n.(1) TITLE AND LOCATION (City and State)(2)YEAR COMPLETED <br />City of Sweetwater Stormwater Pump Station Improvements PROFESSIONAL CONSTRUCTION (if <br />-Phase IV SERVICES aoolicable) <br />Sweetwater, Florida 2011 2014 <br />(3)BRIEF DESCRIPTION (scope, size, cost, etc.) and SPECIFIC ROLE X Check if project performed with current firm <br />Due to repetitive, historical flooding within the 138 acre residential area, Phase IV Stormwater Improvements for <br />the City of Sweetwater received substantial HMGP funding from FEMA. The projects consisted of 130 structures, <br />10,795 linear feet of drainage pipe ranging in size from 15-inch to 72-inch, 1,506 linear feet of 30-inch force main <br />and 2-triplex pumping stations (28 HP). Design fees for the project were $403,151. The $4.09M project began in <br />May 2011 and was completed in January 2014 with no liquidated damages. Work included surveying, utility <br />easement acquisition, utility locating, FEMA grant applications, planning, design, cost estimates, preparation of <br />construction plans and contract documents, permitting, bidding, construction management and limited engineering <br />services during construction. The project was inspected and certified by the City Engineer's office. <br />o. (1)TITLE AND LOCATION (City and State)(2)YEAR COMPLETED <br />Spring Lake Improvement District PROFESSIONAL CONSTRUCTION (if <br />Stormwater Pump Station Rehabilitation SERVICES aoolicable) <br />Sebring, Florida 2015 2018 <br />(3)BRIEF DESCRIPTION (scope, size, cost, etc.) and SPECIFIC ROLE X Check if project performed with current firm <br />Survey for the rehabiliation of a 50-year old stormwater pump station located at the District's eastern boundary <br />service area. Work included replacement of the four (4) stormwater pump 54-inch diameter discharge pipes which <br />were leaking. Additional pump station upgrades included replacement of the electrical control panel, installation of <br />a data flow SCADA system, a new emergency generator with automatic transfer switch (ATS), rehabilitation of the <br />two (2) diesel driven stormwater pumps and replacement of the belt drives of the two (2) electric stormwater pumps. <br />Also the pump station building lights, level controls and security systems were replaced. Lastly, the building interior <br />was painted, the perimeter chain link fence replaced and the existing canal bank was reshaped and rip rap material <br />installed. CAS prepared the record drawings. <br />Total project design cost (including construction management) was $125,000 and total construction cost was <br />$404,477.
