CTA Project No. 19-0035-001-01 01025-2 Golden Shores Street Lighting <br />October 12, 2020 SIB Bid No. 20-09-01 <br />of utility poles during construction, necessary for the start of the work, excluding the cost <br />of construction materials. Payment of Mobilization costs exceeding 10% of the total <br />Contract amount will be deferred to substantial completion. <br />Quality Control is a requirement of all and therefore does not have a separate pay item, <br />but is included within this pay item . Payment for performing site restoration and clean - <br />up including, but not limited to pavement markings, painting, finish grading, driveways, <br />sod, landscaping and any miscellaneous restoration work including all labor, materials <br />and equipment required shall be considered as included. <br />C. Item No. 2 - Maintenance of Traffic (M.O.T.) <br />Payment for maintenance of traffic will be made in equal monthly draws dispensed over <br />the duration of the contract totaling to the lump sum price named in the Bid Schedule. <br />Refer to Maintenance of Traffic, Section 01570, and all other references to traffic control <br />in this document and any regulatory requirements. Price shall include preparation and <br />processing of Maintenance of Traffic Plans for permit approval by a Florida Licensed <br />Professional Traffic Engineer. <br />D. Item No. 3 – Remove Existing Light Pole <br />Measurement and Payment shall be based upon each light pole removed and disposed <br />of. Payment will be based upon disconnecting service, removing pole to minimum of 1’ <br />below grade, and full and complete restoration to all disturbed areas. <br />E. Item No. 4 – Sawcut & Remove 2’ Asphalt Strip <br />Measurement and payment of removing existing pavement shall be based upon the linear <br />footage of asphalt strip, assuming 2’ width, removed in accordance with the requirements <br />of the Contract Documents. Payment for asphalt removal will be made at the unit price <br />per LF. Price will constitute full compensation for remov e and disposal of material, <br />restoration of pavement markings, saw cutting, and cleanup of the area disturbed. <br />F. Item No. 5 – Service Connection <br />Measurement and payment shall be made based upon each service connection provided <br />for lighting circuits. Payment will be based upon connecting service, including <br />coordination with FPL and related equipment and material not otherwise provided by FPL. <br />This pay item is established as an allowance. This item may or may not be used either <br />in part or in whole. Disbursement of fees is entirely at the discretion of the Engineer/City <br />and must be ordered by the Engineer and approved by the CITY. <br />G. Item No. 6 – Conduit Installed in Existing Pavement <br />Measurement and payment of conduit installation shall be based upon the linear footage <br />of conduit installed in accordance with the requirements of the Contract Documents. <br />Payment for conduit installation will be made at the unit price per LF. Price will constitute <br />full compensation for disposal of material, full and complete restoration, and cleanup of <br />the area disturbed. <br />H. Item No. 7 – Conduit Installed in Earth <br />Measurement and payment of conduit installation shall be based upon the linear footage <br />of conduit installed in accordance with the requirements of the Contract Documents. <br />Payment for conduit installation will be made at the unit price per LF.
