Ms. Genesis Cuevas <br />CTA Proposal No. 2018-UO3.383 (Revised) <br />July 18, 2019 <br />Paee 2 <br />Phase 2 — Design and Permitting: <br />In Phase 2 proposed plans are produced. The information collected in Phase 1 will be verified and used <br />for the proposed design. FP&L and other utility providers will prepare designs for their individual <br />systems. The CTA will coordinate the individual designs into a cohesive set of plans with detailed <br />installation requirements. In addition to plans, this phase includes coordination with various permitting <br />agencies. <br />At the conclusion of this phase, all design issues and requirements should be identified, construction <br />documents, coordinating and addressing utility concerns are prepared, easement requirements are <br />acknowledged for procurement, and approvals from appropriate permit agencies obtained. In addition, <br />the project has been circulated among stakeholders and an estimated cost of improvements provided. <br />Phase 3 — Bidding & Construction: <br />Phase 3 involves Construction Phase services. It includes preparation of bid documents, facilitating and <br />making recommendation for award, CEI services, close-out and certifications. Periodic inspections are <br />expected and will be supported with project management to avoid problems with transfer and delivery. <br />After construction of underground infrastructure is complete, the transfer of overhead utilities to the <br />underground infrastructure and switchover of individual services will commence. These services are <br />not included in this proposal as they have yet to be defined. <br />Our scope of services is as follows: <br />SCOPE OF SERVICES <br />I. Phase 1- Proiect Assessment <br />1.1.a. Boundary & Topographic Survey of Pipeline Route (CTA Task No. 110S0) <br />Prepare a boundary & topographic survey of portions of the following multiple <br />roadways and swales within the Golden Shores Neighborhood in the City of Sunny Isles <br />Beach, Florida. The project limits are delineated on the attached Exhibit 'A'. The survey <br />will include locations within the full right-of-way of the roads listed below and all <br />aboveground improvements.
