<br />City of Sunny Isles Beach |Invitation to Bid No. 23-02-01 25 <br /> <br />State Certificate, the respondent must provide one of the following forms of evidence of Signatory <br />Authority with its response: <br />a. A copy of firm’s Articles of Incorporation listing the approved signatories of the <br />corporation; <br /> <br />b. A copy of a resolution listing the members of staff as authorized signatories for the firm; <br />and <br /> <br />c. A letter from a corporate officer listing the members of staff that are authorized signatories <br />for the firm. <br /> <br /> 6. Litigation History <br /> <br />CONTRACTOR shall provide a summary of any litigation or arbitration that the CONTRACTOR, its <br />parent company or its subsidiaries have been engaged in or are currently engaged in, during the <br />past five (5) years against or involving (1) any public entity in Florida for any amount (2) any private <br />entity for an amount greater than $100,000. The summary shall state the nature of the litigation <br />or arbitration, a brief description of the case, the outcome or projected outcome, and the <br />monetary amounts involved. The CITY may disqualify any CONTRACTOR it determines to be <br />excessively litigious. <br /> <br />7. Forms and Attachments <br /> <br />Enclosed Forms; <br />Respondent must complete, sign as required, and submit the Addenda and all forms; and <br />Firm’s Current Certificate(s) of Insurance. <br /> <br />8.
