A RESOLUTION OF THE CITY COMMISSION OF THE CITY OF
SUNNY ISLES BEACH; FLORIDA, APPROVING SECOND
AMENDMENT TO THE PROGRAM GRANT AGREEMENT FOR
ADDRESS VERIFICATION BETWEEN THE CITY OF SUNNY
ISLES BEACH AND SCHOOL BOARD OF MIAMI-DADE
COUNTY, ATTACHED HERETO AS EXHIBIT "A", IN
SUBSTANTIALLY THE SAME FORM; PROVIDING FOR THE
CITY MANAGER AND THE CITY ATTORNEY WITH THE
AUTHORITY TO DO ALL THINGS NECESSARY TO
EFFECTUATE THIS RESOLUTION; PROVIDING FOR AN
EFFECTIVE DATE.
WHEREAS, the City and the School Board entered into a Program Grant Agreement for
Sunny Isles Beach Address Verification Plan on or about January 9, 2017; and
WHEREAS, the City wishes to enter into a Second Amendment to the Program Grant
Agreement to extend the term of the Agreement until October 1, 2018.
NOW THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF THE
CITY OF SUNNY ISLES BEACH, FLORIDA, AS FOLLOWS:
Section 1. Approving the Second Amendment to Program Grant Agreement. The City
Commission hereby approves the Second Amendment to Program Grant Agreement between the
City of Sunny Isles Beach and School Board of Miami Dade County attached hereto as Exhibit
"A" in substantially the same form to extend the term of the Agreement to October 1, 2018.
Section 2. Authorization of City Manager and City Attorney. The City Manager and City
Attorney are hereby authorized to do all things necessary to effectuate the terms of this
Resolution.
Section 3. Effective Date. This Resolution shall become effective upon adoption.

This Second Amendment to the Agreement between the CITY OF SUNNY ISLES BEACH
("City") and THE SCHOOL BOARD OF MIAMI-DADE COUNTY, FLORIDA, ("School Board")
executed this day of July,2018, is made a part of the original Agreement("Agreement")dated January
9, 2017, between the City and School Board, a copy of which attached hereto as Attachment "A", for the
purpose of extending the term of the Agreement.
WHEREAS,the City and the School Board entered into a Program Grant Agreement for Sunny Isles Beach
Address Verification Plan on or about January 9, 2017; and
WHEREAS, the City and the School Board wishes to enter into a Second Amendment to the Program
Grant Agreement for Sunny Isles Beach Address Verification Plan to extend the term of the Agreement
until October 1, 2018; and
NOW, THEREFORE, in consideration of the mutual covenants and for other good and valuable
consideration each to the other,receipt of which is hereby acknowledged by each party,the City and School
Board agrees to the following:
1. EXTENDED DATE. Section 4 of Program Grant Agreement and its Amendment is amended to
extend the term of the Agreement until October 1, 2018.
2. OTHER PROVISIONS REMAIN IN EFFECT. Except as specifically modified herein, all
terms and conditions of the original Agreement between the parties, dated January 9, 2017, and attached
hereto as Attachment"A", shall remain in full force and effect.
IN WITNESS WHEREOF, the parties hereto have executed this document as of the date
mentioned above.

RE: Extension of Program Grant Agreement for Address Verification with Miami-
Dade County Public Schools.
Recommendation:
This Resolution is presented for your consideration.
Reasons:
By letter dated July 6, 2018, Miami-Dade County School District requested an extension
of the Program Grant Agreement for Address Verification between the City and the
School District. As such, the attached Resolution is to extend the Agreement until
October 1, 2018 as requested. Please note the Agreement was set to expire this summer.
