Addendum # 2
Please attach this addendum to the documents in your possession and include the
Acknowledgement of Addenda form with your response incorporated in the RFQ.
Revision: (Note: Deletions will be indicated by strikethrough, additions will be indicated with bold
font):
2.9.4 Professional Liability Insurance
Professional Liability Insurance including Errors and Omissions with minimum limits of One Million Dollars
($1,000,000.00) Two Million Dollars ($2,000,000.00) per occurrence.
Questions & Answers:
1. In regards to the 5 year limit, will there be consideration to projects that are a little older than
that, say projects that were completed in the past ten years, due to the economy coming out
of a recession?
A. For this criteria, evaluation will be focused on government projects of similar size and scope
within the past 5 years. As noted in Section 5 (item E) Concentrate only on those
services rendered by the Firm within the past five (5) years, which mostly match the scope of
services.

references that are not from a government agency.
A. References other than government entities may be submitted, however government and
local government references are preferred.
3.
with but not necessarily designed a civic/government building for?
A. Projects must have been for a similar scope as requested in the RFQ.
4. Section 5, Item E, the City of Sunny Isles is requesting copies of executed agreement. Is this
mandatory? Our client agreements are confidential at the request of our client and our legal
department.
A. Please submit executed agreement(s), if any, for the past 5 years between firm and
government agency (only include documents which are deemed public records). Please indicate
if your firm was a sub-consultant to the firm that was awarded a government contract.
Contracts with private entities are not being requested.
