Project requirements include the collection of field traffic and travel data and the preparation of traffic, access management, <br />engineering reports and environmental documents; a Section 4(f) document; and preliminary plans for this heavily urbanized arterial that <br />services major business and residential communities. Improvements include a new higher, double-leaf Bascule Bridge; intersection <br />reconstructions; and access management modifications. Extensive public involvement efforts are being done as part of this study. <br />c. <br />(1)TITLE AND LOCATION (City and State)Meridian Avenue over Collins Canal - FDOT D6 YEARS COMPLETED <br />PROFESSIONAL SERVICES CONSTRUCTION <br />2014 - 2019 <br />(3) BRIEF DESCRIPTION (Brief Scope, Size, Cost, etc.) AND SPECIFIC ROLE [ ] Check if project performed with current firm <br />Project Manager for the bridge replacement design services for the Meridian Avenue Bridge over Collins Canal. Improvements included: <br />roadway, drainage, signalization, lighting and maintenance of traffic. Specifically, the project included the following: (1) Replacement of the <br />bridge over Collins Canal; (2) Reconstruction of the south approach Meridian Avenue roadway. (3) Aesthetic/streetscape improvements which <br />included: pigmented cement pavers for the roadbed and bridge, stone pavers for the sidewalk, stone veneer for the vertical surfaces of the <br />bridge, and decorative lighting; (4) Drainage improvements which included the installation of a drainage well to dispose of the contaminated <br />water prior to discharge into the Collins Canal and (5) Signalization and non-decorative roadway lighting. <br />d. <br />(1) TITLE AND LOCATION (City and State)PD&E Study for Venetian Causeway – FDOT District 6 YEARS COMPLETED <br />PROFESSIONAL SERVICES <br />2012-Ongoing <br /> CONSTRUCTION <br />N/A <br />(3) BRIEF DESCRIPTION (Brief Scope, Size, Cost, etc.) AND SPECIFIC ROLE [ ] Check if project performed with current firm <br />The project requirements include the inspection of the bridges, engineering reports, environmental documents, a Section 4(f) document and <br />preliminary plans. Project Manager and Structural Engineer responsible for conceptual Structural Analysis and design of the ten low level fixed <br />span bridges, preparation of Preliminary Engineering Report study documents, public involvement workshops and activities, and <br />coordination with sub-consultants and agencies. The Venetian Causeway bridges (ten low-level fixed spans and two movable bascules) from <br />North Bayshore Drive in Miami to Purdy Avenue in Miami Beach were originally built in 1926 and have been designated as historic landmarks by <br />the City of Miami and City of Miami Beach; they are also listed on the National Register of Historic Places (NRHP). Extensive public <br />involvement efforts are being done as part of this study. MDX controls 31 miles of expressways and a $500+ <br />million work program. Mr. Crooks’ primary responsibility included the management and oversight of the GEC. Since 2003, Mr. Crooks has been <br />involved in all phases of the development of the RFPs and concept plans for these projects and had the added benefit of overseeing the <br />construction and successful implementation of the projects in a timely and cost-effective manner.
