All required signatures shall be manual <br />by an authorized representative who has the legal authority to bind the CONTRACTOR in <br />contractual obligations. Each page of the bid should state the name of the CONTRACTOR, the bid <br />number, and the page number. The City reserves the right to request additional data or material <br />to support bid. All material submitted in response to the ITB will become the property of the City. <br /> <br />LABEL EACH SECTION AS NUMBERED <br /> <br />Minimum Qualification Requirements (MQRS) <br /> <br />Bidders must read the MQRs first to ensure you meet these requirements in order to provide <br /> a response to this ITB. Bidders that do not meet all the MQRs stated will be determined non- <br /> responsive and disqualified from the evaluation process and will not be considered. Bidder <br /> must provide all MQRs as requested, in addition to the information requested below. <br /> <br />The proposal must be in the following format specifically addressing each criterion below: <br /> <br />1. Company Information <br />In response to this Proposal, all Contractors must provide the following: <br /> Name of Agency/Company (including any "Doing Business As" names) <br /> Company Locations <br /> Internet Web Site Address (if any) <br /> Office address and telephone number, email address <br /> Proof of insurance <br /> Bidder must submit a copy of Florida Division of Corporations Sunbiz report with <br />your company registered as active. <br /> W9 <br /> Bonding Capacity Letter <br /> <br />2. Qualifications <br />Proposer’s relevant experience, qualifications and past performance <br /> <br /> An explanation of why the Contractor is the best qualified to perform the contract <br />and demonstrate its qualifications including an item-by-item disclosure outlining <br />how the Contractor meets or exceeds the requirements of this ITB. <br /> <br /> Provide a brief introduction letter highlighting the overall experience and <br />qualifications of the Contractor with respect to the services requested under this <br />Solicitation. Project list demonstrating this minimum experience and shall <br />include, at a minimum: the project, date of completion, base contract amount, <br />number and amount of change orders, if any, the name, address, and phone
