failed to provide products, perform services, or furnished goods for which we have been contracted.    In the last five (5) years, CGA has been involved in the following litigation:  1. James A. Cummings, Inc. vs. Calvin, Giordano & Associates, Inc., Case No: 06‐019973(18) in the 17th  Judicial Court, in and for Broward County.  This case which was filed in 2006 was related to the Palm  Beach County Airport Parking Structure.  The case was settled in August 2007.  2. Yvette Lorenzo, personal representative of the estate and survivors of Orestes Lorenzo vs. Florida  Department of Transportation, City of Pembroke Pines, William J. Russo, Natasha Russo, and Calvin,  Giordano & Associates, Inc., Case No: 0601924(12) in the 17th Judicial Court, in and for Broward  County. CGA was a co‐defendant in a wrongful death action brought by the deceased’s relatives. This  case was filed on or about June 30, 2006, and was settled to the satisfaction of both parties in  September 2008.  3. Vue Condominium Association, Inc. vs. Vue Fort Lauderdale, L.L.C., ET AL, Case No. CACE09012422 .Suit  was filed March 3, 2009. This is a typical condominium construction defect suit involving the developer,  contractor, numerous subcontractors and all the design team.  The condominium association  voluntarily dismissed its claim against Calvin, Giordano February 2011.  4. Walnut Creek Community Development District vs. Standard Pacific of South Florida, GP, Inc., Et Al,  Case No. CACE10019353. Suit was filed May 4, 2010.  The case concerns the failure of an HDPE  drainage system and is in the early stages of discovery.  The other parties include the developer, the  pipe manufacturer and the contractor which is defunct but which may have insurance coverage. Our  understanding is that the problems are primarily the result of defective manufacture of the HDPE pipe  although design and construction deficiencies also are alleged.  5. Armando Alvarez vs. Keith and Schnars, PA, Calvin, Giordano & Associates, Inc., Community Asphalt  Corp., Arazoza Brothers Corporation, Arazoza Brothers Corporation f/k/a Aqualawn, Inc., Vila and Son  Landscaping Corp., a Florida Corporation, Edward R. Wiener, Michael Viscount, Friendly Checker Cab  Co., Inc. d/b/a Friendly Checker Taxi, Case No. 08‐042981‐04. This case was filed September 12, 2008  in the 17th Judicial Court, in and for Broward County. Motor vehicle accident involving a turning vehicle  into path of a motorcycle caused by site line obstruction due to bush not properly maintained. CGA was  responsible for QC of roadway plans.  The case has been dismissed.   6. Calvin, Giordano & Associates and Franklin County, Florida filed lawsuit N0. 4:2012cv00001 in the  Florida Northern District Court against B.P. Exploration and Production Inc. in January 2012. The suit  was filed in an attempt to collect payment from B.P. Exploration and Production for services performed  by Calvin, Giordano & Associates on behalf of Franklin County, Florida. Indication of How Long It Would Take to Implement Service After  Authorized to Begin:  Calvin, Giordano & Associates, Inc. can commence work on any project immediately upon receiving the  Notice to Proceed from the City of Sunny Isles Beach. The construction management staff assigned to this  project has the capacity to begin immediately working on the project.  If needed, many of our ancillary  departments are also ready to commence immediately. From that point, all other services would follow  uninterrupted.  Based on our experience with not only the City, but also the regulatory agencies in the area,  CGA can provide an immediate, uninterrupted response to any notices to proceed. transformation was made to Calvin, Giordano & Associates, Inc. (CGA). From the beginning, much of our  reputation was built on repeat business with clients from the public and private sectors. Our success is  reflected in the quality of work we consistently provide and also in the personal approach we take with  each client. The winning combination is affirmed by the fact that CGA grew steadily during an economy that  was not only challenging but also daunting to many larger firms in Florida these past few years. We focus  on adding professionals and technology that address the needs of our South Florida customers instead of  simply adding new branch office addresses.  Founded in Hollywood, our main office is now in Fort Lauderdale, within Port Everglades. Today, our firm  employs more than 200 professionals who provide a broad range of civil engineering, transportation/ Marathon.  CAPITAL IMPROVEMENTS & ENGINEERING PLANNING  CGA provides many forms of engineering planning services for municipalities infrastructure needs. It  begins with assessments of the age, condition, and level of service of existing infrastructure, through  projection of the community’s needs far into the future. Using the latest technology and accepted design  methodologies, CGA then develops master plans/programs to be implemented by the municipality to  address the issues revealed in the evaluation process.  We then prioritize and price the necessary  improvements in order to assist the municipality in developing its infrastructure budget and also to assist  in obtaining alternative funding sources for the improvements. These infrastructure improvements vary  widely in scope and can range from simple sidewalk and ADA improvements, to large complicated roadway  and drainage projects.  CGA currently assists the municipalities of Weston, West Park, Surfside, Sunny Isles  Beach and other communities with their capital improvement programs.  Various projects in the program  include various storm water drainage and pump stations, FP&L utility under‐grounding, sea wall  replacements, City Hall governmental facility, park facilities, roadway reconstruction and traffic calming,  decorative street lighting, new parking lot construction, landscaping improvements and water and sewer  improvements.  GENERAL CIVIL ENGINEERING  Throughout all phases of a project, CGA makes a commitment to provide the utmost levels  of quality control and professionalism in the realization of the client’s vision and plan. engineering staff also creates, review, and update the codes, standards and ordinances  for a number of municipalities.  In addition, we perform engineering site plan and CGA’s roadway design experience ranges from small, two‐lane  residential roadways to large multi‐lane highways and interstates. We  work with the FDOT on re‐grading, striping, drainage improvements,  implementation of ADA requirements, turn‐lane additions, widening,  and median modifications, and public information/participation.   We have designed streetscape improvements with a particular  attention to pedestrian safety for such municipalities as the Cities of  Doral and Weston, and the Town of Bay Harbor Islands.  In additions, we have designed and permitted parking improvements  such as the Dania Beach Park N Go, and the parking fields for  Pembroke Lakes Square, Pembroke Gardens. Specifically, in terms of  roadway/streetscape related services we include;   Calvin Giordano & Associates, Inc. (CGA) has performed engineering services for a number of parks  throughout South Florida. This has involved the design of drainage and under‐drain systems, grading of  sports fields, parking lot/pedestrian/sports field lighting, the design of parking lots, the extension of  utilities to new or renovated structures, and the overall storm water management system design and  permitting.  Some of the parks in which CGA’s engineering have been involved include; Tequesta Trace Park, Eagle Point  Park, Overtown Park,  Weston Regional Park, Cooper City Sports Complex, Bay Harbor Islands 92nd Street  Park, 5th Avenue Linear Park, and many others.  Civil Engineering for Facilities  CGA has provided civil engineering services for many municipal sites including city halls, public works  compounds, community centers, city garages and other city structures.  This includes the design of paving,  grading, and drainage, water & sewer, pavement marking & signage, storm water pollution prevention  plans and offsite roadway improvements among others. We have brought these designs from the  preliminary stages through the permitting, construction, and certification process for a number of  governmental clients.  Water and Sewer  CGA provides professionals who have experience in all aspects of water distribution, water re‐use main,   wastewater collection/transmission main, and lift station design, permitting, and construction, as well as  system evaluation, and analysis. distribution system replacements and upgrades, and booster pump stations.  This includes water main  extension and/or replacement projects for the Cities of Weston, Pembroke Pines, Miramar, North  Lauderdale, Miami Beach, Dania Beach, Hollywood, Lauderhill, and the Towns of Davie, Surfside, and Bay  Harbor Islands.  In addition, we have modeled existing water distribution systems for the Cities of Weston,  and Lauderhill, as well as the Town of Davie. This involved assembling existing as‐built, permit, and atlas  data, and supplementing it with surveyed field data to build the existing model in WaterCad.  CGA then  proceeded to model improvements into the system for enhanced system performance, to account for future  demands, and to accommodate future anticipated redevelopment. The modeled improvements were  prioritized, budgeted and included in the Capital Improvement Plans for the municipality.  Our wastewater experience includes the design, permitting, and construction of collection systems,  transmission systems, lift stations, master pump stations, in‐line booster pumping stations, and force  mains.  CGA has or is currently in the process of modeling the existing wastewater transmission systems  (lift stations and force mains) for the Cities of Weston, Pembroke Pines, Opa‐Locka, Cooper City, Dania  Beach, and the Town of Davie. As with the water distribution system models, we assembled existing as‐ built information, permit information, and atlas data, and supplemented it with surveyed field data to build  the existing computer model. Similar to the water distribution system evaluation and analyses, CGA  modeled improvements into the system for enhanced system performance, to account for future demands,  and to accommodate future anticipated redevelopment.  The necessary improvements were then  integrated in the municipality’s Capital Improvement Plan. In addition, CGA recently performed an I&I  study on the wastewater collection system of the Town of Surfside.   Our team has provided designed pump stations for variety of application including storm water, shallow  wells, deep wells, wastewater lift station, wastewater effluent disposal, potable water distribution, and in‐ line booster station.  As such, we have worked with and coordinated with all design disciplines including  structural, electrical, and instrumentation and controls.  We have experience with a variety of pumps and  pump station types.  As part of the pump station design we also provided hydraulic modeling, solids  removal facilities (i.e. bar racks, strainers) design and emergency power fuel system design. We have  worked on pumps with capacities ranging from 0.1 MGD to 200 MGD (320 CFS).    CGA has designed, permitted, and managed the construction of new lift stations, and lift station  rehabilitation projects for the Cities of Weston, Pembroke Pines, Dania Beach, Miramar, North Lauderdale,  Hollywood, Cooper City, Opa‐Locka, and the Town of Davie   Storm Water Management   CGA has been the Drainage/Storm Water Management Engineer for the South Broward Drainage District  (SBDD) since 1990. The firm was originally contracted to develop the first and only district‐wide basin  hydrologic and hydraulic analysis and storm water comprehensive plan. CGA is also the District Engineer for  Indian Trace Development District and Bonaventure Development District  and has completed basin wide hydrologic and hydraulic modeling, including  the master drainage system modeling, analysis and pumping station  upgrades, large‐scale mitigation/ wetlands plans and the updates of the  comprehensive plan. CGA uses the latest in advance computer‐assisted  software technologies such as Advanced Interconnected Pond Routing  (ADICPR), Hydraflow, Cascade and other current computer programs. construction plans and specifications for conformance with the District criteria, as  well as representation at District Board meetings to address technical and  engineering issues.  In addition, CGA has been successful in obtaining several millions  of dollars in grants to accomplish the District’s goals and objectives.  Furthermore, CGA has also developed storm water master plans for several  municipalities including the Cities of Dania Beach, Lauderhill, West Park, Weston and  the Town of Surfside.  Within these cities, we have provided a number of storm water‐ related activities such as storm water modeling, drainage calculations, pump station  design and/or pump station rehabilitation, NPDES MS4 compliance, culvert design,  canal extensions and other related hydraulic structures.  Our experienced drainage engineers keep abreast of the latest EPA, FDEP and SFWMD regulations with  respect to drainage issues.  They regularly summarize new regulations, their impact on clients and  recommend actions to comply with the new regulation(s).  ELECTRICAL ENGINEERING  CGA’s Electrical Department has broad capabilities for the infrastructure and the end use development for Cable TV   Street Lighting  SCADA   Sports Lighting  Energy Conservation  SURVEYING  The Surveying and Mapping Division of CGA utilizes the latest in cutting edge technology when preparing  surveys including GPS equipment, automated data collection, Laser Scanners, Underground utility  detection technologies and a Vacuum/Slurry subsurface excavation system called the VaXcavator that  exposes existing underground utilities.  Operated by one of the field survey crews, it allows us to obtain  sub‐surface horizontal and vertical locations required for design, while realizing cost and timesavings for  our clients.  Additionally, our state‐of‐the‐art Hypack software with Odom sounder allows us to offer  hydrographic surveying services.  By integrating with our Trimble GPS units, the equipment creates a three‐ dimensional model of the water body being surveyed and significant savings for our clients.  Recognizing the value to our clients, CGA was among the first firms in Florida to acquire the 3D Laser  ScanStation.  Our Surveying and Mapping staff can now scan virtually anything in half the time of  conventional methods, with a level of detail not available before.  The ScanStation can take a virtual, detailed  snapshot of a site including ceilings, undersides of bridges, elevated pipe racks, tall facades, columns and  towers.  Areas that were previously difficult to access.  It collects millions of data points of all items in a 360°  horizontal and 270° vertical field of view, allowing a full field of view. This eliminates an additional visit to  capture data not covered in the original scope of work or the question as to where a shot was taken of an  object. The ScanStation captures all the items at once in a permanent record that can be filed, including  hundreds of high quality photos, and graphics or animations, from 3D to virtual walk‐through of sites.  It is  ideal for litigations, client presentations or commission meetings. For added convenience, the survey data  can be exported to whatever format our clients need, whether CADD or other software.     RFQ #12‐06‐01, Construction Management Services for Gateway Park Page 2| 5  Process  The process we employ is equally as important as the equipment we deploy.  All survey requests are  assigned to a licensed surveyor and mapper who acts as the project manager.  In addition, fieldwork is  researched and calculated in the office for the crews prior to going out into the field.  The required data is  collected electronically using total station instruments, and then downloaded at the end of the day into our  computer system for analysis and processing. This data is then provided to our technicians to create the  final product, all under the watchful eye of the project manager.  The final product is delivered to the client  after undergoing our Quality Assurance/Quality Control process.   Safety  Safety is a primary concern at Calvin Giordano & Associates.  Therefore, a mandatory maintenance of traffic  (MOT) training is held twice a year for our field crews and field crew supervisors.  Monthly Safety meetings  are held for the field personnel with a different topic discussed each month.  Our field vehicles are  equipped with strobe lights, proper signage, safety vests and cones.  One­Stop Shopping The Surveying and Mapping Division client base includes land development clients, FPL, Florida  Department of Transportation, Broward County Real Property Division and many cities within Broward,  Miami‐Dade and Palm Beach Counties, as well as the general public.  Survey Crews and Equipment  The Survey department maintains seven (7) field crews out of the Fort Lauderdale office and three (3) field  crews out of our West Palm Beach office. Our firm has nine (9) survey grade GPS systems available and we  are capable of both static and RTK GPS surveying. negotiation, implementing quality control and assurance programs, contract  document review, shop drawing approval, review and administration of contractor  RFI’s, observation of field activity to assure construction is completed in  accordance with construction documents, payment request recommendation,  change order administration, environmental and NPDES monitoring, field report  and documentation, as‐built record drawing preparation, and agency/permit  closeout documentation and certification. As part of being a Municipal Consultant  for several local governmental entities, we are constantly involved and up to date  in all related agencies requirements and procedures; we have served as  construction management for over $100 million dollars Capital Projects,  successfully assisting in the bidding, selection of contractor, construction and final  certifications, in conjunction with different design consultants and clients.  ENVIRONMENTAL SPECIALTIES AND PERMITTING  The diverse and in‐depth work experience of our environmental staff provides us with a critical  understanding of current local and regional environmental planning and permitting issues and the historic  impacts that have affected the southeast region, as well as the adaptive strategies that will be necessary to  address climate change and rising sea levels. CGA’s multi‐disciplinary approach is reflected in the  integrated partnership of our environmental department with the many disciplines of our firm. Criteria package. Project acquired using the 2‐stage engineer lead Design‐Build method of delivery.   The CGA Team assisted the municipality with reviewing qualification of design/builders, assistance  with award.  The CGA Team was chosen to act as the as the Owner’s Engineering Representative during  construction.       Designed and coordinated construction of a $15.4 mil water treatment facility expansion. This project  included a 12 MGD anion exchange system for color removal, 12 MGD of treatment unit capacity and 12  MGD filtration system capacity.  Final permitted facility capacity was 16 MGD with 4 MGD of redundant  treatment and filtration capacity. Was Engineer of Record and carried out all project management  functions including coordination of project meetings, shop drawing reviews, inspections and approval  of payment requests. Project was completed as a Contractor led Design/Build project.   Designed and coordinated construction of a $14.2 mil wastewater treatment facility expansion. This  Design/Build project included a permitted system capacity increase from 10.1 to 12.6 MGD (TMADF) new Basin cell to Anoxic operation. Additional improvements include replacement/modernization of  the existing Plant‐Wide SCADA system.  Was Engineer of Record and carried out all project  management functions   Coordinated design of modifying an existing WWTP for aquifer recharge and reuse to gain CUP credits  and meet existing WWTP permit requirements.  Prepared a Pilot Study Protocol and Performance  Study.  The Pilot Plant study of new treatment equipment included: Pilot plant and study design,  Approval of Study by Florida Department of Environmental Protection (DEP), Pilot plant study (6  months operation) including analytical testing.  In addition, he supervised Treatment Plant Design and  preparation of Design Build Documents. The Basis of design report (BODR) included: preparation of  construction cost estimate, process design of new equipment, sizing of new equipment and piping, and  siting of equipment.   Prepared a Neighborhood Improvement Plan for the Bonaventure area of the City of Weston which  encompassed an area of 7.8 square mile.  Some of the major items included landscaping/irrigation,  roadway improvements and drainage improvements.   Prepared the design criteria package for an ion exchange system and was owner’s representative  during construction( $1.5 mil)   Prepared consumptive water use permit applications, including the preparation of the reuse feasibility  study, long‐term water supply plan, water conservation plan and coordination of hydrogeological each utility will operate on a financially self‐sufficient basis.  The studies included collecting, reviewing  and evaluating historical and current financial records, operation and maintenance reports, forecasting  customer growth, water consumption and wastewater for each customer class, reviewing capital  improvement programs.  Reports were prepared for these studies.  This technical report included the  data and tabular displays of the analysis listed above.  The reports were presented to the City  Commissions along with conclusions and recommendations at public meetings on this rate study.   Prepared a Utilities master Plan to examine the existing utility system within the Tow of Davie’s utility  service area.  Estimates of the projected water and sewer needs were made.  The physical condition of  the Town’s infrastructure was examined.  Modeling of entire water and sewage transmission system  was performed.  WaterCad was software used.  Recommendations for improvements to the existing  infrastructure were made as well as recommendations for new infrastructure.  The following items  were examined: Drinking Water, Pumping and Distribution System, Storage Tanks, Water Supply,  Water Treatment Facilities, Wastewater Collection System, Wastewater Transmission Piping, Pumping  System, Wastewater Treatment Facilities   Coordinated design of two High Density Polyethylene (HDPE) pipe Horizontal Directional Drills for the  City of Dania Beach under two different SFWMD canals.  One crossing was under the Dania Cut Canal  and the other under the C‐10 canal.  The scope of the project included 1,000 L.F. (Linear Feet) of new  12” W.M. HDPE pipe, tie‐ins and hot taps to existing water mains, water service connections, and the  removal of existing pipe.  The project was conducted on an expedited schedule to meet key deadline set  by FDOT utility relocation schedule.   Sewage Pumping Station Design – Directed the design of new and replacement pumping stations using  both dry pit pump designs and submersible pump designs from 2.5 Hp to 175 Hp pumps.  Designs  included new concrete valve vaults, discharge piping, new concrete pads, fencing and landscape. The  existing wet wells and receiving manhole were structurally rehabilitated. Stations included electrical  control panels as well as some received separate emergency back‐up generators.   Can Pump Station Conversions – Directed the design of many replacement of existing pumps in the  adjacent dry well with new submersible, non‐clog pumps in the wet well.  The wet wells are generally  rehabilitated to prevent infiltration unless the wet well is not of sufficient capacity and then the wet  well is converted to the terminal manhole and a new wet well is designed adjacent to the existing  structure.  The stations are bypassed during construction, which also included installation of new  submersible pumps, emergency electrical connections and site landscaping.   Performed computer modeling of water distribution and wastewater transmission systems using  WaterCad modeling software for utilities serving from 35,000 to 165,000 customers.   Contract City Engineer for six years for a city with a population of over 65,000.  Responsible for all  capital improvement projects including water, sewer, drainage, paving, landscaping and traffic projects.   Projects included new stormwater pumping stations, roadway widening with additional travel lanes,  bike lanes and bus shelters, installation of traffic signals, installation of round‐a‐bouts, construction of  new parks and upgrades to existing parks, construction of a new city hall, construction of a public  works complex with fueling station, installation of SCADA on all pumping stations. Duties include Prepared Engineering Standards for two municipalities.   Managed the Manatee Isles Drive Improvements project. This is two lane divided roadway, with turn  lanes, in a residential area, connecting several neighborhoods to an elementary school on the east end  and Florida Nature and Cultural Center on the west end. This project included added bicycle lanes,  sidewalk, LED street lighting, drainage improvements, curb and gutter and improved school zone Calvin, Giordano & Associates, Inc. (CGA) has an experienced and diverse staff in the field of construction  management. Our methods of approaching studies and projects have proved extremely effective for clients  throughout South Florida.  The CGA Team has provided the City of Sunny Isles Beach with superior quality work for the duration of  our project management contract. As a result of this relationship, CGA is intimately familiar with the City’s  standards and personnel. Aside from these factors, the CGA Team is qualified to be the owner’s  Construction Managers Support for implementation of capital improvement projects because of our  extensive work with local municipalities and their Capital Improvement Programs.  Understanding the Scope of Work  The first step in any successful project is gaining a complete understanding of the scope and your clients  needs and desires prior to commencement.  CGA is familiar with the City’s staff and logistics from previous  projects within the City of Sunny Isles Beach and its surrounding areas.  Moreover, CGA has personal  relationships with the City of Sunny Isles Beach, its surrounding municipalities and all Agencies Having  Jurisdiction (AHJ).  These personal relationships allow us to tailor our expertise to your needs, providing an  expedited process which exceeds the City’s expectations.  Once CGA has been issued a Notice to Proceed we will schedule a “kick off” meeting with the City and all Project Planning & Controls   As a result of our previous experience with the City, we are fully aware of the requirements and  expectations of the City in regards to CIP project management.  We also understand that no matter the task  or phase of the project, the best formula for success is control and planning.  Chris Giordano, Assistant  Project Manager on this RFQ, served as the Projects Control Manager for the 3rd largest heavy civil marine  contractor in the US prior to his employment with CGA.  Mr. Giordano utilizes this experience to lay the  foundation of every project with proper planning at the onset, and proper controls throughout the duration  of the project.  This formula has proven successful on many of CGAs Construction Management projects,  which is shown by the delivery of on‐time projects that are completed within budget.  CGA utilizes  Primavera Scheduling or Microsoft Project (varies by client preference), S Curves, cost loading &  estimation, and baselines for both construction progress and construction costs to quickly identify any  portion of the project which needs to be addressed before it can become an issue.  This step commences with our CGA in house meeting.  At this meeting, the scope will be assigned to the  relevant in‐house departments.  A project plan, schedule, and controls will be developed.  After approval by  the City of our schedule and controls, these will be set as the “baseline” for this project.  This baseline will  allow CGA and the City to more accurately monitor the project’s progress.  Communication  CGA staff communicates with our clients on a daily, if not hourly basis in person, via email and phone to  ensure that the client is always kept up to speed and any adjustments that need to be made are completed  quickly.  We also acknowledge the major stakeholder in any CIP project are the residents.  Recently, CGA aware of the delicate nature of work being performed within neighborhoods and public right of ways and  would not want to jeopardize the relationship between the City and its residents.  This step will also  continue through project completion.  Project Implementation  CGA will aggressively peruse the completion of each phase of this project.  Again, CGA has a vast experience  with local municipalities and we are aware of the sensitive nature that is CIP budgets & schedules.  Many  stakeholders, residents and elected officials are watching these projects from their homes on a daily basis  and this brings with it extra pressure to perform.  CGA’s main office is located in South Florida and much of schedule.  Throughout the process weekly progress meetings will be held to ensure that every entity  involved is up to speed and all loose ends are being followed up.   Construction Management and Administration – Per the RFQ, CGA will provide certified and  registered staff that will remain on‐site a minimum of six hours per working day to guide and  administer all construction processes in representation of the City.  These services provide ease of  mind that conflicts will be quickly resolved, surprises will be minimized and final budgeted costs  will be held through to completion.   Meetings and Visits ­ CGA will coordinate and document the following through the overall team:   Pre‐Construction meeting   Value Engineering meetings   Attendance of resident information meetings   Weekly construction meetings   Contract  specific meetings   Specialty A/E Consultant site visits   Equipment start‐ups   CGA has served as a CM representative for numerous South Florida municipalities.  Throughout this time,  we have proven our knowledge of record retention laws, a very important issue for audits and public  record requests.  In fact, the City of Weston felt so highly of our organizational skills and knowledge of  record retention law, we were hired to remove, reorganize and re‐file all of their building department files.   All relevant files will be kept at CGA’s main office where our file room is fire suppressed and our servers are  backed up to an onsite tape as well as an offsite server vault.  This triple redundant system ensures that our  client’s files are never in jeopardy.  At the completion of your project, the City will be presented with a  binder and electronic copy of the project specific files.    Quality Assurance/Quality Control (QA/QC)  CGA is committed to providing Exceptional Solutions through quality services and work products in a  manner that promotes the highest level of freedom from deficiencies while seeking the highest level of  customer satisfaction.  To that end, CGA has developed the structure and dedicated the staff required to  achieve these commitments.  The QA/QC process is completed under the leadership of Jonathan N. Cooper,  PE, CGA’s Director of Quality Assurance.  Mr. Cooper brings over 24 years of engineering and construction  expertise to this task and will custom craft a QA/QC plan specifically designed to meet the needs of this  project.  As Gateway Park is a Construction Management project, a Quality Control Plan will be developed based on  the following guidelines:  QC Plan Development Guidelines for Construction Management  Introduction  A contractor quality control plan (CQCP) is the documentation of the contractor's process for delivering the  level of construction quality required by the contract. This document is intended to provide guidance to  contractors, subcontractors, and suppliers as to what is expected from CQCPs and what the criteria for  accepting and using the requirements for these plans will be.  The CQCP is a framework for the contractor's process for delivering quality construction. The plans and  specifications define the expected results or outcome. The CQCP outlines how those results will be  achieved. While it is not possible to determine from the CQCP whether the level of construction quality will  be acceptable, it is possible to verify that the contractor, as an organization, has addressed the basic  elements of its quality process. These guidelines address not only what should be in the CQCP in order for it  to be acceptable to the City of Sunny Isles Beach, but also what elements the City of Sunny Isles Beach's QA  process needs to have in order to assure quality without usurping the contractor's responsibilities.  Outline of Contractor Quality Control Plan Requirements  a. Process control testing. List the material to be tested, tests to be conducted, the location of sampling,  and the frequency of testing  b. Inspection/control procedures. Address each of the following subjects in each phase of construction:   1. Preparatory phase   a. Review all contract requirements   b. Ensure compliance of component material to the contract requirements   c. Coordinate all submittals including certifications   d. Ensure capability of equipment and personnel to comply with the contract requirements   e. Ensure preliminary testing is accomplished   f. Review the contract requirements with personnel who will perform the work   b. Inspect startup of work   c. Establish standards of workmanship   d. Provide training as necessary   e. Establish detailed testing schedule based on the production schedule   3. Production phase   a. Conduct intermittent or continuous inspection during construction to identify and correct  deficiencies   b. Inspect completed phases before scheduled Government acceptance   c. Provide feedback and system changes to prevent repeated deficiencies   d. Description of records. List the records to be maintained   e. Personnel qualifications   4. Document the name, authority, relevant experience, and qualifications of person with overall  responsibility for the inspection system   5. Document the names, authority, and relevant experience of all personnel directly responsible for  inspection and testing   Plan Development  Unless the contractor already has a documented company QC plan, it may be helpful to discuss what is  required and how detailed it will be. Generally, discussion of the basic who, what, where, when, and how  should comprise three to six pages, with an additional two to four pages of detailed information for each  major category of work. This is not including supplemental materials such as subcontractor/ supplier  plans, certifications, test data, and personnel résumés. Also, if the QC plan contains a lot of repetitions of  contract specifications, meaningless platitudes from quality textbooks, and other fluff, its necessary length  will be longer.  Quality­and the processes that deliver quality­is a somewhat open ended concept. No matter how much detail  is in the plan, it can always be argued that more could be, or should be, included.   Organizational Structure  One of the first issues a contractor or any organization must face when designing its QC/QA procedures, is  how these systems will relate to and impact its organizational structure.  Separate Quality Staff  It is intended that the Gateway Park project be staffed by separate quality staff as deemed necessary by the  Director of Quality assurance.  Testing  Testing provides a reflection of quality and the process. But only changes to the process can improve  quality. Extensive testing needed to identify defects so that they can be corrected is an indication of a poor  process. Ideally, frequencies of QC testing are dependent on characteristics of the overall process. In a  transition environment, however, when contractors are not used to designing comprehensive QC systems,  it may be necessary for the agency to provide guide frequencies to minimize the risk of serious deficiencies  undetected until late in the process.  The type, quantity and/or frequency of testing will be established by the Director of Quality Assurance  prior to the commencement of construction.  This shall be based on the scope of work and the contract  documents and will be subject to refinement as the project proceeds.  RFQ #12‐06‐01, Construction Management Services for Gateway Park Page 4| 5  Defects  Acceptance of defects will be based on the following schedule:   The plan is based on an acceptable of 5 percent defects    If defects rise to 10 percent, that suggests additional process control testing and other actions maybe  indicated, including product removal   If defects rise to 15 percent or higher, this indicates serious process control problems, and the  contractor must modifies the process control procedure to address the problem   In addition to those tests specifically required by the contract, the contractor is required to tabulate in the  CQCP all process control testing which will be necessary to assure that the work and material comply with  the terms of the contract when they are ultimately subjected to quality assurance testing.  Inspection/Control Procedures  The following areas of construction shall be subject to inspection for substantial compliance:   Grading and Drainage   Utility piping   Masonry    Pavement Structure    Permanent Traffic Control    Safety Appurtenances    Seeding and Landscaping    Structures    Temporary Traffic Control   Who, What, Where, When, and How?  For each category and phase of the operation, the QC plan should answer these questions as they relate to  the category and phase:  Who will be responsible for QC during the operation? The Quality Control Technician may be assigned  responsibility for testing and documentation and perhaps even training and monitoring of startup. As the  operation moves toward production, however, foremen or other supervisory personnel will probably be  assigned increasing responsibility. If the management official is too high in the organization‐say the overall  project superintendent‐then it is less likely he/she will have the time to perform detailed QC functions. In  that case, subordinate personnel should be specifically identified.  What will that person do to ensure contract compliance? What authority will the person have over  operations? What portion of the time the operation is in progress will the identified person actually be  present to perform QC responsibilities? Testers and inspectors cannot control quality if their  responsibilities are limited to testing, measuring, and documentation. "What" should address not only  personnel but materials and equipment used in the construction. These items often have stated or implied  contract requirements, and the QC system must verify that those requirements are met.  Where will these activities be performed? Will optional process control testing be performed on‐site or  at a commercial laboratory? Will manufactured materials be inspected at the plant, at the contractor's  facility, or at the site of work? Will the equipment be inspected at the yard, or will inspections be performed  at the site?  When will these activities be performed? The earlier QC activities are performed, the more latitude the  contractor has in dealing with problems. However, when activities are performed too early, there is a risk  of unforeseen changes or glitches prior to actual construction. When will test results be available? This is a  key component of the QC plan that determines largely how responsive it can be to deficiencies.  RFQ #12‐06‐01, Construction Management Services for Gateway Park Page 4| 6  How will inspections be performed? Using a standard checklist? Using the specifications themselves  (quality assurance criteria), etc.? The more generalized and vague the inspection procedures are, the more  likely they will not be consistently effective. However, not having a checklist is not a cause for disapproving  a QC plan unless a checklist is specifically required.  The CQCP should minimize any parroting or paraphrasing of requirements in the contract, and should  avoid simply promising to comply with the contract. These kinds of statements and assurances are of  essentially no added value. The CQCP must go beyond the contract requirements and address the  contractor's organizational process for consistently delivering those requirements.  Subcontractors and Suppliers  When subcontractors and suppliers provide part of the work, then the QCP needs to be clear whether their  QC responsibilities will be independent or a part of the prime contractor's responsibilities. If they are  independent, then the subcontractors or suppliers QCP must be developed and submitted for approval,  through the prime. Otherwise, the prime must address how it will monitor and verify  subcontractor/supplier quality as a part of its plan. In either case the prime is contractually responsible for  all the work, but being contractually responsible is not the same as having an active role in the quality  delivery process.  Manufactured Materials  An important part of the CQCP is the process for verifying that manufactured materials comply with the  requirements of the contract.  Materials inspection will subject to the quantity of materials provided per  category.  Records and Documentation  While good documentation is often a reflection of good quality control, documentation is not the same  thing as quality control. Documentation should be the minimum necessary to concisely document the  adequate function of the process.  As‐built survey documentation will be provided based on the  requirements of the Contract Documents.  Personnel Qualifications  While some contracts may have specific required qualifications for contractor quality control and testing  personnel, the initial judgment as to whether a given person is or is not qualified is generally left to the  contractor. However, during contract administration, CGA may be more assertive in monitoring the  qualifications of these personnel. When the contract has specific experience requirements, the contractor  should describe how the person's previous training and experience addresses these requirements.  Contractor Evaluation  Evaluations are made of five individual elements, plus an overall evaluation:   Quality of Work    Timely Performance    Effectiveness of Management    Compliance With Labor Standards    Compliance With Safety Standards   The first of these five elements, Quality of Work, essentially overlaps the contractor's inspection system  requirements. That is, a contractor that fails to maintain an effective quality control (inspection system)  will generally warrant an unsatisfactory rating in the Quality of Work category. Deficient contractors must  be clearly notified of the deficiencies and provided an opportunity to correct them.  RFQ #12‐06‐01, Construction Management Services for Gateway Park Page 4| 7  Evaluations may be used in part for determinations of responsibility prior to award of sealed bid contracts  or in evaluating past performance as a part of source selection for a negotiated contract.  If the prime contractor's performance would be evaluated as satisfactory but for the performance of a  major subcontractor, it is permissible to execute a separate evaluation of the subcontractor, following the  same rules as if it were a prime.
