The Bidders must provide a copy <br />of the State Certificate of good standing listing the officers of the company. If the signatory <br />is not one of the officers listed on the State Certificate, the Bidder must provide one of the <br />following forms of evidence of Signatory Authority with its response: <br /> <br />a. A copy of firm’s Articles of Incorporation listing the approved signatories of the <br />corporation; <br /> <br />b. A copy of a resolution listing the members of staff as authorized signatories for the <br />Bidder; and <br /> <br />c. A letter from a corporate officer listing the members of staff that are authorized <br />signatories for the Bidder. <br /> <br /> 6. Litigation History <br /> <br />Bidder shall provide a summary of any litigation or arbitration that the Bidder, its parent <br />company or its subsidiaries have been engaged in or are currently engaged in, during the <br />past five (5) years against or involving (1) any public entity in Florida for any amount (2) <br />any private entity for an amount greater than $100,000. The summary shall state the <br />nature of the litigation or arbitration, a brief description of the case, the outcome or <br />projected outcome, and the monetary amounts involved. The City may disqualify any <br />Bidder it determines to be excessively litigious. <br /> <br />7. Forms and Attachments <br /> <br />Enclosed Forms; <br />Bidders must complete, sign as required, and submit the Addenda and all forms; and <br />Bidders’ Current Certificate(s) of Insurance. <br /> <br />8.
