'• c' . • f„l Pro Of 5u Section 5 PROPOSAL SUBMITTAL FORM The undersigned Proposer agrees, if this Proposal is accepted, to enter into an agreement with the City of Sunny Isles Beach to perform and furnish all Services as specified or indicated in the Contract Documents for the Contract Price and within the Contract Time indicated in this Proposal and in accordance with the terms and conditions of the Contract Documents. The Proposer accepts all of the terms and conditions of the Request for Proposal. This Proposal will remain subject to acceptance for 90 days after the day of Proposal opening. The Proposers agrees to sign and submit the Agreement with the other documents required by the RFP within ten days after the date of the City's Notice of Award. In submitting this package, the Proposer represents: The Proposer acknowledges the City has the right to reject any or all Proposals, to accept any Proposal, and to waive any informality or irregularity in the Proposals received. The Proposer has familiarized himself/herself with the nature and extent of the Contract ® Documents, Work, locality, and all local conditions and law and regulations that in any manner may affect cost, progress, performance, or furnishing of the Work. • The Proposer shall guarantee all of the Work performed under this Agreement to be done in accordance with the RFP specifications and related documents including the established schedule for implementation. • The Proposer shall obtain and maintain proper insurances and furnish proof of insurance. • This Proposal is genuine and not made in the interest of or on behalf of any undisclosed person, firm or corporation and is not submitted in conformity with any agreement or rules of any group, association, organization, or corporation; the Proposer has not directly or indirectly induced or solicited any other Proposers to submit a false or sham package; the Proposer has not solicited or induced any person, firm or corporation to refrain from submitting a proposal;; and Proposer has not sought by collusion to obtain for itself any advantage over any other Proposers or over the City.
