CO R PO RATECOACHES <br /> Florida's Leading Transportation Provider <br /> TAB 11- ADDITIONAL INFORMATION I DOCUMENTS <br /> Pre-Trip inspections <br /> All Inspections performed by drivers, fleet manager and his staff comply with FMCSR's and are performed daily, <br /> weekly, and monthly. See Section 4.4.1.14.1 for criteria <br /> System for handling driver write-ups and subsequent repairs <br /> We have a driver communication board, where a driver will list a critical repair, including description and bus number. <br /> This is in addition to the driver turning in a Daily Motor Vehicle Report. This report is entered into system. Mechanics, <br /> Safety Office and Dispatch have access to this system, and will place a vehicle out of service until the safety sensitive <br /> repair is made.The fleet manager has to sign off on the repair,then finally the driver who reported the problem has to <br /> also sign off. <br /> See 4.4.1.14.8 <br /> Vehicle grounding criteria (referred to in Industry as Out of Service Criteria) <br /> Vehicle Out of Service Criteria: Corporate Coaches will declare "out of service" any motor vehicle that by reason of its <br /> mechanical condition or loading would likely cause an accident or a breakdown or does not meet the FMCSA criteria for <br /> a vehicle that is safe to operate. An "out of service" sticker shall be used to mark the vehicle "out of service". Corporate <br /> Coaches will not require or permit any person to operate any motor vehicle marked "out of service" until all repairs by <br /> the notice have been satisfactorily completed. Additionally, we have our own criteria of what an Out of Service vehicle <br /> is, which is above and beyond the FMCSA criteria. <br /> Maintenance record keeping <br /> Maintenance Record Keeping: This is done with maintenance software that tie in to the Dispatch software. Drivers <br /> write up complaints, a service writer inputs into system, and writes work orders for the mechanics. On a daily basis, <br /> mechanics will be assigned a certain number of Preventive items and Repair orders. Once completed, the supervisor <br /> inspects the work and sign off, then the driver has to also sign off in order to finalize the work order. The system keeps <br /> track of all work and parts that have been used on each vehicle. Then the hard copy gets filed in the maintenance office. <br /> Corporate Coaches has adopted and adheres to Federal Motor Carrier Safety Regulations Part 396.21. All inspections <br /> and maintenance records shall: <br /> • Identify the individual performing the inspection. <br /> • Identify Corporate Coached as the carrier operating the vehicle. <br /> • Identify the date of the inspection. <br /> • Identify the vehicle inspected. <br /> • Identify the vehicle components inspected. <br /> • Certify the accuracy and completeness of the inspection as complying with the requirements of this <br /> part. <br /> Corporate Coaches Inc. 14500 South State Road 7 I Ft.
