17 <br />safety of the open trench, contact your superintendent before encountering any <br />conditions which you bel ieve to be unsafe. <br /> <br />C. When the excavation operation of a trench begins to approach the existing <br />underground utilities, the precise location of these utilities shall be determined with <br />caution and with primary attention focused on the safety of those involved in the <br />excavation operation. <br /> <br />D. While an excavation remains open, the existing utilities shall be protected or <br />supported or moved as necessary in order to safeguard all employees involved in the <br />excavation operations, or pipe installations or backfill operations. <br /> <br />Access/egress to trench <br /> <br />A. In order to enter (access) or leave (egress) a trench, a stairway, ladder, ramp or other <br />safe means shall be utilized. A stairway, ladder, ramp or other safe means of entrance and <br />egress shall be provided in all trench excavations that are four- (4) feet (1.22m) or more in <br />depth. The stairway, ladder, ramp or other safe means of egress shall be located so that no <br />employee in the trench is required to travel more than 25 feet in order to reach the point of <br />egress. In the event that a ladder is not the safe means of egress, a ramp or stairway or <br />other safe means of egress shall be approved by the Project Manager before it is utilized. <br /> <br />B. In the event a structural ramp is utilized for egress, the ramp shall be designed <br />by a Competent Person qualifi ed in structural design and shall be constructed in <br />accordance with the design. <br /> <br />C. In the event a structural ramp is utilized for egress and the ramp is designed with <br />two or more structural members, the structural members shall be connected with cross <br />member s. These cross members shall prevent displacement. <br /> <br />D. In the event a structural ramp is utilized for egress and the ramp is designed with <br />two or more structural members, the structural members shall be of uniform thickne ss. <br />Any lack of uniformity may cause unsafe conditions or possible displacement. <br /> <br />E. In the event that the ramp is designed with fixtures to connect the structural <br />members, the fixtures or connections shall be attached to the bottom of the ramp in a <br />manner that prevents tripping.
