Product Data: Submit data on joint devices, attachment accessories, admixtures, form <br />release agents and bonding agent. <br /> <br />C. Material Certifications: For each item listed, submit information indicated. <br /> <br />1. Coarse and Fine Aggregate <br /> <br />a. Producer Name. <br />b. Quarry Location. <br />c. Contact Person and Phone Number. <br />d. Material Test Reports: From a qualified testing agency, indicating <br />compliance with requirements. <br /> <br />
