<br />Ronda Municipal Insurance Trust <br /> <br />Medical Master Plan of Beneflts <br /> <br />Physical Therapist means a person who is duly registered or licensed by the state in <br />which such person is engaged in the practice of physical therapy and who is a <br />member of the American Physical Therapist's Association. <br /> <br />Physical Therapy means the diagnosis, treatment, prevention, or rehabilitation of <br />any injury, disease, or other health condition, including the use of apparatus and <br />equipment directly related thereto, by the use of physical, chemical, and other <br />properties of air; electricity; exercise; massage; radiant energy,' including <br />ultraviolet, visible or infrared rays; ultrasound; or water; or by the use of <br />acupuncture or tests of neuromuscular functions; provided such diagnosis, <br />treatment, prevention, or rehabilitation is performed pursuant to minimum <br />criteria and standards of practice established by a statutorily created board that <br />primarily consists of physical therapists and/ or physicians and pursuant to a <br />written plan of treatment prescribed and approved by a physician. <br /> <br />Physician means a doctor of medicine (M.D.) or doctor of osteopathy (D.O.) legally <br />qualified to practice medicine and perform surgery at the time and place the <br />service is rendered. For services covered under this Plan, doctors of dental <br />surgery (D.D.S.), doctors of podiatry (D.P.M,), and doctors of chiropractic (D.C.), <br />when acting within the scope of their licenses, are deemed to be physicians. <br /> <br />Plan means this Master Plan of Benefits, including any Schedule of Benefits attached <br />hereto. <br /> <br />Pre-existing Condition means any condition, physical or mental, for which medical <br />advice, diagnosis, care or treatment was recommended or received within the <br />twelve (12) month period ending on the enrollment date. <br /> <br />Preventative Care means services and supplies ordered and/ or provided by or under <br />the direction of a physician for which there is no medical diagnosis or does not <br />seek to diagnose, treat, or cure a sickness or injury. <br /> <br />Reasonable Fee means the benefit allowances as determined by the Trust for all <br />eligible expenses incurred by a participant. The basis will be the relative value <br />studies and schedules utilized and evaluated by the Trust. The benefit <br />allowances utilized by the Trust are determined by studies of charges for similar <br />benefits within a common geographical area. These studies are used to develop <br />benefit value schedules that are updated on a routine basis. <br /> <br />Ree:istered Graduate Nurse or Licensed Practical Nurse means a person duly <br />licensed by the state in which such person is engaged in the practice of nursing. <br /> <br />Rehabilitative Services means health care services for the purpose of which is to <br />restore functional defects. <br /> <br />Routine Care means services and supplies ordered and/ or provided by or under the <br />direction of a physician for the purpose of the diagnosis, treatment, or cure of a <br />sickness or injury. <br /> <br />Second Surgical Opinion means the second opinion contained in a written statement <br />on the necessity for the performance of a covered surgical operation given by a <br />board-certified specialist who, by the nature of the physician's specialty, qualifies <br />the physician to consider the surgical opinion being proposed and who is not <br />associated With the physician initially recommending the surgical operation. <br /> <br />· .Unless otherwise stated In the Schedule of Benefits (7/98) <br /> <br />Page 5 <br />
