SECTION 4 <br />BID RESPONSE FORMAT <br />4.0 FORMAT <br />'14 Or SOO W <br />Submittals shall be submitted in duplicate. Submit one (1) original, three (3) copies and one (1) <br />electronic copy of the submittal on USB Drive. All required signatures shall be manual, in blue ink <br />of an authorized representative who has the legal authority to bind the CONTRACTOR in contractual <br />obligations. Each page of the bid should state the name of the CONTRACTOR, the bid number, and <br />the page number. The City reserves the right to request additional data or material to support bid. <br />All material submitted in response to the ITB will become the property of the City. <br />LABEL EACH SECTION AS NUMBERED <br />The proposal must be in the following format. All submittals shall contain no more than fifteen <br />(15) pages specifically addressing each criteria below: <br />1. Company Information <br />In response to this Proposal, all Contractors must provide the following: <br />■ Name of Agency/Company (including any "Doing Business As" names) <br />■ Company Locations <br />■ Internet Web Site Address (if any) <br />■ Office address and telephone number, email address <br />■ Proof of insurance <br />■ Proposer must submit a copy of Florida Division of Corporations Sunbiz report <br />with your company registered as active. <br />■ W9 <br />2. Qualifications <br />Proposer's relevant experience, qualifications and past performance: <br />■ An explanation of why the Contractor is the best qualified to perform the <br />contract and demonstrate its qualifications including an item -by -item disclosure <br />outlining how the Contractor meets or exceeds the requirements of this ITB. <br />Provide a brief introduction letter highlighting the overall experience and <br />qualifications of the Respondent with respect to the services requested under this <br />Solicitation. Project list demonstrating this minimum experience and shall include, <br />at a minimum: the project, date, base contract amount, number and amount of <br />change orders, if any, the name, address, and phone number of the owner or <br />owner's representative familiar with the work. Specifically list any projects, either <br />complete or underway, that are located in Miami -Dade, Broward or Palm Beach, <br />Florida Counties. <br />■ Your firm must address the Minimum Qualification Requirements per <br />Section 2.2 of ITB. <br />City of Sunny Isles Beach I ITB 22-12-02 <br />
