Horizontal Work: Recovered P.N. Points at 1000 foot intervals with intervisibility between points placed away from block <br />corners and out of construction limits. <br />• Control Points Level Run: <br />• Digital Terrain Model (D.T.M.): Performed in areas of topography with the exceptions of topo, areas for M.O.T. and areas on <br />side streets as determined by the Design Project Manager as needed. Under this task we collected additional field data in order <br />to produce a D.T.M. of the additional 100 feet from the point of termination along 42 intersecting streets along the survey route <br />of a previously completed Survey by this firm. <br />• Drainage Survey: Obtain all drainage details information within the DTM AREA, collect ed Rim & Invert elevations, pipe sizes, <br />materials and direction of approximately 80 drainage structures. <br />Dates of Service: 02/2018 to 9/2018 <br /> <br />Miami-Dade Aviation Department. <br />Scope of Service Provided: <br />Performed Location and Existence Verification of Sites located within Opa-Locka Airport, Miami International Airport and Kendal Executive Airport. HC <br />located all sites in question with GPS, HC provided a site sketch showing all gathered information. As part of this task HC developed a standard form <br />to be used for the length of this contract with CDM Smith. <br /> Services have been provided from 07/2015 to 01/2016 <br /> <br />“Flagler County Line’s Phases II and III” located on the square mile between NW 97th Ave & NW 107th Avenue and NW 154th Street & NW <br />170th Street in the City of Hialeah, FL. <br />Scope of Service Provided: <br />Boundary and Topographic Survey <br />Perform a Boundary and Topographic Survey of the parent tract including portions of white rock lake bank and all canal bank a long NW 170th Street <br />Tentative / Preliminary Plat <br />Final Plat <br />Construction Layout of the Entire Project <br />Sketch to Accompany Legal Description for Several Locations <br />As-Built excavation <br />Elevation Certificate for multiple buildings <br />Services have been provided from 08/2015 – On-going <br /> <br />B-17365 - Shorecrest Area Survey within the City of Miami <br />Scope of Service provided <br />Perform Topographic Survey of approximately 45,000 linear feet of road, survey to include the following items: <br />Depict Right of Way information based on the City of Miami Street ATLAS sheets, record plats and property appraisal database including all curren t <br />dedicated areas. <br />Survey limits to extend from Right of Way to Right of Way along the corridor 15 feet into the private properties. <br />Topo survey to extend 100 feet from the end of radius return into the intersecting streets. <br />Locate all above ground features, including site lot lines, trees (Height, caliper, species), House number. <br />Collect Rim elevations, invert elevations, bottom of structure and pipe material. <br />Take elevation shots (Cross section elevations) at 25-foot intervals. <br />• Elevations to be referenced to NGVD29 <br />• Locate all existing ADA ramps and existing driveway approaches. <br />• HC will deliver three signed and sealed copies of the resulting map of survey in addition <br />• Collect Finished Floor elevation of all Buildings within the corridor <br />• Limits of the Survey to extend 15 feet into the private properties along the corridor (when not accessible take elevation shots on <br />the ground) <br />Services have been provided from 06/2018 to 10/2018 <br /> <br />77
