RESPONSE TO REQUEST FOR QUALIFICATIONS FOR <br />Continuing Professional Consulting Engineering Services -Environmental Engineering <br />#BID 20-07-01 <br /> <br />11 <br />Employees are required to report all accidents and near-misses to their first-line supervisor. Non-emergency medical <br />evaluation of injuries is mandatory for all employees where such care is requested by the Safety Director, the <br />employee’s supervisor, or other GBTS management personnel; typically this requirement will apply to all injuries <br />requiring more than simple first aid. Additionally, work related injuries may requ ire drug and alcohol screening at the <br />time of the medical evaluation. A Supervisor Accident and Injury report will be completed by the first -line supervisor <br />and submitted to the Safety Director within 24 hours of the incident. <br />Employees may provide suggestions and/or comments on any part of the Health and Safety Program at any time, <br />without fear of reprisal. Employees are encouraged to first discuss any health and safety questions with their <br />immediate supervisor to attempt resolution of the situation at the supervisory level. Employees may contact the <br />Corporate Safety Director directly at (305) 374-8300 in the Miami Lakes office to discuss or report a work-related <br />hazard, regardless of the location. Work-related accidents / injuries must be reported immediately, or at the earliest <br />available time, to the employee’s first-line supervisor, Corporate Safety Director or HR Manager. Failure to report a <br />known work-related hazard or accident / injury is grounds for disciplinary action. <br />The full support of all employees is essential to the effectiveness of this program and the control of work -related <br />injuries. Disregard of the GBTS Health and Safety Program or of safe working practices shall constitute grounds for <br />disciplinary action up to and including termination. It will be the responsibility of the immediate supervisor and / or <br />the Corporate Safety Director to determine the severity of such an incident. If it is determined that the incident <br />occurred due to a lack of training, the employee's supervisor will be responsible for providing the required training <br />prior to the employee's return to active status. <br />Insurance <br />Gallagher Bassett Technical Services currently has in -force a combined General <br />Liability, Contractors Pollution Liability and Professional Liabil ity (E&O) Insurance <br />policy. This policy is written through insurance carrier(s) with “A” ratings by AM <br />Best, Standard and Poors and Moody’s. The policy provides coverage for all <br />claims arising out of GBTS’s professional services, both in its capacity as a <br />professional consultant and as a construction manager of demolition, <br />remediation, abatement and environmental remediation proje cts, including, but <br />not limited to, asbestos, lead, mold, contaminated soils and groundwater, and the excavation, transportation and <br />disposal of hazardous materials. There are no exclusions for microbiologi cal contamination or pandemics. GBTS <br />has a primary insurance policy with limits of $1,000,000 per occurrence and $2,000,000 in the policy aggregate. <br />We also have an umbrella poli cy providing an additional $25,000,000 per occurrence and an additional <br />$25,000,000 in the policy aggregate. This policy cover's GBTS for all work performed throughout the policy year. <br />In addition to the GL, Professional, Pollution, and Umbrella, GBTS also has in-force a $1,000,000 CSL Automobile <br />policy and Worker’s Compensation coverage with limits meeting statutory requirements in all 50 states. These <br />policies are also provided by a carrier with an A+ rating by AM Best. Specialty coverages such as Railroa d <br />Protective Liability, marine, etc. are available. Project specific policies with dedicated limits are also readily <br />available and can be acquired upon request. <br /> <br />Safety <br />GBTS is dedicated to construction site safety for our projects and our personnel.
