A RESOLUTION OF THE CITY COMMISSION OF THE CITY OF
SUNNY ISLES BEACH, FLORIDA, APPROVING THE PROGRAM
GRANT EXTENSION FOR ADDRESS VERIFICATION BETWEEN
THE CITY OF SUNNY ISLES BEACH AND SCHOOL BOARD OF
MIAMI-DADE COUNTY, ATTACHED HERETO AS EXHIBIT "A",
IN SUBSTANTIALLY THE SAME FORM; PROVIDING FOR THE
CITY MANAGER AND THE CITY ATTORNEY WITH THE
AUTHORITY TO DO ALL THINGS NECESSARY TO
EFFECTUATE THIS RESOLUTION; PROVIDING FOR AN
EFFECTIVE DATE.
WHEREAS, the City and the School Board entered into a Program Grant Agreement for
Sunny Isles Beach Address Verification Plan ("Program Grant Agreement") on or about January
9, 2017; and
WHEREAS, the City wishes to extend the Program Grant Agreement until July 31,
2019.
NOW THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF THE
CITY OF SUNNY ISLES BEACH, FLORIDA, AS FOLLOWS:
Section 1. Approving the Extension for Program Grant Agreement. The City Commission
hereby approves the extension for the Program Grant Agreement for Sunny Isles Beach Address
Verification Plan between the City of Sunny Isles Beach and School Board of Miami Dade
County attached hereto as Exhibit "A" in substantially the same form to extend the term of the
Agreement to July 31, 2019.
Section 2. Authorization of City Manager and City Attorney. The City Manager and City
Attorney are hereby authorized to do all things necessary to effectuate the terms of this
Resolution.
Section 3. Effective Date. This Resolution shall become effective upon adoption.

PROGRAM.GRANT EXTENSION
BETWEEN
THE CITY OF SUNNY ISLES BEACH
AND
THE SCHOOL BOARD OF.MIAMI-DADE COUNTY, FLORIDA .-.
This Extension Amendment to the Agreement between the City of Sunny Isles Beach
("City") and The School Board of Miami-Dade County, Florida, ("School Board") as of
the 30 day of June 2018, is made part of the original Agreement ("Agreement") dates
January 9, 2017, between the City and the School Board incorporated herein by
reference.
WHEREAS, the City and the School Board entered into a Program Grant Agreement for
Sunny Isles Beach Address Verification Plan on or about January 9, 2017; and
WHEREAS, the City and the School Board agreed to extend the Term of Agreement for
the 2017 —2018 school year; and
WHEREAS, the City and the School Board agreed to extend the grant end date to
October 1, 2018; and
NOW, THEREFORE, the City and School Board agrees to the following:
The address verification program will continue to be fully funded by the City and shall
extend the address verification plan for the 2018-2019 school year.
All other terms of the above referenced Agreement for Contracted Services entered into
as of the 9th day of January 2017, shall remain in effect. •
IN WITNESS WHEREOF, Parties hereto have caused this Extension Amendment to the
Agreement for the Sunny Isles Beach Address Verification Agreement

RE: Extension of Program Grant Agreement for Address Verification with Miami-
Dade County Public Schools.
Recommendation:
This Resolution is presented for your consideration.
Reasons:
This resolution will extend the Program Grant Agreement ("Agreement") for Address
Verification between the City and the Miami Dade County School District until July 31,
2019. All terms and conditions of the original Agreement dated January 9, 2017 shall
remain in effect.
