Pull out original RFP Page # 33 Replace with Revised Page # 33  Moving forward all vendors MUST submit a readable / searchable PDF CD or flash drive containing the contractor’s entire proposal.  Response to Questions:  How are permit fees handled? Permits are required from the City of Sunny Isles Beach Building Department, Miami- Dade County Department of Environmental Services, and Miami Dade County Fire Department. Permit fees will be a direct reimbursement of actual fees paid and are expected.  Can the City provide copies of the building plans for the facilities? All building plans are exempt documents. However, contact the City Clerk’s Office for obtaining and completing a form and a CD of the plans will be furnished.  Are prevailing or Davis Bacon wages required? No.  Is there a local preference? No.  What area is available for staging, staff parking, and dumpsters? Parking is very tight in the City. The surface lot south of City Hall and on-street parking is on a first come first served basis. In addition, portions of the secured lot north of City Hall (the “Alamo” property) can be made available for no less than six (6) months from award.  What is the contract time? Substantial completion in one hundred eighty (180) calendar days. Per City Code, no construction work is allowed on New Year’s Day, Fourth of July, Labor Day, Yom Kippur, Thanksgiving Day and Christmas Day.  Is Builders Risk Insurance required? No, Builders Risk Insurance is not required. Note that the City must be listed as an additional insured on General Liability/Completed Operations Liability with a $5,000,000 limit. See Revised Page # 11.  Will a geotechnical report be required for any element of the work? No.  The RFP includes a testing protocol for the existing building. Can a proposer get approval for alternate test method(s) or protocol(s)? Yes, as long as the tests provide the needed results.  When is the project expected to be awarded? We expect to ask City Commission to award the bid at their September 18, 2014 meeting. Request for Proposal Goods & Services Government Center Repairs Request for Proposal No. 14-07-01 Publish Date: Bid Due and Bid Opening Date: July 3, 2014 August 6 13, 2014 at 2:30 3:00 PM Mandatory Pre-Bid Conference: Where to Deliver Proposal July 16, 2014 at 2:30PM Sunny Isles Beach Government Center 18070 Collins Avenue Sunny Isles Beach, Florida 33160 CITY OF SUNNY ISLES BEACH |Request for Proposal No. 14-07-01 - REVISED 1 CITY OF SUNNY ISLES BEACH |REQUEST FOR PROPOSAL NO. 14-07-01 - REVISED 10 Section 2 Special Terms and Conditions 2.1) PURPOSE OF BID: The City of Sunny Isles Beach Florida (“City”) intends to secure services of a qualified contractor for repairs to the Government Center and the Government Center Garage. Bids shall include the replacement of the existing 500 kw emergency generator and the addition of a new 5001000 kw stand-by generator and a one or more phase maintenance and renovation projects for the provision of building envelope repairs, fountain repairs, cast in place concrete and pre cast concrete panel repairs, painting, sealing, tile and metals replacement. These repairs shall take place at the Government Center and other related buildings and facilities and parks along with Public Works. The work will require the contractor to obtain engineered signed and sealed shop drawings and obtain permits. 2.2) PRE-BID CONFERENCE: A mandatory pre-bid conference will be held on 07/16/2014 10:00AM at the Sunny Isled Beach Government Center, 18070 Collins Avenue, Sunny Isles Beach, FL 33160; 1st Floor Conference Room to discuss the special conditions and specifications included within this solicitation. Those who fail to attend this mandatory pre-bid conference will not be considered by the City and their bid will be rejected. Bidders are requested to bring this solicitation document to the conference, as additional copies may not be available. 2.3) TERM: Work on the base contract shall start in August 2014 or no later than 14 days after the contractor receives a Notice to Proceed (“NTP”) and completed within 9 calendar months from the Notice to Proceed. This contract shall commence upon the date of Notice to Proceed and shall remain in effect until such time as the commodities, equipment and/or services acquired in conjunction with this request for proposal have been completed and accepted by the City’s authorized representatives and upon completion of the express and/or implied warranty periods. 2.4) OPTIONS TO RENEW: Intentionally Omitted CITY OF SUNNY ISLES BEACH |REQUEST FOR PROPOSAL NO. 14-07-01 - REVISED 11 2.5) METHOD OF AWARD: See Section 4 2.6) PRICE: Prices shall remain fixed and firm through the term of the contract. 2.7) PERFORMANCE BOND: The City of Sunny Isles Beach shall require the successful Proposer to furnish a Performance and Payment Bond in the amount of 100% of the total Proposal Price, with the City of Sunny Is les Beach as the Obligee, as security for the faithful performance of the Contract and for the payment of all persons performing labor of furnishing materials in connection herewith within ten (10) calendar days after notification of the award by the City. The binds shall be with a surety company authorized to do business in the State of Florida. 2.8) BID BOND The City of Sunny Isles Beach shall require that every Proposer furnish a Bid Bond in the amount of 5% of the total Proposal Price. 2.9) DELIVERY: Intentionally Omitted 2.10) INSURANCE: 2.10.1) Comprehensive General Liability Insurance Comprehensive General Liability with minimum limits of One Five Million Dollars ($1,000,000.00) (5,000,000.00) per occurrence combined single limit for Bodily Injury Liability and Property Damage Liability. Coverage must be offered in a form no more restrictive than the latest edition of the Comprehensive General Liability policy, without restrictive endorsements, as filed by the Insurance Services Office, and shall specifically include the following with minimum limits not less than those required for Bodily Injury Liability and Property Damage Liability:  Premises and Operation;  Independent Contractors; CITY OF SUNNY ISLES BEACH |REQUEST FOR PROPOSAL NO. 14-07-01 - REVISED 17 Section 3 Scope of Services / Technical Specifications 3.1 SCOPE OF SERVICES The City of Sunny Isles Beach, Florida, (the “City”) a municipality located in Miami-Dade County, Florida, seeks Request for Proposals (“RFP”) responses from interested licensed and certified Florida general contracting professionals (“Contractors”) to submit proposals to provide the City of Sunny Isles Beach with all the necessary work, labor, and equipment to secure services of a qualified contractor for repairs to the Government Center and the Government Center Garage. Bids shall include the replacement of the existing 500 kw emergency generator and the addition of a new 5001000 kw stand-by generator and a one or more phase maintenance and renovation projects for the provision of building envelope repairs, fountain repairs, cast in place concrete and precast concrete panel repairs, painting, sealing, tile and metals replacement. Work shall include but shall not be limited to building envelope water proofing and painting, concrete repairs, stucco and finish repairs, precast vertical panel stains, stainless steel stair rail replacement, sprinkler hea d and escutcheon replacement, repainting of doors, replacement of camera housing, removal and replacement of existing emergency / standby generator, installation of new 5001000 KW diesel stand-by generator and fountain waterproofing with tile/wall repair. Contractor must demonstrate his/her qualifications by providing experience in the areas of streetscape construction as a prime contractor to the FDOT or other Municipalities/Local government on similar projects with similar conditions. Contractor must provide sufficient information that clearly demonstrates the Contractor’s ability to provide such services in a prompt and efficient manner. This includes but is not limited to the Contractor’s ability to coordinate with FDOT, Miami-Dade County, City staff and neighboring property owners that may be impacted by construction. It is understood that the selected Contractor is of a professional status, licensed to provide services in the State of Florida and licensed for all applicable professional discipline(s) requiring licenses. 3.2 PROJECT SPECIFIC REQUIREMENTS 3.2.1) Building envelope and water proofing and painting Building has numerous cracks and points of water intrusion throughout the envelope. The contractor is required to examine the building and provide services to install and repair new paint and caulking on all previous painted surfaces and caulked joints on the building exterior and in the parking garage including the interior and exterior of stairway #4 and #5, Includes painting entire parking garage and exterior of administrative building (previously painted surfaces only including stairs and rails) Caulking around edges of previously caulked parking garage) CITY OF SUNNY ISLES BEACH |REQUEST FOR PROPOSAL NO. 14-07-01 - REVISED 21 New 500 1000 KW or equivalent diesel generator to replace existing Emergency /standby Generator currently housed in first floor generator room, NTE existing load. New generator shall be in a sound attenuated, aluminum UL listed marine enclosure capable of withstanding rooftop exposure, with integral radiator and muffler. Generator shall be wired into existing Emergency circuits and must meet the existing electrical loads and characteristics of the system it replaces. Perkins or equivalent. The generator shall be fueled from existing site tank by means of a new pump and piping extending the existing point of connection in or near the generator room. Wiring switch gear and conduit, labor, piping required to install and connect new generator to the first floor equipment locations shall be included. It is expected that no new conduit or wiring is required from the existing generator in the old generator room to the loads. Exterior Switch gear, junction boxes and devices, conduit, piping required for proper connection must meet roof top exposure requirements, including NEMA III as may be necessary. System must be added to lightning protect loop. Generator shall be equipped with a double walled steel base tank capable of 8 hours minimum continuous operation on diesel fuel. Entire system must be connected to existing main storage tank through existing plumbing in the current emergency generator room. All controls, switches, valves, pumps must be installed to provide the same operation and intent of the existing day tank and system configuration as shown in attached diagram. Work will include tie downs, and generator and associated equipment and shall be installed on a level Aluminum frame or suitable slab capable of distributing the weight across the foot print of the generator and adjacent area required for weight distribution, meeting all wind load and building code requir ements. The generator will be protected on driving side of the parking garage by a 4 inch schedule 40 steel bollard painted yellow - minimum 2 locations City may elect to exercise a direct purchase option to save on sales tax expenses. 3.2.13 New 500 1000 KW diesel stand-by generator In an aluminum or marine enclosure capable of withstanding rooftop exposure, with integral radiator and muffler. Generator must have the capacity to power parts of the Administrative building 4th floor and miscellaneous loads. The included loads are TBD during shop drawing development process but shall include air conditioning of fourth floor, lights and facilities NTE 450 1000 KW. Generator system is to be equipped with ATS installed in or near the old generator room, and accessory controls and distribution sub panels and wiring to provide standby capacity for the fourth floor of the Government Center including providing air conditioning and task lighting. Generator shall be compatible with existing voltage, and wiring in the building. The generator shall be fueled from existing tank by means of a new pump and piping extending the existing point of connection in the generator room. Generator to be fueled from point of connection in old generator room, work shall include, pumps, piping and CITY OF SUNNY ISLES BEACH |REQUEST FOR PROPOSAL NO. 14-07-01 - REVISED 33 Section 5 Proposal Format 5.0 FORMAT Proposers must submit (1) original and four (4) copies of the Proposal ALONG WITH A READABLE SEARCHABLE PDF CD OR FLASH DRIVE CONTAINING THE CONTRACTORS ENTIRE PROPOSAL. The original Proposal must contain an original signature. Be sure that the individual signing the Proposal is authorized to commit the Proposer's organization to the Proposal as submitted. Each page of the Proposal should state the name of the Proposer, the RFP number, and the page number. The CITY reserves the right to request additional data or material to support Proposals. All material submitted in response to the RFP will become the property of the CITY. LABEL EACH SECTION AS NUMBERED The proposal must be in the following format. 1. Company Information  In response to this Proposal, all Proposers must provide the following:  Name of Agency/Company (including any "Doing Business As" names)  Company Locations  Internet Web Site Address (if any)  Details of Entity Business Structure (Corporation, Partnership, LLC)  Date Founded  Home office address and telephone number, and local address and phone number  List of any outstanding litigation that would threaten the viability of the firm or the performance of this contract  Proof of insurance  Indication of how long it would take to implement service after authorized to begin  Financial Stability of Consultant 2. Qualifications Proposer’s relevant experience, qualifications and past performance  An explanation of why the Proposer is the best qualified to perform the contract and demonstrate its qualifications including an item-by-item disclosure outlining how the firm meets or exceeds the requirements of this RFP.  A schedule of proposed services. The schedule should include the Proposer's understanding of the issues and tasks of the project at hand. 3. Staffing Relevant experience and qualifications of key personnel, including key personnel of subcontractors, that will be assigned to this project and experience and qualifications of subcontractors, the size and experience of the company staff pool from which staff assigned to the management contract can be drawn.
