<br />CITY OF SUNNY ISLES BEACH |REQUEST FOR PROPOSAL NO. 14-11-02 11 <br /> <br /> <br /> <br />2.8 BID BOND <br />The City of Sunny Isles Beach shall require that every Proposer furnish a Bid Bond in the amount <br />of 5% of the total Proposal Price. <br /> <br /> <br />2.9 DELIVERY: <br />Intentionally Omitted <br /> <br /> <br />2.10 INSURANCE: <br />Comprehensive General Liability with primary limits of $1,000,000 per occurrence/$2,000,000 in <br />the annual aggregate and an additional Excess Umbrella limit of $1,000,000 per occurrence and in <br />the aggregate. Coverage must be afforded on a form no more restrictive than the latest edition of <br />the Comprehensive General Liability policy, without restrictive endorsements, as filed by the <br />Insurance Services Office, and must include: <br /> <br />o Premises and Ongoing Completed Operations – on a primary and non-contributory basis including <br />waiver of subrogation on behalf of the City of Sunny Isles Beach. <br />o Independent Contractors <br />o Products and/or Completed Operations Hazard <br />o Broad Form Property Damage <br />o Broad Form Contractual Coverage applicable to this specific Contract, including any hold <br /> Harmless and/or indemnification agreement. <br />o Personal Injury Coverage with Employee and Contractual Exclusions removed, with minimum <br />limits of coverage equal to those required for Bodily Injury Liability and Property Damage Liability. <br /> <br />At the Contractor’s expense, Workers’ Compensation will be provided in accordance with Florida <br />Statute including Employers’ Liability with separate Employer Liability limits of One Million Dollars <br />(1,000,000.00). <br /> <br />Business Automobile Liability with minimum limits of One Million Dollars (1,000,000.00) per <br />occurrence combined single limit for Bodily Injury Liability and Property Damage Liability. Coverage <br />must be afforded on a form no more restrictive than the latest edition of the Business Automobile <br />Liability policy, without restrictive endorsements, as filed by the Insurance Services Office, and <br />must include: <br /> <br />o Owned Vehicles; <br />o Hired and Non-Owned Vehicles; <br />o Employers’ Non-Ownership. <br />
