<br />City of Sunny Isles Beach |Request for Qualifications No. 22-06-01 12 <br /> <br />2.8.2 Professional Errors & Omissions Liability: Contractor agrees to maintain Professional <br />Error’s & Omissions Liability at a limit of liability not less than One Million ($1,000,000) <br />Each Occurrence, Two Million ($2,000,000) Annual Aggregate. The Contractor agrees <br />the policy shall include a minimum three (3) year Discovery (tail) reporting period, and <br />a Retroactive Date that equals or precedes the effective of the Contract, or the <br />performance of services hereunder. The Contractor agrees the Self-Insured Retention <br />shall not exceed $25,000. This coverage may be provided on a Per-Project Basis. <br /> <br /> 2.8.3 Business Automobile Liability <br />Business Automobile Liability with minimum limits of Five Million Dollars <br />($5,000,000.00) per occurrence combined single limit for Bodily Injury Liability and <br />Property Damage Liability. Coverage must be afforded on a form no more restrictive <br />than the latest edition of the Business Automobile Liability policy, without restrictive <br />endorsements, as filed by the Insurance Services Office, and must include: <br /> <br />• Owned Vehicles; <br />• Hired and Non-Owned Vehicles; <br />• Employers’ Non-City ship. <br /> <br />Before starting the Services, the vendor will file and make sure that all certificates of <br />insurance required by this document and by the Contract are in the City’s possession. <br />These certificates shall contain a provision that the coverage afforded under the <br />policies will not be canceled or materially changed until at least thirty (30) days prior <br />written notice has been given to the City by certified mail. <br /> <br />a. Umbrella Insurance <br />Firm shall be required to purchase, maintain, and keep in full force, effect, and good <br />standing, Umbrella Liability Insurance above the primary commercial general liability, <br />automobile liability, and employers' liability policies required herein. The limit shall <br />not be less than One Million Dollars ($1,000,000.00) each occurrence and annual <br />aggregate per occurrence during the initial and any renewal term. <br /> <br />b. Worker’s Compensation Insurance <br />Worker's Compensation Insurance for statutory obligations imposed by Worker's <br />Compensation or Occupational Disease Laws, including, where applicable, the United <br />States Longshoremen's and Harbor Worker's Act, the Federal Employers’ Liability Act <br />and the Homes Act.
