6.4. Requests for Leave and Documentation and Confidentiality: A staff member must <br />provide appropriate advance notice of 15 days of the need for leave, except in cases of <br />imminent danger to the health or safety of the staff member or to a staff member's family or <br />household member. The request must also be accompanied with sufficient documentation of <br />the act of domestic violence. The documentation may include copies of police reports, <br />restraining orders, legal papers to be filed with the court, orders to appear in court, or any other <br />documentation to establish the need for a leave as set out in this policy. All information <br />regarding a staff member's leave will be kept confidential by the Company. The request for <br />leave must be submitted confidentially to the Human Resources Representative, <br />6.5. Definitions: <br />(a) Domestic violence: means any assault, aggravated assault, battery, aggravated <br />battery, sexual assault, sexual battery, stalking, aggravated stalking, kidnapping, false <br />imprisonment, or any criminal offense resulting in physical injury or death of one family or <br />household member by another family or household member. <br />(b) Family or household member: means spouses, former spouses, persons related by <br />blood or marriage, persons who are presently residing together as if a family or who have <br />resided together in the past as if a family, and persons who are parents of a child in <br />common regardless of whether they have been married. With the exception of persons <br />who have a child in common, the family or household members must be currently <br />residing or have in the past resided together in the same single dwelling unit. <br />(c) Victim: means an individual who has been subjected to domestic violence. <br />7. SOCIAL SECURITY <br />By law, employees are covered by Federal Old Age and Survivors' Insurance (Social Security), <br />which provides retirement income, survivors' benefits and medical care on retirement, <br />disablement, or death. The employee's share of the cost for the benefit is deducted from the <br />paycheck. Information on the social security program is available from the administration. <br />8. UNEMPLOYMENT BENEFITS <br />VisualScape Inc. complies with all Federal, state, and local laws, rules and regulations regarding <br />unemployment benefits. Information on the unemployment compensation program is available <br />from the administration. <br />9. WORKERS' COMPENSATION BENEFITS <br />By law, employees are insured for workers' compensation, which provides hospital and medical <br />payments for injury or illness caused on the job, or death benefits to the employee's beneficiary <br />if the employee is killed on the job. Job-related injuries and illnesses must be reported to the <br />company and to the insurer as soon as they occur. The cost of the Workers' Compensation <br />Insurance is paid by the company. Information about this program is available from the <br />administration. <br />Any workers' compensation leave will be run concurrently with Family and Medical Leave. <br />22 <br />
