Lighting
The following is an item-by-item disclosure outlining how the firm meets or exceeds the requirements of
this RFQ.
Construction Administration and Project Management
1. Support Responsibilities- CGA has, in the past, successfully fulfilled the role of support
responsibilities in many different capacities throughout the past year of SIB's Capital Improvement
Projects. CGA has a close working relationship with the Assistant City Manager and the Public
Works Director. This close working relationship allows us to provide the proper deliverables and
updates in the City's preferred format from day one, no learning curve will be required.
a. Daily inspection logs will be kept by the CGA CPR and signed and sealed inspection reports
will be generated by the subcontracted Pile Inspector, Special/Structural Inspector and
Geotechnical Engineer. These reports will be submitted in a weekly format to the City for
their review and records.
b. CGA has knowledge and experience working with Construction Scheduling and will provide
the City with a baseline schedule to which the weekly updated schedule can be compared to.
This will highlight any areas of construction that are either ahead of or behind schedule.

c. CGA has a clear understanding of the entities and processes which are required for
environmentally sensitive permitting. Our experience with these processes has been
demonstrated throughout the pier permitting process along with our other recently
permitted coastal projects.
2. Coordination of Contractor's submittals-CGA has completed this task on numerous previous
projects. We will utilize our standard forms for tracking the submittal process. CGA can organize
all of the submittal and other project files and upon completion of the project submit a final,
properly organized binder of approved submittals.
3. Site Observations/Close-out- CGA's staff has performed quality control and assurance
observations on many large projects including the 595 Express Lane and the 1-75 Interchange. We
have completed many projects in which we have utilized subcontractors to perform inspections and
testing. In each of these cases we have successfully read, interpreted and reacted to these testing
and inspection reports. CGA is also familiar with vibration monitoring. Our staff has utilized this
monitoring during blasting, beach construction and pier test pile installation.
4. On-site Presence-In many municipal applications, a part time on-site presence is required. CGA
has worked with many municipalities to acquire the perfect balance between budget and on-site
presence. We will work with the City to ensure that we stay within budget while still providing
adequate on-site presence.
S. Safety-Each of CGA's field employee has participated in al0-hour OSHA Course. This course is
designed to help identify potential safety hazards on construction sites.
6. Schedule & Budget-CGA's staff is experienced in construction scheduling and budgets. CGA can
create Primavera schedules, S Curves and Cash Flow graphs. Each of these reporting tools is to help
identify schedule and budget concerns early on so they can be addressed prior to becoming critical
path items.
PROFESSIONAL CONSULTING SERVICES
Prime Consultant
Over the course of several decades and dozens of projects CGA has excelled at assuming the leadership role
of directing the efforts of the assembled team of sub consultants required of projects with similar or greater
scope. CGA staff is experienced and comfortable with taking the lead role and responsibilities required to
command the quality of work and schedule demands these projects will require.
Specifically, CGA's resume of successfully completed work includes transportation, roadway, bridge,
building construction and marine projects that demonstrate the flexibility and diversity of the many
professional disciplines under the CGA mantle. CGA is uniquely adept at combining the outside professional
resources with the CGA staff needed to assemble the team to manage these two projects.
A clear distinction CGA brings to the table is the history of involvement with both the Pier and Bridge
projects. Not only from leading the permitting efforts but also working closely with the Architects,
Engineers and other professionals already in place. CGA has previously demonstrated a depth of knowledge
and understanding of the acceptable methods of construction to be implemented i.e. test pile driving &
monitoring; pile driving, jetting and limitations; precast tees; poured in place concrete bents; drilled shafts;
reinforced masonry, etc. CGA has an unmatched familiarity of the plans, specs, existing utilities as well as
knowledge of all work contracted and completed to date.

Professional Permit Processor and Coordinator with AUJs:
CGA has a productive collaboration with the environmental permitting subconsultant, EAS Engineering, Inc.
led by Mr. Ed Swakon. Together EAS and CGA have managed and expedited the Federal, State and Local
approvals to date for both the Pier and Bridge. It will be the intent of CGA to rigorously continue the daily
pursuit of each Agency Having Jurisdiction with the expectation of flushing out the requests for additional
Information (RAI's) and expeditiously answering each one until culminating in all required permits
necessary to commence the work.
Designer
CGA by definition is a multi-disciplined professional services firm. Civil Engineering & Design is at the
historical forefront of the company's work record. CGA is second to none in the ability to take on and
successfully complete the civil, transportation, traffic, drainage, water & sewer, landscaping and when
necessary structural, MEP and architectural disciplines. Through CGA professional staff and the utilization
of competent subconsultants all of the required submittals, RFI's, concrete design mixes, test pile programs,
production pile recommendations, precast concrete elements, geotechnical issues and others will be
reviewed and processed for approval.
Geotechnical Services & Material Testing Services
CGA intends to retain as subconsultant a highly referenced and experienced professional geotechnical
engineering firm: Professional Service Industries, Inc. (PSI). The PSI corporate resumes and project
references detail substantial field experience particularly in projects similar to the Pier and Bridge.
Additionally, PSI has been evaluated for the professional services recently rendered for the geotechnical
engineering evaluation work and pile installation monitoring accomplished in connection with the test pile
installation at the Pier this past March 2011. Along with their geotechnical expertise PSI will bring a full
service materials testing services component to the project. CGA will also stand ready to assist with
additional project requirements should they arise.
PROPOSED SERVICES
CGA proposes to meet the RFQ requirements by offering services of Prime Consultant, Project Manager
and Capital Projects Representative:
With the termination of Marlin Engineering the project no longer has a Prime Consultant engineer to
coordinate the engineering sub-consultants. Marlin Eng. had contracted BHA for the structural and
Architectural design of the Pier & Restaurant. BHA currently has no design contract with the City. Along
with coordination duties, Marlin Engineering was also the civil engineer and surveyor of record for the
project. Both of these entities, along with a Prime Consultant to coordinate design and permitting are
required immediately in order for the local permitting to proceed. The City recently approved CGA as a sub-
consultant to provide required civil engineering design for the Pier only (missing from the Marlin plans).
In the capacity of Prime Consultant, CGA will retain sub-consultants in the areas of:
. Special Inspector (structural inspection and certification of Pre-Stress/Pre-Cast piles and tees,
Reinforcing Steel Placement, Concrete Placement)
. Geotechnical Engineer of Record (test piles monitoring, pile driving monitoring, vibration &
settlement monitoring, certification)
. Materials Testing Services ( concrete, mortar/grout, soil, certification).

As Prime Consultant, CGA will coordinate and supplement these new sub-consultants as well as: CGA/EAS
and pending permitting issues; BHA: Architectural, MEP & Structural; CGA Civil Engineering and CGA
surveying services. As Prime Consultant for the Pier, CGA will also coordinate local permitting with DERM
& WASD and local service entities: FPL/TECO Gas/CNMB/AT&T & ABB. As Prime Consultant for the
Bridge, CGA will also coordinate local permitting with DERM/FPL/FDOT and Miami Dade Traffic.
In the capacity of Project Manager (PM), CGA will administrate and coordinate all of the efforts of the
Prime Consultant, permitting, A&E sub-consultants, inspection & materials testing services and the
Contractor. As PM, CGA will manage the Contractor and the Contractor's completion schedule; provide
reports and coordination with the City; chair construction progress meetings; process Pay Requests,
Submittals and RFI's; review Change Order Requests; and review Substantial and Final completion
requests.
CGA will also provide Capital Projects Representation (CPR) services. As CPR, CGA will provide field
representation on a periodic basis to monitor the specialty inspectors and services, construction progress
and Contractor's adherence to schedule and quality control. Upon conclusion and receipt of all sub-
consultant certifications, the CPR will issue a Letter of Compliance.
Additionally, and specifically in connection with the North Bay Road Bridge CGA is prepared to offer

CGA provided controlled access roadway design services for this 1-95 Design-Build project. The project
consisted of milling, resurfacing and shoulder rework of SR 9 (1-95) from the Broward County Line to
South of Linton Boulevard. The project also included the milling and resurfacing of the on and off ramps
at Palmetto Park Road, Glades Road, Yamato Road, and Congress Avenue, Palmetto Park Road and
Congress Avenue were milled and resurfaced within the limited access right of way,

Review and evaluate the entire power distribution system including five electric services, five power
distribution units, feeders, 72 pedestals, and voltage drop calculations to each pedestal.
Backaround
The management of Lake Park Harbor Marina retained Calvin, Giordano & Associates to evaluate the
electrical distribution installation of the Marina located at 105 Lakeshore Drive, Lake Park Florida. The
following incidents precipitated the request for evaluation:
1) Receptacles in Pedestal caught on fire and damaged the wiring requiring replacement of the
receptacle and wiring.
2) The motor circuit conductor to the diesel fuel pump melted together, faulted due to insulation
failure.
3) A microwave on a boat caught on fire.
4) Faulty feeder
5) Failed circuit breakers,
Recommendations
CGA made the following recommendations:
1) Voltage and current measurements be taken for the pedestal experiencing the problem
2) Boats with electrical loads greater than 40 amperes for single 50A receptacle or 24 amperes for
single 30A receptacles should be served from pedestal with larger capacity.
Upgrading of the existing circuit breaker in the distribution panel, the feeder and the circuit
breakers and receptacles in the pedestals.

NORTH BAY ROAD BRIDGE SURVEY
Sunny Isles Beach, Florida
CGA performed Design and Route Surveys for new bridge connecting 2 portions of the City. As part of this
project CGA established horizontal and vertical control points along with establishing a baseline that will
be used for design and construction of the bridge, CGA also calculated new easement areas based on our
control survey, prepared sketch and descriptions to be used in acquiring right-of-way for the proposed
bridge and then staked the easements on the ground for inspection by City officials and adjacent land
owners. Recently CGA has prepared a sketch and description of a portion of an existing Conservation
Easement that will be vacated as part of the bridge construction.

CGA utilized a leica ScanStation and Global Postioning System with the use of Real Time Kinematic to
perform a survey of the existing location to accurately establish that the parameters of the submerged
land lease. This was a requirement to ensure that the newly designed pier was being constructed within
the State mandated area. CGA also verified environmental monuments such as the Mean High Water Line
(MHWl), Erosion Control Line (ECl) and Mean Low Water Line (MlWl) for Miami-Dade County
Enviornmental Resources Management (DERM) and Florida Department of Environmental Protection
(FDEP). Finally, CGA performed a survey of the bottom contours to provide water depth information for
the bid documents. This information was integral to the Contractors during the estimating process.

CGA was contracted by Orion Dredging Services LLC to perform a pre-construction, layout, progress and
post construction hydrographic and topographic survey for the completion of a new cruise ship berth and
pier. This berth and pier was constructed to facilitate the world's largest cruise ships afloat today.
Labadee is a "private island" resort leased by Royal Caribbean International (RCI), which is a private
beach located on a peninsula in the mainland of Haiti, The creation of this pier and berth allowed RCI to
dock their vessels in lieu of anchoring off shore and tendering in their passengers, CGA's scope in this
project included the location and confirmation of control monuments, acquisition of hydrographic and
topographic field data, development of layout plans and maps,
To successfully complete this project, CGA had to work through environmental issues including working
in open seas and other inclement weather, logistical issues, including customs, power outages, and
sporadic/inadequate GPS signals in the location. The project was completed ahead of schedule and now
services the Allure, Oasis and Freedom of the Seas,

Chris Giordano has a total of 7 years construction management experience. After
graduating from the University of Central Florida with a Bachelor of Science in Electrical
Engineering Technology he pursued a career in the construction management industry.

for a Four Seasons Hotel and Casino, an 18 hole Greg Norman designed golf course, and
a 23 acre, 150 slip marina accommodating ships up to 200 feet in length.
City of Oakland Park, Kimberly Lake Flood Control and Water Quality
Enhancement Project., Oakland Park, FL; Project Director. The 72 acre Kimberly
Lake drainage basin had been an area prone to extreme flooding for a number of years.
The City of Oakland Park commissioned wished to develop alternate solutions to
mitigate the flooding issues and present those to the City Commission. Helped the City to
secure a $2,600,000 FEMA HGMP grant for a major portion ofthe construction. Directed
surveying, engineering, consensus building, liaison with governmental agencies, and
construction related services on the project. The new designed has met and even
exceeded original expectations.

SUMMARY OF QUALIFICATIONS
Mr. Jackson has 40 years of experience in all phases of land surveying including
boundary, coastal control, hydrographic, route surveys, topographic, and as-built
surveys; sketch and descriptions; legal descriptions, record plats; and, boundary
descriptions for annexations and de-annexations for municipalities.
Mr. Jackson is highly active in professional surveying organizations and is currently
President of the Florida Surveying and Mapping Society. In this role, he reviews Florida
surveying laws and is helping to develop a plan to professionally address GIS mapping
and new technologies in surveying. As part of this, he implemented memorandums of
understanding with allied professions (planners, geologists, biologists, environmental
scientists, lawyers, engineers, architects, landscape architects). He assisted the
promotion of an effective voice in the Florida legislature and taught Florida Laws, a six-
hour course required for professional surveyors. He supports the four-year-degree
requirement and to benefit the University of Florida Scholarship Fund for the Geomatics
program, he has organized golf tournaments which raised approximately $10,000. As
part of the Planting Seeds Program, he is spear-heading a plan to introduce the
surveying profession to students throughout the nation. Prior to CGA, Mr. Jackson
worked for the Florida Department of Transportation as a rodman, instrument man, and
party chief. His last position there was in the Right-of-Way (R/W) Engineering
Department Calculation Section, where he prepared R/W maps.

variety of Mr. Badri's public sector projects have included work on roadways, bridges, canals, schools,

completed by Fall 2015.
. Port of Miami Tunnel, Geotechnical Data Report, Miami-Dade County, Florida. Project
Manager/Project Geotechnical Engineer. The project consists of a depressed roadway and tunnel
between Watson Island and Dodge Island beneath the Government Cut main channel in Biscayne Bay, to
provide improved access for cargo and passenger vehicles destined for the Port of Miami. PSI's services
included a geotechnical study to obtain data regarding the general subsurface conditions near the

1-95 Express Lanes Project in Miami-Dade County was designed to create "Express" toll lanes (two in
each direction) along 1-95 by converting the existing High Occupancy Vehicle (HOV) lanes, At a cost of
$122 Million; the project includes the portion of 1-95 between SR 112 (also known 1-195 and the Airport

. W18th Avenue (from W 60th Street to N. of W 64th Street), Hialeah, Florida. Project Geotechnical
Engineer. PSI conducted a roadway soil survey to provide geotechnical roadway recommendation for the
pavement reconstruction and drainage improvements of W. 18th Avenue from W. 60th Street to north of
W. 64th Street in Hialeah, Florida. Our services included performing field reconnaissance, utility clearance,
subsurface exploration, percolation tests, laboratory testing, measuring groundwater depths, providing
engineering evaluations and recommendations.
. W 11th Avenue (from 23rd Street to 26th Street), Hialeah, Florida. Project Geotechnical Engineer. The
project involves the reconstruction of approximately 4,177 linear feet of roadway. PSI conducted a
Roadway Soil Survey with the purpose to obtain information concerning the general subsurface conditions
along the roadway alignments to make engineering estimates and recommendations regarding soil
stratigraphy at the boring locations, assessment of the existing soil subgrade and groundwater conditions
along the roadway alignments for subgrade suitability and pavement support, general location and
description of existing fills, soils and potentially deleterious materials encountered in the borings that may
affect reconstruction activities and pavement performance, site preparation requirements and engineering
criteria for placement and compaction of in-situ soils and approved fill materials, identification of critical
design and/or construction considerations based on the soil and groundwater conditions encountered in
the borings, and groundwater level measurements in the borings,
. S.R. 15 in Palm Beach County, Florida. Project Geotechnical Engineer. PSI performed a Roadway
Distress Evaluation along State Road 15, from E. 5th Street to SR700 in Pahokee, FL under our district
wide contract with the Florida Department of Transportation. The subject roadway consists of an
approximate 4 mile segment of two lanes (one in each direction), The conducted field exploration,
laboratory testing and engineering evaluation allowed for a formal explanation of the distress cause and to
provide the different alternatives for the roadway rehabilitation.

Current Status
Mr. Swakon is President of EAS Engineering, Inc., a firm offering comprehensive
environmental permitting and engineering services to the South Florida community and the
Caribbean. Activities undertaken by Mr. Swakon include assistance to local governments,
land developers, attorneys, property owners and others in acquiring the necessary permits
from local, state, and federal regulatory agencies. These permits include dredge and fill,
coastal and freshwater wetland construction, Florida Department of Environmental
Protection coastal construction control line, county and federal flood criteria variances, and
other specialized permits. Mr. Swakon also specializes in applied computer modeling of
various environmental situations such as waterway flow simulations, flushing studies for
existing or proposed marinas, as well as other coastal and hydrologically related
phenomena. Mr. Swakon is familiar with both the Regional Planning Council's
Development of Regional Impact (DRI) process and Monroe County's Major Conditional Use
process, having worked on both.
Previous Experience
Prior to forming EAS Engineering, Inc., Mr. Swakon was Director of Environmental
Permitting for a local Miami engineering firm. During that time, he personally coordinated
all of the environmental permitting assignments for the firm. These activities included
preparation of DRls, numerous applications to local governments, Florida Department of
Environmental Regulation and Florida Department of Natural Resources (now FDEP), and
the U.s. Army Corps of Engineers. Mr. Swakon has provided services to clients such as
Miami-Dade County, the City of Miami, and many local law firms such as Thomson Zeder et
ai, Greenberg Traurig et ai, and Carlos Abbott and Ferro.
Before entering the private sector in 1984, Mr. Swakon was employed by the Dade County
Department of Environmental Resources Management (DERM) in 1977, one of the largest
and most widely respected local environmental organizations in the country. He served as
the Chief of the Water Management Division, and was responsible for all tidal and
freshwater wetland permitting programs. He was also a key participant in the
development of the Biscayne Bay Management Plan and the implementation of the
Biscayne Bay Restoration and Enhancement Program. He worked closely with other
County departments in securing permits for county projects. He was the coordinator of all
the County's efforts regarding beach restoration including the world's largest beach
restoration project, the Miami Beach project and the controversial but much needed Key
Biscayne beach project. Finally, his responsibilities included the administration of the

Mr. Langley has more than thirty five (35) years of experience in conducting environmental
impact assessments in coastal areas, twenty of which have been in South Florida with local
engineering firms. As Senior Project Scientist of EAS Engineering, his primary

Previous Experience
Prior to joining EAS Engineering, Mr. Langley was Senior Project Scientist for a local Miami
engineering firm, where he was responsible for gathering support data and technical
information for the firm's engineering projects and preparing DRI applications, permit

Letters of Interpretation, and special exception and zoning variance petitions to various
local municipalities. He also conducted field inspections to assess environmental impacts
of proposed projects.
Mr. Langley also was a Project Manager over a monitoring team for one of the largest
seagrass mitigation efforts ever attempted in the U.s. In this capacity at another local
engineering firm he was responsible for budget preparation and control, staffing,
scheduling, quality control, and report preparation. Before being promoted to Project
Manager, Mr. Langley executed the following tasks: preparation of ORis and master plans
for three Miami-Dade County parks and marinas, an office complex in South Miami-Dade
County, and a mobile home development in Cape Coral; Environmental Impact Statements
for the Southwest Florida Regional Airport in Fort Myers and a u.s. Army Communications
Facility in South Miami-Dade; feasibility studies for several major developments from Vero
Beach to the Florida Keys; and numerous federal, state, and local permit applications. Mr.
Langley was principal author of a literature survey of water quality in the Key Largo Coral
Reef Marine Sanctuary and was involved in a study of American crocodile populations at
Florida Power and Light's Turkey Point Power Plant in South Miami-Dade and the Port

Mr. Morgado is the Chief CAD Operator at EAS Engineering. His responsibilities involve
producing permit sketches of existing and proposed structural and environmental
conditions, including detailed digitizing, mapping, and site planning. His computer aided
drafting and design capabilities allow for rapid generation of information in a demanding
atmosphere, while his clarity and creativity have contributed to all of the projects
undertaken by the firm. He has worked closely with the GIS specialist for two of
Miami-Dade County's recent large scale efforts to create digital maps for the lakebelt and
pineland regions of South Florida, performing photointerpretation and digitizing for the
projects.
As a graphic artist, Mr. Morgado has contributed to creating marketing and project-related
exhibits. (He is also the company cartoonist.)
Previous Experience
Mr. Morgado's prior experience includes eight years with Filigree Wideslab of Florida, Inc.,
where he drew framing plans, top reinforcements, shoring, and individual slab sheets. He
then moved to Outinord Universal where he prepared plans and designed structural and

SECTION 4 - QUALITY ASSURANCE/QUALITY CONTROL
It is important through project management, to resolve the many challenges on projects facing
municipalities today. Whether a project's primary focus is a pedestrian bridge or pier construction, it will
require clear and concise communication as well as clearly defined project objectives and a detailed and
concise management program from the inception of project implementation.
At Calvin, Giordano & Associates, Inc. (CGA), excellent Project Management skills are seen as the
cornerstone to any successful project. The CGA Project Manager for this contract will be Joel Wadsworth.
Throughout the professional coordination, and construction management phases, Mr. Wadsworth will be in
regular communication with the appointed representative for Sunny Isles Beach to update them on the
project's status and progress. All communication between the CGA project team and the City staff will
occur through the CGA Project Manager.
The CGA Team's approach to projects is principally based on a keen understanding and awareness of the
technical issues surrounding the uniqueness of each project, the establishment of a management process
targeted and tailored to resolve any potential issues and conflicts, and the selection of specific staff to
accomplish the projects' goals.
The distribution of assignments among the varying expertise of the CGA Team members is orchestrated to
ensure that all aspects of the projects are monitored by experienced personnel, as well as ensuring that
there are overlaps to guarantee seamlessness in the process, quality and delivery of the services as a
cohesive team.
Components of your QA/QC control include the following considerations:
Pre-Construction
The team under Mr. Wadsworth's direction will immediately begin Constructability /Build ability /Bid-
ability reviews of each of the two project's scope in order to arrive at an analysis of any risk assessment
issues necessary for a prioritized strategy of action, an essential step to evade or mitigate potential
conflicts, avoid claims and minimize project costs and timely extensions. In addition, we will review
project requirements against unique project commitments (including environmental issues, agency
coordination) in order to establish a Targeted Action Plan focused on successfully achieving the project's
goals and milestones.
Another major component of our pre-construction activity processes include a thorough analysis of
impending circumstances that need to be addressed, particularly with the impact of the environmental
issues and their regulating agencies. It has been our experience that both scheduled and unforeseen field
conditions can critically affect the schedule and budget of construction projects if not adequately planned
for and addressed as necessary, In addition, in order to minimize potential utility conflict issues, CGA will
contact and schedule a meeting with each utility provider with existing facilities within the project area to
review the schedule and scope of all planned removal/relocation operations, as well as develop an Action
Plan for identification, notification and resolution of all conflicts, including the contact information for the
agencies' responsible personnel.

The undersigned Proposers proposes and agrees, if this Bid is accepted, to enter into an agreement with The City of
Sunny Isles Beach in the form included in the Contract Documents to perform and furnish all Work as specified or
indicated in the Contract Documents for the Contract Price and within the Contract Time indicated in this Bid and in
accordance with the other terms and conditions of the Contract Documents.
The Proposers accepts all of the terms and conditions of the Advertisement or Invitation to Bid and Instructions to
Proposers, including without limitation those dealing with the disposition of Bid Security. This Bid will remain subject
to acceptance for 90 days after the day of Bid opening. The Proposers agrees to sign and submit the Agreement with
the Bonds and other documents required by the Bidding Requirements within ten days after the date of the City's
Notice of Award,
In submitting this Bid, the proposer represents, as more fully set forth in the Agreement, that:
. The proposer has familiarized himself/herself with the nature and extent of the
Contract Documents, Work, site, locality, and all local conditions and Law and
Regulations that in any manner may affect cost, progress, performance, or
furnishing of the Work.
. The Proposer has studied carefully all reports and drawings of subsurface
conditions and drawings of physical conditions.
. The Proposer has obtained and carefully studied (or assumes responsibility for
obtaining and carefully studying) all such examinations, investigations,
explorations, tests and studies which pertain to the subsurface or physical
conditions at the site or otherwise may affect the cost, progress, performance, or
furnishing of the Work at the Contract Price, within the Contract Time and in
accordance with the other terms and conditions of the Contract Documents, and
no additional examinations, investigations, explorations, tests, reports or similar
information or data are or will be required by proposer for such purposes.
. The proposer has given the City written notice of all conflicts, errors,
discrepancies that it has discovered in the Contract Documents and the written
resolution thereof by City is acceptable to the proposer.
. This Bid is genuine and not made in the interest of or on behalf of any
undisclosed person, firm or corporation and is not submitted in conformity with
any agreement or rules of any group, association, organization, or corporation;
the proposer has not directly or indirectly induced or solicited any other
Proposers to submit a false or sham Bid; the Proposer has not solicited or
