HomeMy WebLinkAboutAdd No 10 Addendum 10 CITY OF SUNNY ISLES BEACH CONSTRUCTION OF THE NORTH BAY ROAD PEDESTRIAN EMERGENCY BRIDGE CITY RFP NO. 14-09-01 To All Bidders: Bidders for the above referenced project shall take note of the following changes, additions, deletions, clarifications, etc. to the Plans and Specifications, which in accordance with the Contract Documents shall become a part of and have precedence over anything shown or described otherwise.  Attachments: Table 1 – MAJOR ITEM SCHEDULE OF VALUES US Coast Guard Permit #9-14-7 dated 20 February 2015 Bridge Plan Sheets BW-2, BW-3, BW-4, and B-24 (Revised) Roadway Plan Sheets 1, 4, 5, 11, 12, 14, 16-23, 26 (Revised) Roadway Plan Sheet 18-A (New)  Questions/Clarifications: Q1: Will additional plan sheets be provided for MOT? If not, will MOT restrictions be provided to clarify work time restrictions, minimum lanes required, pedestrian requirements, etc. A1: MOT plans for approval shall be submitted by the Contractor to the City for review and approval. MOT shall be placed and maintained in accordance with FDOT Standards and provide for the safe movements for both vehicles and pedestrians at all times. Work time restrictions are listed in the City of Sunny Isles Beach Code of Ordinances Article III – Construction Noise §193-6. Q2: It appears that the drainage plans on the north side are incomplete – plan view page 12 does not show any drainage while the cross sections, sheet 19- show 19” x 30” pipe and 5’x5’ exfiltration trench. Will roadway plans be revised to show drainage? A2: The City of Sunny Isles Beach has a separate drainage construction project not associated with this Emergency Bridge project. The drainage project includes a new outfall and related infrastructure within the North Bay Road right of way between 174 Street and the canal. The drainage scope of work is currently included in the 174th St project (By Others) and as such, the cross sections are depicted as existing. Extensive coordination with the 174th St Contractor will be required as part of this project. In the event the drainage project is delayed or withdrawn, the City will solicit additional pricing from the contractor for this Emergency Bridge project to construct the outfall and related drainage improvements. Q3: Typical section sheet 4 shows an interior barrier wall adjacent to the 5’6” sidewalk. MSE plans and roadway plans do not show this wall. Will revised drawings be provided for the typical sections, MSE or roadway drawings? A3: The plans have been revised to eliminate the interior barrier wall. Q4: Typical section sheet 4 shows a pedestrian barrier wall, but does not show a sidewalk or standard moment slab. Which moment slab detail should be followed here? A4: The Roadway Drawings have been revised to show the standard moment slab/junction slab of the MSE Wall Details. Q5: Ground improvement are shown from 20+70 to 21+70 and 24+12 to 26+40. Will any demucking be required beyond this area? A5: The limits of demucking will be dependent upon the means and methods employed by the contractor noting that the plan data is based upon the conditions found at the individual boring locations. While best professional efforts and due care were put forth in quantifying this item, field conditions may or may not match these limits exactly. The contractor shall satisfy himself of the limits of muck that must be removed for the project be constructed in accordance with the plans and specifications. Refer to Note 2 on Sheet BW-1 of the Bridge Drawings. Further, the regular excavation and embankment volumes shown in the plan sheets have been calculated without regard to subsoil excavation because the contractor is responsible to choose from the method of soil improvements shown in the Geotechnical Report, which was issued with and is a part of the contract documents. The chosen method of soil improvement would impact the volume, so the excavation and embankment volumes are calculated based upon the existing ground surface elevation. The contractor must account for the differential cost of excavation and embankment in the cost of soil improvement Addendum No.10  CONSTRUCTION OF THE NORTH BAY ROAD PEDESTRIAN EMERGENCY BRIDGE  CITY RFP NO. 14‐09‐01        3      Q6: Document 00300 Bid Form 1. A) 1 indicates ………the base contract for the lump sum prices with schedule of values which includes the entire project except for the following items: Install asphalt pavement, sidewalks and stormwater drainage. These items are included in the schedule of values. Please clarify this is confusing. A6: Please see attached revised Table 1, MAJOR ITEM SCHEDULE OF VALUES. Q7: Please clarify the requirements for vibration monitoring for this project. A7: The purpose of the vibration monitoring is to protect the contractor from claims of inadvertent damage to nearby private properties resulting from construction activities. The extent of monitoring during appropriate phases or elements of construction is to be determined by the contractor and shall conform to FDOT requirements. See Note 10 on Sheet B-11 of the Bridge Drawings. Q8: Will the contractor be required to supply a Builders Risk insurance policy? A8: Please refer to RFP Section 2.10 Insurance. Q9. Section 6, Bid Submittal Forms, page 22 of the RFP, refers to an excel form to be inserted, we have not received this form, when will it be issued? A9: See attached Table 1 MAJOR ITEM SCHEDULE OF VALUES. Q10: Does the concrete for the structure have to be FDOT certified? A10: Yes. Q11: There is no requirement for an Engineer’s Field Office or Trailer, please confirm that we are not to provide one as part of our bid price. A11: The contractor will be allowed to occupy a portion of the North Bay Road right of way from Station 26+00 south to the canal, outside the limits of the 174th St drainage improvements referenced in Q2). This area can be used for staging, storage of non-hazardous materials, and for a required construction trailer to be used for maintaining record drawings and documentation as well as hosting periodic site meetings during construction. This trailer shall have electrical service, Addendum No.10  CONSTRUCTION OF THE NORTH BAY ROAD PEDESTRIAN EMERGENCY BRIDGE  CITY RFP NO. 14‐09‐01        4      space for maintenance of project drawings and records and at a minimum a suitably sized conference table and sufficient chairs for up to five (5) persons in addition to contractor staff. The overall area is to be secured as needed with perimeter fence and screening and be kept in a neat and orderly manner at all times. The exact limits of this area shall be coordinated upon mobilization to address needed access for a contractor who may be under construction of drainage improvements in NE 174 Street and a portion of North Bay Road. Q12: The MSE Wall Plans on BW-2, show the begin south MSE wall station as 21+29.73, and BW-3 show end north MSE wall at stations 24+90+00 and 25+00+00. This does not match the roadway plans which show south MSE wall limits at stations 20+34.17 and 20+60.14, and north MSE wall limits at 26+23.96. Please clarify which is correct. The MSE Wall Plans on BW-2, show the begin south MSE wall station as 21+29.73, and BW-3 show end north MSE wall at stations 24+90+00 and 25+00+00. This does not match the roadway plans which show south MSE wall limits at stations 20+34.17 and 20+60.14, and north MSE wall limits at 26+23.96. Please clarify which is correct. A12: The limits of the MSE wall shown on the Roadway and Bridge Drawings have been revised, please see the attached plan sheets. Q13: The bridge plan B-5, show variable height median barrier wall beyond the MSE wall limits per FDOT index 410. This index allows a four foot maximum height differential which will be exceeded per the elevations shown on plan sheets BW-2 and BW-3. Please clarify whether this index will be allowed for height differentials exceeding four feet. A13: The limits of the wall have been revised so that no portion exceeds four (4) feet in height. See revised plan sheets. Q14: Plan sheet B-4 requires an ADA rail on one bridge barrier, the ADA rail is not shown on the approach slab plan (see B-68) or on the roadway cross sections. Please clarify that the ADA rail is only on the bridge. A14: In addition to the bridge barrier, railings are required for the Approach Slab & MSE Walls that shall terminate at an appropriate location above adjacent grade. Addendum No.10  CONSTRUCTION OF THE NORTH BAY ROAD PEDESTRIAN EMERGENCY BRIDGE  CITY RFP NO. 14‐09‐01        5      Q15: The drilled shaft plans (sheets B-6 through B-11) do not indicate any load tests being required for the foundation shafts. Please confirm that none are required. A15: Shaft capacity has been designed assuming shaft will be installed as per FDOT Standard Specifications without a load test. The Resistance Factor was used accordingly and as such a load test is not required. If considered necessary by the contractor, then the contractor can perform a load test without any additional cost or time to the project Q16: Will a “method shaft” be required for the drilled shaft foundations? A16: As per FDOT Standard Specifications, Method Shaft is required. Q17: Are SID tests required for the drilled shafts? A17: SID tests are not required. Q18: For any testing of drilled shafts, who is to provide for the inspection of the testing? A18: The City of Sunny Isles Beach shall provide for the inspection of the testing. Q19: If the Contractor elects to utilize permanent casing for the drilled shafts, is there a required cutoff elevation of the casing? A19: If the Contractor elects to use permanent casings for drilled shafts, the cutoff elevation shall be two (2) feet below the mudline. Q20: Please clarify that the Contractor is to provide QC testing for concrete, and that the Owner will provide QA testing and inspection. A20: The contractor shall provide QC testing for concrete and the Owner shall provide QA testing and inspection. Sheet 26, Storm water Pollution Plan, indicate a much larger quantity being required. Please clarify. A21: See attached Table 1 MAJOR ITEM SCHEDULE OF VALUES. Q22: The schedule of values, Item 80, requires 300 LF of decorative fence with gates, please clarify where the fence is located. A22: Please refer to the Roadway Plans Sheet 11 that states “Replace Existing Fence, Field Verify Limits. Match Existing”. Q23: Please clarify whether this Contract is LUMP Sum or Lump Sum with Unit Pricing per the schedule of values, (i.e. Lump Sum for the Bridge and Approaches and Unit Price for the Roadway items). It’s not clear what the term “Lump SUM Price with Schedule of Values” means that is referred to in the bid form (Document 003300 – sheet 00300-1). A23: The contract is lump sum with the Schedule of Values to be used as the basis for additions and/or deletions. Q24: After carefully reviewing all project specifications, by hiring a subcontractor with extensive bridge experience who will be performing the majority of the construction, meet or oversee the Contractors qualification stated in section 4.2 of the project manual? A24: The City intends to award to the contractor it determines to be the most responsive and responsible.
