THE CITY OF SUNNY ISLES BEACH <br />PROJECT NO. 21-1011 <br />BUILDING DEPARTMENT RELOCATION TO ANNEX <br /> <br />Project No. 21-1011 November 2024 028200 Asbestos Abatement-16 <br />ATTACHMENT A <br />ASBESTOS COMPLIANCE WORK PLAN OUTLINE <br /> <br /> <br />In accordance LBNL’s Asbestos Abatement Specifications, the Subcontractor is required to prepare a writ- <br />ten (typed or word-processed), site-specific Asbestos Work Plan, and submit to the University prior to <br />start of work. This plan is required for the Subcontractor to meet OSHA requirements as well as the Pro- <br />ject Specifications, and shall describe work procedures and control methods that will protect LBNL facili- <br />ties and staff. <br /> <br />The Subcontractor shall prepare the Asbestos Work Plan to include, at a minimum, the following infor- <br />mation: <br /> <br />I. Location of Work: <br /> <br /> The work to be completed under this Asbestos Work Plan will be completed at: <br /> Lawrence Berkeley National Laboratory <br /> One Cyclotron Road <br /> (Building name) <br /> (Location within building) <br /> <br /> Previous hazardous materials inspections or surveys have found that asbestos-containing <br />materials are present at the following locations: <br /> <br /> (list all materials and locations, to assure LBNL that the Subcontractor is aware of all <br />hazardous materials locations) <br /> <br /> The presence of asbestos represents a hazard to workers who may disturb these materials during <br />the course of this work. <br /> <br />II. Description of Work: <br /> <br /> Describe the anticipated work scope, including: <br /> <br /> A. Removal (list materials and locations) <br /> B. Encapsulation (list materials and locations) <br /> C. Repair (list materials and locations) <br /> D. Decontamination (list materials and locations) <br /> E. Any other activities that will or may result in worker exposures to asbestos. <br /> <br />III. Schedule: <br /> <br />A. General <br />Start Date: <br />Completion Date: <br /> <br />The competent person [ ], will conduct worksite visual inspections on a daily basis, <br />or more often as necessary. <br /> <br />B.
