(CGA) is pleased to submit our Statement of Qualifications to provide professional consulting services for the above referenced project. CGA is a local, well-established multidisciplinary firm with offices located in Fort Lauderdale, West Palm Beach, Orlando, and Homestead with great familiarity in providing landscape architectural services to the City of Sunny Isles Beach. For over 75 years, our firm has been providing consulting services exclusively to the South Florida area. Beyond having experience in providing exceptional solutions to the City of Sunny Isles Beach, we also have served as consultants for several municipalities throughout the region, including the cities of Hollywood, Miramar, Miami Beach, Weston, the Delray CRA, and the towns of Davie, Bay Harbor Islands, and Surfside. In addition, we continuously serve various governmental agencies including the Florida Department of Transportation. Lastly - and pertinent to this project - we have extensive experience in permitting projects with the City, Miami-Dade PERA (former DERM), and numerous state and federal agencies. Collectively, these demonstrate our ability to effectively and continually satisfy our governmental and municipal clients with high-quality services. We have carefully reviewed the scope of services to be provided for this contract, and we have selected a team of qualified professionals possessing the diversity of skills to meet the objectives of the City of Sunny Isles Beach in this park design project. If selected, CGA, located at 1800 Eller Drive, Suite 600, Fort Lauderdale, Florida, will be the responsible office for this contract. Russo, Natasha Russo, and Calvin, Giordano & Associates, Inc., Case No: 0601924(12) in the 17th Judicial Court, in and for Broward County. CGA was a co-defendant in a wrongful death action brought by the deceased's relatives. This case was filed on or about June 30, 2006, and was settled to the satisfaction of both parties in September 2008. 3. Vue Condominium Association, Inc. vs. Vue Fort Lauderdale, L.L.C., ET AL, Case No. CACE09012422 .Suit was filed March 3, 2009. This is a typical condominium construction defect suit involving the developer, contractor, numerous subcontractors and all the design team. The condominium association voluntarily dismissed its claim against CGA in February 2011. 4. Walnut Creek Community Development District vs. Standard Pacific of South Florida, GP, Inc., Et AI, Case No. CACEI0019353. Suit was filed May 4, 2010. The case concerns the failure of an HDPE drainage system and is in the early stages of discovery. The other parties include the developer, the pipe manufacturer and the contractor which is defunct but which may have insurance coverage. Our understanding is that the problems are primarily the result of defective manufacture of the HDPE pipe although design and construction deficiencies also are alleged. 5. Armando Alvarez vs. Keith and Schnars, PA, Calvin, Giordano & Associates, Inc., Community Asphalt Corp., Arazoza Brothers Corporation, Arazoza Brothers Corporation f/k/a Aqualawn, Inc., Vila and Son Landscaping Corp., a Florida Corporation, Edward R. Motor vehicle accident involving a turning vehicle into path of a motorcycle caused by site line obstruction due to bush not properly maintained. CGA was responsible for QC of roadway plans. This case is pending. 6. Calvin, Giordano & Associates and Franklin County, Florida filed lawsuit NO. 4:2012cvOOOOl in the Florida Northern District Court against B.P. The landscaping staff assigned to this project has the capacity to begin immediately working on the project gathering as-builts and permits. Our survey department staff can place a crew in the field within 48 hours of receiving the Notice to Proceed to begin the design survey. From that point, all other services would follow uninterrupted. Based on our experience with these types of projects and more specifically our permitting experience with FDOT and Miami-Dade County on these types of projects, CGA would expect that our team would have an advantage with shorter length of time permitting and fewer RAls than most other firms would. These revealing analyses allow us to better comprehend the built form to identify opportunities and constraints, identify the use of architectural form, space, materials and treatments, open spaces and their design qualities, create micro-climates, and foster sustainable practices. This process is briefly outlined as follows: Inventory - Throughout the inventory phase, all pertinent information affecting the design and impact of a project are assembled and graphically represented, if required by the client. Analysis - Once all the information has been assembled through the inventory phase, design staff conducts analyses to interpret the data - principally to identify and foresee any potential conflicts and constraints, as well as to shed light on additional opportunities available that will further the success of the project, be it via design or through the logistical process of documentation, permitting or construction. This "Kick-off Meeting" will occur after the initial project award meeting between City staff and the CGA project manager. The Project Manager will also be responsible for assigning tasks to the various departments within CGA and also to any applicable sub-consultants. At the "Kick-off Meeting", the team will be introduced to the scope, the budget and the timeline. The Project Manager will define the various elements of the projects and describe the quality control/quality assurance requirements at this meeting. The team's Project Manager will periodically review the progress of the project to assure meeting the project timeline and budget as initially approved by the City. The Project Manager will then update City Staff of the project progress. Regular project meetings will be held with City staff for all phases of the project. The CGA attendees present at the meetings will be dependent on the complexity and the phase of the project. These meetings will be focused on adherence to the project timeline, quality control, budget, and on the resolution of any outstanding issues. Close attention to the budgetary constraints of available funding will be a primary focus of the CGA team. In addition, value engineering analyses will playa critical role in insuring the most efficient design possible. No work will be added to the project without review and agreement by both the City and the CGA project manager; such review will include cost and time impacts in order to identify alternative solutions which can maintain the project's budget. An initial review of the long range estimate will be conducted as one of the first work items under this contract. Verifying the estimated work against the agreed upon project scope will establish the baseline cost for construction. Just as in the design controls stated above, no additions to the scope will be made without a review by the City and the CGA project manager. CGA's project management approach makes quality control an integral part of the project. Thorough and diligent quality control has a direct effect on keeping the project on time and on budget. The quality control concept is a major agenda item at the kickoff meeting with the design team. All team meetings will address the quality by measuring progress against the City's expectations. The team will meet all product require- ments, technical, budget, and schedule or take immediate corrective action to bring the project back in line. Weekly progress meetings are supplemented by a peer review, where a design group not assigned to the particular assignment will review the plans with a fresh perspective. This quality review is performed as part of our quality assurance program. CGA has instituted the policy of a multi-disciplined review of the product before it is released to the client to ensure adherence to the big picture and to counteract any potential lack of congruence in the overall product. We have facilitated significant water reduction use for the Town of Surfside, brought about the implementation of signal progress for more efficient traffic flow as well as the construction of roundabouts in the cities of Weston and Hollywood. We are currently working with the City of Pembroke Pines to implement a wastewater reuse program and on the development of their LEED certified Public works campus. We were the first firm in Florida to assist our clients in implementing energy conservation measures into local comprehensive plans as required by the DCA. Incorporating sustainable, "Green" design into projects has become a priority for municipalities, businesses and homeowners alike. Sustainable Development is not only good for the environment but it has become essential for the responsible operation of municipalities and agencies seeking to foster greater stewardship and extend the benefits of this paradigm to the affected community. Affecting Run-off - As with all of South Florida, Sunny Isles Beach is a relatively flat area of land in a climate that produces above-average rainfall amounts, providing the perfect opportunity to capture and store rainwater. The opportunity to treat or store rainwater to be used later for the supply and enhancement of the immediate area is a reality. . Treating Run-off - Treating the run-off can be effectively done through the use and implementation of rain gardens as a part of the landscape treatment. These water treatment areas can be detailed and designed to be very aesthetically pleasing, while effectively contributing to the green-footprint of the City. . Capturing Runoff - The use of pervious pavements and pavers could be utilized instead of traditional impervious pavements, which direct runoff overland to the structures. This cuts down on the possibility of the runoff mixing with oils and chemicals that contaminate the water, improving the overall quality, as well as keeps the roadways relatively water free after large rain events. CGA designed and managed the construction of a parking lot in the Delray CRA that used these pervious pavements and pavers as a way to capture runoff. We have successful experience, beyond just the design of these, in actually detailing them and administering their construction to guarantee that the system works effectively. . Storing Runoff - Finding a place to store the captured rainwater is a challenge, particularly if the captured rainwater is to be used weeks or months after its collection and should be considered in the design. Storage areas should include some sort of agitation device to keep the water from becoming stagnant so it does not fester into a breeding ground for insects, like mosquitoes, nor becomes septic. The most common use of captured drainage is for irrigation. Our irrigation consultant, Masuen Consulting, has an extensive amount of experience in developing water conservation systems that provide necessary irrigation at minimal uses. Our commitment to sustainability is not only for our projects, as we also apply them to our operations and company policies. We electronically transmit documents as much as feasible to reduce paper use and conduct all of our internal Q/C reviews electronically to avoid printing. We have invested in Fuel efficient Vehicles for of our inspectors that are required to drive to multiple locations as a part of their work schedules. When necessary, we have also established remote office locations to reduce travel requirements. Capital Improvements & Engineering Planning CGA provides many forms of engineering planning services for municipalities infrastructure needs. It begins with assessments of the age, condition, and level of service of existing infrastructure, through projection of the community's needs far into the future. Using the latest technology and accepted design methodologies, CGA then develops master plans/programs to be implemented by the municipality to address the issues revealed in the evaluation process. We then prioritize and price the necessary improvements in order to assist the municipality in developing its infrastructure budget and also to assist in obtaining alternative funding sources for the improvements. These infrastructure improvements vary widely in scope and can range from simple sidewalk and ADA improvements, to large complicated roadway and drainage projects. CGA currently assists the municipalities of Weston, West Park, Surfside, Sunny Isles Beach and other communities with their capital improvement programs. Our water experience includes the design and permitting of new water distribution networks, water distribution system replacements and upgrades, and booster pump stations. This includes water main extension and/or replacement projects for the cities of Weston, Pembroke Pines, Miramar, North Lauderdale, Miami Beach, Dania Beach, Hollywood, Lauderhill, and the towns of Davie, Surfside, and Bay Harbor Islands. In addition, we have modeled existing water distribution systems for the cities of Weston, and Lauderhill, as well as the Town of Davie. This involved assembling existing as-built, permit, and atlas data, and supplementing it with surveyed field data to build the existing model in WaterCad. CGA then proceeded to model improvements into the system for enhanced system performance, to account for future demands, and to accommodate future anticipated redevelopment. The modeled improvements were prioritized, budgeted and included in the Capital Improvement Plans for the municipality. Our wastewater experience includes the design, permitting, and construction of collection systems, transmission systems, lift stations, master pump stations, in-line booster pumping stations, and force mains. CGA has or is currently in the process of modeling the existing wastewater transmission systems (lift stations and force mains) for the cities of Weston, Pembroke Pines, Opa-locka, Cooper City, Dania Beach, and the Town of Davie. As with the water distribution system models, we assembled existing as- built information, permit information, and atlas data, and supplemented it with surveyed field data to build the existing computer model. Similar to the water distribution system evaluation and analyses, CGA modeled improvements into the system for enhanced system performance, to account for future demands, and to accommodate future anticipated redevelopment. In addition, CGA recently performed an 1&1 study on the wastewater collection system of the Town of Surfs ide. Our team has provided designed pump stations for variety of application including stormwater, shallow wells, deep wells, wastewater lift station, wastewater effluent disposal, potable water distribution, and in- line booster station. As such, we have worked with and coordinated with all design disciplines including structural, electrical, and instrumentation and controls. We have experience with a variety of pumps and pump station types. As part of the pump station design we also provided hydraulic modeling, solids removal facilities (Le. bar racks, strainers) design and emergency power fuel system design. We have worked on pumps with capacities ranging from 0.1 MGD to 200 MGD (320 CFS). CGA has designed, permitted, and managed the construction of new lift stations, and lift station rehabilitation projects for the cities of Weston, Pembroke Pines, Dania Beach, Miramar, North Lauderdale, Hollywood, Cooper City, Opa-locka, and the Town of Davie. Stormwater Management CGA has been the Drainage/Stormwater Management Engineer for the South Broward Drainage District (SBDD) since 1990. The firm was originally contracted to develop the first and only district-wide basin hydrologic and hydraulic analysis and storm water comprehensive plan. Furthermore, CGA has also developed stormwater master plans for several municipalities including the cities of Dania Beach, Lauderhill, West Park, Weston and the Town of Surfside. Within these cities, we have provided a number of stormwater-related activities such as stormwater modeling, drainage calculations, pump station design and/or pump station rehabilitation, NPDES MS4 compliance, culvert design, canal extensions and other related hydraulic structures. Our experienced drainage engineers keep abreast of the latest EPA, FDEP and SFWMD regulations with respect to drainage issues. Surveying The Surveying and Mapping Division of CGA utilizes the latest in cutting edge technology when preparing surveys including GPS equipment, automated data collection, laser scanners, underground utility detection technologies and a vacuum/slurry subsurface excavation system called the VaXcavator that exposes existing underground utilities. Operated by one of the field survey crews, it allows us to obtain subsurface horizontal and vertical locations required for design, while realizing cost and timesavings for our clients. Additionally, our state-of-the-art Hypack software with Odom sounder allows us to offer hydrographic surveying services. By integrating with our Trimble GPS units, the equipment creates a three-dimensional model of the water body being surveyed and significant savings for our clients. Recognizing the value to our clients, CGA was among the first firms in Florida to acquire the 3D Laser ScanStation. Our Surveying and Mapping staff can now scan virtually anything in half the time of conventional methods, with a level of detail not available before. The ScanStation can take a virtual, detailed snapshot of a site including ceilings, undersides of bridges, elevated pipe racks, tall facades, columns and towers. Areas that were previously difficult to access. It collects millions of data points of all items in a 3600 horizontal and 2700 vertical field of view, allowing a full field of view. This eliminates an additional visit to capture data not covered in the original scope of work or the question as to where a shot was taken of an object. The ScanStation captures all the items at once in a permanent record that can be filed, including hundreds of high quality photos, and graphics or animations, from 3D to virtual walk-through of sites. The performance of the designed improvements has met the City's expectations. Hallandale Beach NE Quadrant Drainage Study and Improvements, Hallandale Beach, FL, Project Manager. There are a number of drainage facilities within the 200 plus acre project area with limited conveyance capacity and high head loss. Using ICPR, CGA modeled a proposed system of two pump stations connected to sixteen wells while maintaining the existing gravity wells and gravity outfall. These sixteen new wells will be pressurized with fifteen (15) feet of head in order to produce the necessary discharge into the G3 aquifer. FDEP and the Broward County Environmental Resource Department and FDEP have permitted the project. Indian Trace Community Development District, Weston, FL. The City received a small grant from the State of Florida, Division of Recreation and Parks to assist with the development of the park. CGA oversaw all aspects of the development of the park from the conceptual design and site details to the permitting, bidding and construction. From the beginning, the City wanted a strong connection between the park and the Library. A paved walkway leads visitors from the Library directly to a large circular fountain and the "Ring of Florida Authors." The low walk and seating area contains 20 bronze plaques highlighting the literary works of famous authors and playwrights with a connection to Florida. In addition, there is a series of gazebos with terraced seating to accommodate groups from 5 to 6 people, up to 40 to 50 people, in size. The design of the park also features a "walk of discovery," consisting of a series of 10 bronze plaques inlaid in the park's meandering sidewalk that depicts the history of the area dating back to the Tequesta Indians. The park project also includes both site lighting and specialty lighting, as well as a restroom building and a large open, grassed area. GATOR RUN PARK Weston, Florida \ J Gator Run Park is located in the City of Weston adjacent to Gator Run Elementary School. Early design concepts incorporated three-dimensional landforms to create a more spatially interesting and interactive park. The landforms were limited in size to the amount of muck fill that had been relocated on the site. Thus, careful consideration and calculation of fill material was undertaken. Three earthen mounds of varying elevations were designed along with appropriately graded handicap accessible walkways. The range in topography facilitated a range of planting concepts and diverse programmatic elements. The park is primarily passive in nature with ample walk and pathways. There are several active elements including a multi-purpose field associated with the adjacent school and a sand volleyball area. One goal or program element for the park was to create an area designed to attract butterflies, display native plant species, and serve as a nature trail. The program for the park included an open green lawn area that could serve for multiple functions and a dedicated dog run area. Under a very tight and demanding schedule, CGA designed, permitted and provided construction administration services for the entirety of the park. At its core, the park design sought to establish an urban relationship with its surrounding walk-to context. The design provides an urban plaza that is open and accessible at all times, in addition to the multi-use field and dog areas, which are only open during daytime hours. '. 5TH AVENUE LINEAR PARK Dania Beach, Florida The site for the linear park is a 2-mile stretch of parcels that abut 5th Avenue in Dania Beach. The site posed several challenges that needed innovative design solutions in order to successfully work. The first of these was that the site was not continuous - it was a fragmented context interrupted by repetitive street intersections and of inconsistent width throughout. The second is that the site has different frontages - at times it is bound by residential properties' side fences, at times it is bound by the front property line of homes and their driveway access, and at times it is bound by suburban-style shopping centers. The third is that the area presently serves as the drainage swale for 5th Avenue, and, because of the presence of West Lake Park's wetlands to the east of 5th Avenue, the soil had poor percolation qualities and it best serves as retention areas. The budget and the context did not facilitate engineered drainage solutions; therefore the park design had to account for solutions to these as well. The result was a design that unified all the fragments of the park through a sightline organization of gardens spaces: these were intimately scaled so they best related to the residential scale and qualities, while it provided for a strategy that facilitated the provision of art at strategic locations to create a unifying experience throughout. The drainage was resolved through the use of sustainable strategies to site design. SOMBRERO BEACH INTERPRETIVE SIGNAGE Marathon, Florida \ J ) ) \ ) As a component of the overall Sombrero Beach Park redesign and construction performed, CGA also developed a series of interpretive signs to accompany the rules and information signs included in the park. The interpretive signs were focused on providing an educational component to the park, by educating park visitors on the importance of the park site as a turtle nesting habitat and on the impact of human intervention on the sensitivity of their survival. The signs were designed in a way so each plaque (only one shown above) was dedicated to each of the four species know to nest on Marathon Key. The plaques were also accompanied by a series of informational matrices that provided basic information about each species, as compared to one another - such as size, habitat, food and unique characteristics. The signs were all fabricated using an innovative polymer laminate that was UV A and UVB resistant, as well as they could sustain severe impacts without incurring any damage (we tested the material by taking a hammer to a sample of the product where it didn't get a scratch) and spray paint could be easily washed off with a quick solvent. In short, the approach to the scope detailed below is intended on meeting the following goals: . Promoting a sense of cohesion and unity throughout the several park sites; . Promoting a safe connection across all driveways; . Verifying and supplementing any necessary survey data required to design the parks; . Conducting structural analyses of the existing seawalls to assess their load-bearing capacities and/or any required upgrades for the installation of the requested wall and fencing; . Providing a passive park design; . Specifying and detailing the future children's playground; . Ensuring that ADA access is always provided to all facilities; . Detailing the inclusion of possible infrastructure, such as conduits alongside or sleeving at intervals beneath the trails to facilitate for any future provisions of new services such as pedestrian lighting; . Designing park signage and potential interpretive sign age; and . Locating and detailing any furnishings and/or exercise equipment throughout the park. The CGA Team will meet with City Staff and respond to directives provided on the vision and goals anticipated for the project as a means for developing a design solution that meets the City's timetable and allocated budgets. Task 2 - Schematic Design and 30% Plans Based on the design parameters established in Task 1, the CGA team will develop a design for the park and use-components identified in the project parameters, including layout of pedestrian walking paths, lighting, landscape enhancements, wayfinding signage and interpretive markers, furnishings and vita- course elements, pavilions and shelters. 2.1 Existing and Demolition Plans (Scale: 1"=20'-0") The CGA Team will prepare an existing plans defining the project area and identifying any existing elements that need to be demolished or removed, as well as the protective areas around all existing vegetations to ensure there is not root damage. 2.2 Schematic Walkways and Facilities Layout Based upon initial City Staff input, the CGA Team will prepare a Schematic Layout that locates all planned improvements. Landscape Enhancements (Scale: 1"=20'-0") Using the Schematic Walkways and Facilities Layout as a basis, the CGA team will prepare preliminary landscape plans to identify any affecting vegetation, locate planting areas, and identify, by type, significant canopy trees, screening planting and other planting types as well as maintenance requirements for all landscape areas. Based on City feedback, the CGA team will refine the selected landscape plan supplemented with a proposed plant list. Use, Wayftnding, Distance and Interpretive Markers Using the Schematic Walkways and Facilities Layout as a basis, the CGA team will prepare and present alternative conceptual plans, supplemented with sketches and images, identifying possible locations, subject matter and design approaches for use, wayfinding, distance and interpretive features throughout the park. Preliminary Opinion of Cost Estimate The CGA Team will prepare a preliminary Opinion of Cost Estimate to gauge all construction elements to ensure that all elements can be implemented within the established $1 million construction budget. City Review 30% CGA will submit a complete set of all 30% plans to City Staff for review and comment. CGA will meet with City Staff review all comments and obtain direction for further development to be reflected in 60% plans. Task 3 - Design Development and 60% Plans 3.1 Design Development Using the reviewed 30% Plans as a basis coupled with city-issued directives, the CGA team will further develop the design to incorporate all modifications requested and modify all plans accordingly. Additionally, CGA staff will further the detailing of all components to the project to include installation details of selected equipment, furnishings, lighting fixtures, etc. Inherent in the development of the 60% construction plans, the CGA team will conduct a thorough "Constructability Analysis" of all project elements through our Quality Assurance Program to ensure that construction issues and design conflicts are addressed pro actively. 3.2 Preliminary Technical Specifications The CGA Team will develop revised Technical Specifications to reflect construction and installation specifications for all planned components, particularly with an attention to criteria necessary to address 3.3 Refined Opinion of Cost Estimate The CGA Team will prepare a refined Opinion of Cost Estimate to gauge all construction elements modifications to ensure that all elements can be implemented within the established $1 million construction budget. 3.4 City Review 60% CGA will submit a complete set of all 60% plans to City Staff for review and comment. Task 5 - Prepare Permit Applications 5.1 Submittal Packages CGA will prepare all necessary permit application packages to be provided to all applicable and regulating agencies, including but not limited to the City's Building Department, Miami-Dade County's PERA and Water and Sewer Department. The City will be responsible for all permit application fees. 5.2 Agency Comments and Revisions CGA will meet with the City and address any comments needed to obtain approvals from the reviewing agencies. Task 6 - 100% Construction Plans and Bidding Documents After securing all necessary permits, the CGA team will finalize all plans and issue four copies of signed and sealed sets for City records.
