Contractor: Envirowaste Services Group, Inc. <br />Contract: E2V99 <br />Evaluation Date: May 21, 2018 <br />Cateaoty Five <br />37502043 <br />MAINTENANCE <br />06!14 <br />Interaction, coordination, and cooperation with Department personnel, traveling public, other <br />Performance <br />contractors, property owners and other Governmental agencies <br />Performance <br />Interaction with Department personnel was outstanding; no complaints from the traveling public or <br />Excellent <br />adjacent property owners were noted, The Contractor handled any issues that arose, notifying the <br />Department of the outcome. Positive feedback from the public was noted. Expectations were often <br />Satisfactory <br />exceeded. <br />so <br />Few, if any, complaints from the traveling public or adjacent property owners were noted. When a <br />Satisfactory <br />concern was expressed, the Contractor was quick to resolve it. Positive interaction with Department <br />0 <br />personnel with some positive feedback from the public was noted. Expectations were always met and <br />Unsatisfactory <br />occasionally exceeded. <br />0 <br />Some complaints from outside parties and Department personnel were noted (mostly minor in nature). <br />Most handled in timely manner. Expectations were not consistently met. <br />Unsatisfactory <br />0 <br />complaints were a <br />Poor <br />Numerous complaints were received about work operations, staging of equipment, and/or poor attitude. <br />The Contractor repeatedly failed to follow instructions; communication and cooperation was inadequate. <br />Poor <br />0 <br />Notes & Comments <br />The Contractor's communication and interactions with the Department were outstanding as any requests made by the <br />Department were responded to in a professional and timely manner. Daily contact was made by the Contractor to ensure <br />all work needs were addressed. Constructive exchanges of information such as unforeseen concerns found in the field by <br />the Contractor was routinely practiced. The Contractor's attitude was always optimistic and courteous. <br />v_aielVEY_0AA <br />Disadvantaged Business Enterprise (DBE) Reporting <br />Performance <br />Contractor's staff was very well qualified and capable to address sufficlent utilization of the Equal <br />Opportunity Compliance (EOC) System application to collect, review, and report any DBE <br />Excellent <br />commitmentsfpaymeets. Periodic checks of the EOC System by Department personnel revealed no <br />issues and no follow up with the Contractor in regard to DBE issues was required. <br />Contractor's staff sufficiently utilized the Equal Opportunity Compliance (EOG) System to adequately <br />Satisfactory <br />collect, review and report DBE commitments/payments. Periodic checks of the EOC System by <br />so <br />Department personnel revealed very few issues and minimal follow up with the Contractor in regard to <br />DBE issues was required. <br />Contractor demonstrated little interest in utilizing the EOC system to collect, review, and report any <br />Unsatisfactory <br />DBE commitments/payments. Requests for the Contractor to utilize the system were repeatedly made <br />0 <br />by Department personnel and follow up with the Contractor was often required. <br />Contractor failed to adequately report DBE commitments/payments. Requests for the Contractor to <br />Poor <br />correct errors within the system were repeatedly made by the Department; excessivelrecurring <br />0 <br />assistance to the Contractor was necessary. <br />N/A <br />N/A (To be used when the Contractor is exempt from DBE reporting requirements.) <br />0 <br />Motes & Comments <br />e a is checks o a Equal Opportunity System y e e men required minima now -ups in regards to <br />the being <br />reporting of DBE commitments.
