(CTA) is currently providing design <br />services for the above referenced project. Within the process of design, additional <br />scope and extended services resulted in efforts exceeding the estimated budget <br />amounts. The additional effort included extended coordination with utilities (FPL, <br />AT&T, and Atlantic Broadband), additional sidewalk improvements along Atlantic <br />Ave, and provisions for Urban Trail along Collins Avenue north and south of 189' <br />Street. The reference to tasks in description of services refers to tasks from <br />previous proposal. <br />Our scope of services is as follows: <br />SCOPE OF SERVICES <br />I. Extended Services for Task 2.6, Prepare Construction Documents <br />The budget for this task assumed the integration of utility designs to be <br />complete with the preparation of Design Development plans (Task 2.3). <br />Construction Documents were prepared using the information provided by <br />the utilities. However, due to deficiencies, designs provided by all three of <br />the utilities were revised, causing CTA to revise a significant portion of the <br />Construction Documents already prepared. Previously approved task <br />amount was $57,800. With approval of this proposal total for task will be <br />$92,600. <br />LumpSum..................................................................... $34,800.00 <br />II. Additional Sidewalk Improvements, Atlantic Avenue <br />Subsequent to award of design services, the City requested a sidewalk be <br />added along the east right-of-way of Atlantic Avenue extending from 185th <br />Street to 191St Terrace. This additional task includes plan revisions <br />accounting for demolition, grading, drainage, and driveway improvements <br />associated with the proposed concrete walk. <br />Lump Sum..................................................................... $27,000.00 <br />
