The Company and its insurance carrier shall not be responsible for the payment of worker's <br />compensation benefits for any injury which arises out of an employee's voluntary participation in <br />any off-duty recreational, social, or athletic activity which is not part of the employee's work- <br />related duties. <br />10. UNIFORMS <br />As an additional benefit and convenience to our employees, the Company provides uniform t - <br />shirts and caps. It is the employees' responsibility to keep uniforms clean and to wear non ripped <br />long pants. <br />11. HEALTH INSURANCE <br />All Full -Time employees are eligible to enroll in the company's Health Insurance Program after 60 <br />days of continuous fulltime employment. Employee's effective day of enrollment is the first of <br />the month following 60 days. <br />Upon eligibility employees may enroll dependents on the Health Insurance Program at their own <br />expense. Information about the Health Insurance Program is available from the Human <br />Resources Department. <br />If an employee wishes to make changes to their selection or enroll in coverage after their initial <br />enrollment date, they will be only be eligible to do so during annual open enrollment. <br />12. DENTAL PLAN <br />All Full -Time employees are eligible to enroll in the company's Dental Program after 60 days of <br />continuous fulltime employment. Employee's effective day of enrollment is the first of the month <br />following 60 days. <br />You will be responsible for the full cost of this plan through payroll deduction.
