Concrete removal and paver installation work is excluded at all ADA ramps lo- cated at the pedestrian crossings. The cost of providing all necessary labor, speci- fied materials and equipment to paint the ADA ramps with approved colors and approved pattern, complete as required by the Owner (City if sunny Isles) included in our bid. 2. H.A.C.C. will provide owner approved FDOT/ Miami Dade County, ADA de- tectable warning surfaces at all crosswalks. All materials used on this project will be approved by the owner via material shop drawings prior to purchase. 3. H.A.C.C. will submit owner furnished drawings for new permits from FDOT and subsequently to other AHJ. 4. H.A.C.C. is familiar with the FDOT variation attached to RFP. The City engi- neer will provide all necessary information to H.A.C.C.. Contractor will apply and obtain a new permit variation if required by FDOT. 5. H.A.C.C. will place owner approved pavers in the owner approved exact pattern and color as in other locations through -out the City along Collins Ave. H.A.C.C. will field verify all dimensions and copy the same for the pavers proposed under this RFP. 6. H.A.C.C. shall replace all drive ways indicated in RFP with owne approve pav- ers at elevations and pitches consistent with FDOT Standard indexes. 7. H.A.C.C. shall obtain any new permits and maintain existing active permits required by RFP for the duration of the project. Contractor will comply with all permit requirements and ensure the cost associated with that compliance with the RFP is included in the contractor's proposal. 8. H.A.C.C. is responsible for obtaining all necessary permits required by this RFP, which include but are not limited to FDOT, Miami Dade County and the City of Sunny Isles. H.A.C.C. will ensure that the permits remain active for the project duration. Cost is included our bid. 9. H.A.C.C. includes all costs associated with pedestrian and vehicular signage as necessary and required by FDOT permit and MUTCG are included in our bid. Adjustments to properly accommodate pedestrian traffic, at the request of the City, State or County shall be provided at no extra cost. H.A.C.C. shall be responsible for all underground utility implications result- ing from work required by this RFP. H.A.C.C. shall call for utility locations prior to commencement of work. Utility disruptions caused by the Contractor will be resolved immediately and Contractor's expense. 11. H.A.C.C. shall comply with FDOT indexes 300, 515, 600, 613, 660 as required. 12. H.A.C.C. shall repair existing sidewalks as required by RFP. Repairs will ac- complished per the Owner's requirements. All existing signage and bus shelters are to be removed protected and properly replaced. 13. After installation all new pavers shall be sealed with owner approved solvent based acrylic sealer. 14. Pavers shall be in conformance with ASTM C- 936 -82 15. H.A.C.C. will ensure to maintain the integrity of existing curb and gutters. 16. H.A.C.C. will schedule regular weekly progress meetings to update the City. Contractor shall prepare and distribute minutes of all meeting to all those in atten- dance. 17. H.A.C.C. is familiar with all site conditions that might hinder our ability to per- form. Any accommodations and resolution there -of are included in our proposal. 18. H.A.C.C. will obtain, submit and retrieve all applications for permits, final per- mits and pay for all fees and implement all requirement of said permits.
