<br />CITY OF SUNNY ISLES BEACH |INVITATION TO BID NO. 19-08-01 23 <br /> <br />Section 5 <br />Bid Format <br />5.0 FORMAT <br /> <br />Submittals shall be submitted in duplicate. Submit one (1) original, four (4) copies and one (1) <br />electronic copy of the submittal on USB Drive or CD. All required signatures shall be manual, in <br />blue ink of an authorized representative who has the legal authority to bind the CONTRACTOR in <br />contractual obligations. Each page of the bid should state the name of the CONTRACTOR, the bid <br />number, and the page number. The City reserves the right to request additional data or material <br />to support bid. All material submitted in response to the ITB will become the property of the City. <br /> <br />LABEL EACH SECTION AS NUMBERED <br /> <br />The bid must be in the following format at the time of submittal. <br /> <br />1. Company Information <br /> In response to this Proposal, all Proposers must provide the following: <br /> Name of Agency/Company (including any "Doing Business As" names) <br /> Company Locations <br /> Internet Web Site Address (if any) <br /> Details of Entity Business Structure (Corporation, Partnership, LLC) <br /> Current IRS Form W-9 <br /> Date Founded <br /> Home office address and telephone number, and local address and phone <br />number <br /> List of any outstanding litigation that would threaten the viability of the firm or <br />the performance of this contract <br /> Proof of insurance <br /> Indication of how long it would take to implement service after authorized to <br />begin <br /> <br />2. Qualifications <br />Proposer’s relevant experience, qualifications and past performance <br /> <br /> An explanation of why the bidder is the best qualified to perform the contract <br />and demonstrate its qualifications based on the specs. List all equipment the <br />Contractor’s own for this service. <br /> <br /> A detailed annual schedule/work plan of proposed services. The schedule should <br />include the Proposer's understanding of the issues and tasks of the project at <br />hand and outline a timeframe with estimated man hours for completing each <br />task. <br /> <br /> Indicate license or registration number for each jurisdiction, qualified to practice <br />in. <br />
