Submit one (1) original, four (4) copies and one (1) <br />electronic copy of the submittal on USB Drive. All required signatures shall be manual, in blue ink <br />of an authorized representative who has the legal authority to bind the CONTRACTOR in <br />contractual obligations. Each page of the bid should state the name of the CONTRACTOR, the bid <br />number, and the page number. The City reserves the right to request additional data or material <br />to support bid. All material submitted in response to the ITB will become the property of the City. <br /> <br />LABEL EACH SECTION AS NUMBERED <br /> <br />The proposal must be in the following format specifically addressing each criterion below: <br /> <br />1. Company Information <br />In response to this Proposal, all Contractors must provide the following: <br /> Name of Agency/Company (including any "Doing Business As" names) <br /> Company Locations <br /> Internet Web Site Address (if any) <br /> Office address and telephone number, email address <br /> Proof of insurance <br /> Bidder must submit a copy of Florida Division of Corporations Sunbiz report with <br />your company registered as active. <br /> W9 <br /> Bonding Capacity Letter <br /> <br />2. Qualifications <br />Proposer’s relevant experience, qualifications and past performance <br /> <br /> An explanation of why the Contractor is the best qualified to perform the <br />contract and demonstrate its qualifications including an item-by-item disclosure <br />outlining how the Contractor meets or exceeds the requirements of this ITB. <br /> <br /> Provide a brief introduction letter highlighting the overall experience and <br />qualifications of the Respondent with respect to the services requested under this <br />Solicitation. Project list demonstrating this minimum experience and shall <br />include, at a minimum: the project, date of completion, base contract amount, <br />number and amount of change orders, if any, the name, address, and phone <br />number of the owner or owner’s representative familiar with the work. <br />Specifically list any projects, either complete or underway, that are located in <br />Miami-Dade, Broward or Palm Beach, Florida Counties. <br /> <br /> Bidder must address the Minimum Qualification Requirements A – D, per <br />Section 2.2 of ITB. <br />
