SUBJECT: Addendum No. 1
SUBMITTAL DEADLINE: Tuesday, September 4, 2018 at 2:30 PM
Please attach this addendum to the documents in your possession and include the Acknowledgement
of Addenda form with your response incorporated with the Request for Qualifications.
Revisions: (Note: Deletions will be indicated by strikethrough, additions will be
indicated with bold font):
1.Section 3.1 Scope of Services:
The firm will incorporate such data with new design information to be provided to the public
utility service providers as required. The firm will assist in the conversion of existing overhead
utilities into underground services from North Bay Road to Atlantic Boulevard and from 185th
street to 191st. The engineering services will also include incorporation of a new community wide
(Golden Shores) roadway systems, upgrades to the stormwater system as needed, limited
new sidewalks from the entrances of Collins to Atlantic Blvd and along the east side of Atlantic
Blvd, new landscaping, restoration of all disrupted facilities and driveways caused by
construction activities, and a design of new street lighting.
2.Section 5(F) Page 24:
Provide a resume for all proposed personnel on the Respondent’s team that will be assigned to the
Contract to meet the requirements of the Solicitation documents. The resume shall concentrate on
the person’s experience and qualifications as it relates to the requirements for the services.
The SF330 Form and/or resume will suffice.
3.Section 3.3 and Project Verification Form:
Proposer must have provided various utility Underground Consultation Services for projects at
least three (3) Projects over the past five (5) years of similar size, nature and complexity with a
construction amount exceeding $1,000,000 and provide evidence using the Project Verification
Form provided herein.
