C R A I G A S M I T H. C O M
As you are aware, CAS is near completion of its design of the 174th Street project.
During the design process we have made several revisions and adjustments to the
plans per the City's request. Recently, while working our way through the permitting
process, we were advised of another project which will likely conflict with the scope of
the project we are designing and requires modifications to our design plans.
At the request of the City we have continued and made the necessary adjustments in
order to minimize the impact to the project schedule.
Additional services which have been required include survey, engineering and
permitting. The following is a summary description of the services and cost associated
with the services:
SURVEY
Physically locate all above ground visible improvements to a minimum of 10
feet beyond the Right of Way of 174th Street within the overall scope limits of
the project. This also included trees and landscaped areas, and additional
vertical information in driveways, as well as underground utility locations and
ground penetrating radar (GPR) points as field marked by CAS Utility
Locates Department. Additional plan view elevations were shown as relative
to North American Vertical Datum of 1988 (NAVD '88) at pertinent points for
facilitation of Engineering Design. Control points both in the vertical and
horizontal plane were re- checked and the information was provided to Calvin
Giordano & Associates for assurance of congruency between projects in the
design phase. The base map prepared for the original project was updated
and provided to the Engineering Department as well as an updated Map of
Specific Purpose Survey for submittal to entities of interest.
Fee
ATTACHMENT "A"
$5,911.34
9 5 4. 7 8 2. 8 2 2 2 - - - - -- 5 6 1. 7 9 1. 9 2 8 0 P h o n e -- 5 6 1. 3 1 4. 4 4 4 5 -- 3 0 5. 4 6 1. 4 4 1 0
9 5 4. 7 8 6. 8 9 2 7 F a x 5 6 1. 7 9 1. 9 8 1 8
