Copy of firm’s Articles of Incorporation listing the approved signatories of the corporation. <br />b. Copy of a resolution listing the members of staff as authorized signatories for the firm. <br />c. Letter from a corporate officer listing the members of staff that are authorized signatories for <br />the firm. Non applicable, there is no other member authorized to sign. <br /> <br />6. Litigation History <br /> <br />The company does not have any litigation or arbitration history since the company was created. <br /> <br />7. Forms and Attachments <br /> <br />See attached the following documents on the next pages: <br /> <br />Enclosed Forms; <br />Respondent must complete, sign as required, and submit the Addenda and all forms; and <br />Firm’s Current Certificate(s) of Insurance. <br /> <br />8. Exceptions <br /> <br />1) Exceptions to the bid are the permit fees, which are not included in the Grand Total of the <br />bid form, and they are as an allowance. <br />2) Janitor sink and floor drain covers are not described in plans, the allowance indicate the <br />amounts for each material. <br />3) Parking fees are not included, it was mentioned during the prebid meeting that the City <br />will provide parking passes for our laborers and subcontractors. <br />4) Working after hours, weekends, and holidays is not included. If the project is delayed by <br />us, or if any activity needs to be performed to coordinate better between subcontractors, <br />the contractor will determine if working after hours, weekends, or holidays is necessary, <br />upon coordination and approval from the City. <br />5) New Landscaping and sod are excluded. <br />6) Camera inspection for sanitary lines is excluded. <br />7) MOT shop drawings and permits is excluded. <br />8) Bacteriological tests and backflow certifications are excluded.
